Mrs. Zhang suffered from stagnant qi mechanism, with symptoms resembling panting but not quite so, resembling rebellious qi but not quite so, resembling sighing but not quite so, resembling deficiency-induced rapid breathing but not quite so, resembling shortness of breath but not quite so, resembling chest oppression but not quite so. Her face was red and she experienced dizziness. She had no appetite and could not lie down. Treatments for tonifying deficiency, clearing fire, moving qi, and resolving phlegm were ineffective. Wang Mengying diagnosed her and said, "This is a minor ailment; it will heal within ten days. However, you must control your anger; this is my only instruction." He prescribed a large dose consisting of Huanglian (Coptidis Rhizoma), Huangqin (Scutellariae Radix), Zhizi (Gardeniae Fructus), Chuanlianzi (Meliae Semen), Biejia (Testudinis Carapax et Plastrum), Lingyangjiao (Saigae Tataricae Cornu), Xuanfuhua (Inulae Flos), Zhesishi (Hematitum), Haizhe (Metereorites), and Didou (Dioscoreae Hypoglaucae Rhizoma), along with Danggui Longhui Wan (Angelica Sinensis and Gardenia Pill) taken as a supplement. Within ten days, her menstruation arrived with dark, ink-like blood, and the illness vanished. Subsequently, she was treated with formulas to nourish blood and harmonize the liver, leading to full recovery.
Mr. Wang, father of Mrs. Wang's neighbor, suffered from phlegm-cough, anorexia, asthma preventing lying down, retraction of the scrotum, constipation, and mental instability where he could not grasp things steadily, in a critical condition. His daughter requested Wang Mengying's help. Wang stated, "The root of vitality is about to collapse; this is not a typical disease." He used Ba Wei Di Huang Tang (Eight-Ingredient Rehmannia Decoction) removing Mudanpi (Moutan Cortex) and Zexie (Alismatis Rhizoma), combined with Sheng Mai San (Generate the Pulse Powder), adding Qingqian (Lead), Longgu (Fossilia Ossis Mastodi), Muli (Concha Ostreae), Zi Shiyun (Fluoritum), Hetao (Juglandis Semen), Chuanlianzi (Meliae Semen), and Rongrongsu (Cynomorii Fructus). After bowel movements occurred, symptoms subsided. Rongrongsu and Maidong (Ophiopogonis Radix) were then removed. After ten days, he recovered.
Mrs. Shoyi Tang, at the age of sixty, suffered from asthma and cough in mid-winter, unresponsive to medication. She had been unable to lie down for ten days. Upon diagnosis, Mr. Shao reported that drinking Ginseng soup temporarily relieved the asthma, but sweating persisted despite tonic use, raising fears of collapse. The pulse was wiry and slippery, especially on the right side. Wang Mengying remarked, "The difficulty of observation, listening, inquiry, and palpation is profound; one must have internal standards. This case should be treated based on the pulse; absolutely avoid Ginseng soup." He prescribed Gualou (Trichosanthis Fructus), Xiebai (Allii Macrostemonis Bulbus), Xuanfuhua (Inulae Flos), Suzi (Perillae Fructus), Huafen (Trichosanthis Radix), Xingren (Armeniacae Semen Amarum), Geqiao (Cockle Shell), Fuling (Poria), Qingdai (Indigo Naturalis), and Haizhe (Metereorites), mixed with Zhuli (Bamboo Sap) and Laifuji (Radish Juice). Symptoms decreased after the first dose, and she recovered after more than ten doses.
At the same time, an elderly woman surnamed Shi suffered from a very similar condition, with a deficient, wiry, fine, and slippery pulse. Wang Mengying prescribed Shashen (Glehniae Radix), Geqiao (Cockle Shell), Xuanfuhua (Inulae Flos), Xingren (Armeniacae Semen Amarum), Suzi (Perillae Fructus), Beimu (Fritillariae Bulbus), Guizhi (Cinnamomi Ramulus), and Fuling (Poria), heavily supplemented with Shudi (Rehmanniae Radix Praeparata), leading to recovery. This illustrates that while diseases may be identical, constitutions differ, making rigid adherence to fixed formulas difficult.
