Discussion: The so-called malpresentation (Ni Chan) generally occurs because, when a pregnant woman first feels abdominal pain before the actual time of delivery, the disturbance frightens the fetus, causing it to turn but not complete the rotation. If force is then applied to deliver, malpresentation results. Alternatively, if the place of delivery, as well as sitting and lying positions, do not conform to the directions of the four seasons and fail to avoid Five Elements taboos,触犯 (violation) often leads to disaster.

For treating difficult labor lasting several days, or abnormal presentation where conventional methods have failed to ensure safety, urgently administer this medicine. Upon administration, the infant will be born immediately. If taken from the full month of pregnancy, delivery will be easier, and the birth may pass unnoticed. Butter Paste Formula
True Beef Butter (1 jin), Autumn Okra Seeds (1 sheng), White Honey (0.5 jin), Soybean Yellow Sprouts (2 liang), Talcum Powder (ground into powder, 2 liang), Dianthera Herba (Qumai).
The above six ingredients: First, grind the first four ingredients into powder. Then, take the beef butter and melt it over low heat. Add honey and stir evenly. Add the medicinal powder and mix thoroughly. Store in a porcelain container. Take one spoonful each time, mixed with warm wine. Dosage can be increased to two spoonfuls.

For treating transverse presentation and breech presentation, fetal death in utero, retention of the placenta, and maternal exhaustion approaching collapse. Pinellia Powder Formula
Pinellia Rhizome (ground into powder, made into cakes with fresh ginger juice, dried, 0.5 liang), Ampelopsis Radix (Bailian, 1 liang).
The above two ingredients: Pound and sieve into a powder. Take 2 qianbi each time, mixed with warm wine. For difficult labor, take once; for transverse presentation, take twice; for breech presentation, take three times; for fetal death with retained placenta, take four times. This formula is more effective if 1 liang of Dianthera Herba (Qumai) is added and decocted.

For treating irregular malpresentation. Wheat Drink Formula
Wheat, Adzuki Beans (each selected clean and stir-fried).
The above two ingredients in equal parts: Coarsely crush and sieve. Take 3 qianbi each time, with one cup of water, decoct until reduced to 70%, remove dregs, and take warm.

For treating women with transverse or breech presentation, dead fetus in the abdomen causing bloating and putrefaction without expulsion, to promote delivery. Oak Leaf Drink Formula
Oak Leaves (one handful, cut finely), Licorice Root (one stem, length of a fist, broken like a little finger).
The above two ingredients: Decoct with 3.5 sheng of water over low heat until reduced to 1.5 sheng. Remove dregs and store in a porcelain bottle, sealing the mouth with paper. Place in the delivery room and keep warm over low heat. Take one small cup first. When the chest feels comfortable, take another small cup. Regardless of the number of doses, within three to four cups, foul matter will be discharged. This is very effective for routine induction of labor.