Madam Zhang, mother of Vice Prefect Zhang Menggao, over seventy years old, suffered from rebellious qi accompanied by fecal incontinence, irritability, and insomnia. Wang Mengying felt a slippery and solid pulse, along with constipation, red face, crimson tongue, and profuse phlegm. He administered Chengqi Tang (Chengqi Decoction) to purge, resulting in immediate relief. She passed away the following year from another illness.
Madam Wang, wife of Salt Commissioner Wang Zhiqing, suffered from phlegm-asthma. Dr. Hu had prescribed kidney-tonifying and qi-grasping formulas, Er Chen Tang (Two-Cured Decoction), and Sanzi Yangqin Tang (Three-Seed Nourishing Parents Decoction), pushing her condition to the brink of danger. Wang Mengying diagnosed a deep and choppy pulse, cold body with spontaneous sweating, resembling deficiency collapse, except for blocked urination and defecation, epigastric fullness, and greasy coating. He identified this as phlegm-heat obstructed by tonics, causing stagnation of the entire body's qi mechanism. He prescribed Gualou (Trichosanthis Fructus), Xiebai (Allii Macrostemonis Bulbus), Xuanfuhua (Inulae Flos), Zhesishi (Hematitum), Xingren (Armeniacae Semen Amarum), Beimu (Fritillariae Bulbus), Zhizi (Gardeniae Fructus), Ziwan (Asternatis Herba), Doulou (Sinosenecionis Flos), Haifushi (Calcitum), and Zhuli (Bamboo Sap) to open and descend. Relief came after one cup, and stability after two doses.
Wang Xiaogu, obese and fond of drinking, occasionally suffered from rebellious qi. Many doctors treated him for deficiency, leading to generalized edema resembling the case of Transport Commissioner Kang recorded in *Huichun Lu*. Wang Mengying diagnosed a very fine and rapid pulse, crimson tongue without fluids, and occasional delirium, identifying true yin depletion. Although external symptoms were milder than Kang's, recovery was impossible. He prescribed Xiyangshen (American Ginseng), Xuanshen (Scrophulariae Radix), "Er Di" (Shengdi and Shudi), "Er Dong" (Tiandong and Maidong), Zhimu (Anemarrhenae Rhizoma), Huafen (Trichosanthis Radix), Zhuru (Bambusae Caulis in Taenia), Beimu (Fritillariae Bulbus), Zhuli (Bamboo Sap), and Congxu (Scallion Stamen). After three doses, the swelling disappeared. Wang refused further treatment due to the persistent pulse condition, and the patient died soon after.
Magistrate Wu Yunxiang governed Jinxī. Starting from spring cold, by late summer and early autumn, he had profuse phlegm, rebellious qi, and emaciation. By early winter, symptoms multiplied: tinnitus, lower back pain, sensation of heat rising when lying down, fighting dreams, aversion to cold, and irritability. Doctors insisted on tonification with no effect. After half a year, his sons invited Wang Mengying. The pulse was wiry and fine, with rapid left cun and right chi, and urgent, irregular right cun and guan. The patient's neck hung low, unable to lift it; he had rapid breathing, dark tongue without coating, sallow complexion, and no thirst. Wang identified the root as long-term alcohol abuse causing internal accumulated heat, exacerbated by inappropriate tonics, damaging yin fluids and disrupting ascending/descending mechanisms. He prescribed Shigao (Gypsum), Zhimu (Anemarrhenae Rhizoma), Huangqin (Scutellariae Radix) to clear lung heat; Qinghao (Artemisiae Annuae Herba), Biejia (Testudinis Carapax et Plastrum), Zhizi (Gardeniae Fructus), Jinlingzi (Meliae Fructus) to soften liver and drain heat; Xuanshen (Scrophulariae Radix), Nvzhenzi (Ligustri Lucidi Fructus), Tiandong (Asparagi Radix), Huangbai (Phellodendri Cortex) to nourish water and control fire; Zhuru (Bambusae Caulis in Taenia), Xuanfuhua (Inulae Flos), Pipaye (Loquat Leaf), Juhong (Citri Reticulatae Pericarpium Rubrum) to regulate middle jiao and descend qi. He also used Longhui Wan (Longhui Pill) to drain gallbladder heat from alcohol and Zixue Dan (Purple Snow Pellet) to remove lingering pathogens. After three doses, yellow coating appeared on the tongue, indicating heat release. After six doses, cough decreased, hunger returned, and phlegm resolved. By mid-winter, he could hold court. After ten doses, chills ceased, feet warmed, and lung qi descended. Later, Xiyangshen (American Ginseng), Shengdi (Rehmanniae Radix), Maidong (Ophiopogonis Radix) were added to nourish fluids; Yinhua (Lonicerae Japonicae Flos), Lvdou (Phaseoli Radiatus Semen), and Xue Geng (Snow Broth) were used to resolve accumulation. By New Year's Day, he recovered fully.
Bao Jizhong suddenly felt cold with asthma, sweating, and near-syncope on the fifteenth day of late spring. Wang diagnosed a deep, wiry, soft, slippery, and slightly rapid pulse, attributing it to pre-existing fluid retention exacerbated by mistaken warm-tonifying herbs. The family reported taking Shenqi Tang (Kidney Qi Decoction) previously with temporary relief but subsequent retention of phlegm. Wang explained that Lung qi governs the distribution of fluids; the strong warming herbs (Aconite, Cinnamon) forced phlegm aside temporarily, but heavy yin-nourishing herbs (Rehmannia) trapped the pathogenic heat. This led to obstruction of the pivot mechanism, dry stool and urine, and hidden phlegm accumulating until sudden blockage of clear yang pathways. He prescribed Xiebai (Allii Macrostemonis Bulbus), Gualou (Trichosanthis Fructus), Zhishi (Aurantii Fructus Immaturus), Xingren (Armeniacae Semen Amarum), Xuanfuhua (Inulae Flos), Zhesishi (Hematitum), Banxia (Pinelliae Rhizoma), Ziwan (Asternatis Herba), Zhuru (Bambusae Caulis in Taenia), Lugeng (Imperatae Rhizoma), Gefen (Clam Shell Powder), and Xue Geng (Snow Broth) to calm the condition. Subsequent treatment regulated lung qi and cleared residual phlegm, curing him in a month.
Madam Pan Kentang, in mid-winter, suddenly developed asthma worsening daily under other doctors. He Xin zhi found irregular pulses and referred her to Wang Mengying. Wang diagnosed that both qikou pulses are governed by the Lung; current phlegm obstruction prevented qi flow. Although the pulse suggested deficiency, at thirty years old with good appetite and acute onset, greasy coating and thick phlegm indicated phlegm blocking the pivot mechanism, causing upward rebellion without descent, explaining cold feet, red face, anorexia, insomnia, and sweating. He prescribed Shigao (Gypsum), Huangqin (Scutellariae Radix), Zhimu (Anemarrhenae Rhizoma), Huafen (Trichosanthis Radix), Xuanfuhua (Inulae Flos), Zhesishi (Hematitum), Louren (Trichosanthis Semen), Tongcao (Tetrapanacis Medulla), Haizhe (Metereorites), Zhuli (Bamboo Sap), Lafuzhi (Radish Juice), and Lizhi (Pear Juice). Improvement after one dose, stability after two. He then removed Shigao and Zhesishi, adding Xuanshen (Scrophulariae Radix) and Xingren (Armeniacae Semen Amarum). After ten days, stability was achieved. Finally, Shashen (Glehniae Radix), Dihuang (Rehmanniae Radix), and Maidong (Ophiopogonis Radix) were used to nourish yin for convalescence.
Madam Yu Langzhai, in autumn, suffered from latent summerheat, cured by Wang but neglected in aftercare. She later developed rebellious qi (asthma), spontaneous sweating, cold limbs, and poor appetite. Inappropriate attacking or tonifying treatments failed. Wang prescribed methods to fill the Chong and Ren vessels and clear latent phlegm, combined with Gancao (Licorice), Xiaomai (Wheat), and Dazao (Jujube) to nourish blood, leading to recovery.
