Commentary states: In the "Rites of Zhou" (Zhou Li), surgeons (Yang Yi) and physicians for internal diseases (Ji Yi) had distinct divisions of labor and different treatment methods. Those suffering from sores and ulcers were treated by surgeons with medicinal applications, as this was not a specialized discipline sufficient to deeply investigate the subtle distinctions of blood exhaustion and stasis obstruction.
The "Inner Canon" (Neijing) states: "Sores and carbuncles must not be delayed for even an instant," lest they cause internal rotting of muscles and bones, penetrating through the zang-fu organs. Is this not taboo to heaven and humanity?
Auspicious and inauspicious directions for treating diseases:
In the third, seventh, and eleventh months, one must not face west when treating diseases.
In the fourth, eighth, and twelfth months, one must not face south when treating diseases.
In the first, fifth, and ninth months, one must not face east when treating diseases.
In the second, sixth, and tenth months, one must not face north when treating diseases.
When treating any disease, one must examine the patient's natal year and current annual fate (Xing Nian Ben Ming). If it aligns with auspicious stars such as Sheng Qi (Vital Energy), Tian De (Heavenly Virtue), and Fu De (Fortune Virtue), going there will surely lead to recovery. One must also observe whether the person observing the patient in accordance with Sheng Qi indicates auspiciousness, and diagnose based on facial complexion colors. If a patient has red veins in the eyes running vertically through the pupil, one vein indicates death within one year, two veins indicate death within two years... (text missing here).
Discussion on acupoints contraindicated for the development of sores and carbuncles.
Nine areas on the back and front are forbidden for sores.
1. Entering the hairline is Yuzhen (玉枕), also known as Sheben.
2. The cervical vertebrae of the neck.
3. Chonggu (崇骨).
4. Dazhui (大椎), corresponding to the five zang organs.
5. Both sides of the spinal column, Feishu (肺俞) acupoint.
6. Both sides of the paravertebral muscles, Pishu (脾俞) and Ganshu (肝俞) acupoints.
7. Both sides of the spinal column, Shenshu (肾俞) acupoint.
8. Posterior heart Jiwei (鸠尾) acupoint.
9. Jiwei bone acupoint.
Five areas on the front are forbidden for sores:
1. Throat bone, called Chuiping (垂膺).
2. Center of the chest, called Shenshe (神舍).
3. Xinjiuwei (心鸠尾).
4. Between the two breasts.
5. Two cun below the umbilicus, between the intestinal folds.
Three areas on the side are forbidden for sores:
1. The depression at the tip of the jaw near the ear behind the ear, called Houmai (喉脉); one point is under the mandible, one is the shoulder bone.
2. Three cun above Chengshan (承山), one point, for the large intestine.
Method for distinguishing good and bad prognoses of sores and carbuncles:
Generally, external sores and carbuncles are easy to understand; however, internal sores and carbuncles are hidden and invisible, making treatment extremely difficult since they cannot be seen. However, the five zang and six fu organs have shu points... (text missing here). Breathing is calm, voice is clear and bright; this is the fifth sign of good prognosis. Body qi is peaceful; this is the second sign of good prognosis. However, if symptoms match the seven signs of bad prognosis, the skin is tight and contracted like in good cases... (text missing here). Slight elevation... Liver abscess... Spleen abscess... Kidney abscess.
Shu Er (枢二) acupoint is beside the umbilicus. Tian (田), also named Shimen (石门), Jing Shi (精室), or Ming Men (命门), is one acupoint two cun below the umbilicus. If Guanyuan (关元) acupoint presents with dull pain, it is Small Intestine Carbuncle (Xiao Chang Ju). If the upper abdomen muscle slightly bulges, it is Small Intestine Abscess (Xiao Chang Yong).
Various methods for treating organ-related sores and carbuncles:
When sores, carbuncles, and boils first appear, they are about the size of millet grains, with slight pain or itching. If touched or broken, they spread... (text missing here). Upon initial sensation, one must quickly administer Rhinoceros Horn Decoction (Xijiao Dihuang Tang - inferred contextually though text says "Xi"), apply patches around the four sides... These are sores and boils... Apply ointment... Stagnation occurs... Wait for necrotic tissue...
Method for distinguishing when moxibustion is suitable or unsuitable for sores and carbuncles:
When sores and carbuncles on the back first appear, sized like millet grains, causing itching or pain, suggesting pathogenic presence, use hot water irrigation and apply medicinal patches... After one or two days... Penetrate... The sore... Over the sore... Especially good... Scorched and dry... Three moxa cones' fire... Chengguang (承光) (two cun posterior to Wuchu acupoint on the head), Shenting (神庭) (straight above the nose at the hairline), Zhukong (竹空) (in the depression behind the eyebrow), Yingchuang (膺窗) (one cun six fen below the chest, between the breasts), Naohu (脑户) (in the depression beside the large tendon one cun inside the posterior hairline), Yanmen (喑门) (in the depression at the posterior hairline of the neck), Yangluo (阳络) (one cun above the major pulse groove on the arm), Xiaguan (下关) (at the arterial pulse anterior to the ear), Ear major pulse responding to hand, flanking the larynx connecting to five zang organs), Shimen (石门) (two cun below the umbilicus, prohibited for women), Futu (伏兔) (on the leg... lateral to Xiaxi, one cun five fen distal to the base of the second toe). According to the "Classic of Acupuncture" (Zhenjing), these acupoints are prohibited for moxibustion. If sores or boils appear on them, do not apply moxibustion immediately. Instead, use various methods and water injection. Even if needling and cauterization are used, there is no harm. For chronic cases, take a small amount of sulfur, burn it over fire, pick it up with the tip of a silver hairpin, and use the flame to cauterize... to the appropriate degree.
Method for distinguishing when needle cauterization is suitable or unsuitable for sores and carbuncles:
For sores and carbuncles with little swelling at the head but extensive surrounding swelling, and slow pus discharge, needle cauterization is suitable. Before the pus becomes abundant, one must not fail to use various medicinal patches for rescue and treatment to stabilize... until... place, excessive heat... stasis without... is best... thin skin... merit... like... of... as... Do not drain pus abruptly; let it flow slowly. For large sores and carbuncles with ruptured pus and damaged muscle, fearing momentary pain, patients refuse multiple needle cauterizations around the periphery, opening only two or three places. Desiring early healing is not difficult? It is observed that some open swellings without assessing depth; cauterization may yield no pus initially, leading to suppuration overnight, or needles fail to release pus, causing new heads to form elsewhere, or pressing causes movement, increasing damage and pain, further injuring true qi. The Classic states: "If the disease is superficial but the needle is deep, qi and blood become submerged; if the disease is deep but the needle is shallow, toxic vapors are not discharged, resulting in a larger carbuncle." If one seeks rapid recovery while residual pus and pus roots remain, closing the wound will inevitably lead to recurrence. All swollen masses that are soft and painless should undergo needle cauterization. If occurring on the back, cupping therapy (Shui Jiao/Water Horn) is required for recovery.
Method for distinguishing when cupping therapy (Shui Jiao) is suitable or unsuitable for sores and carbuncles:
When treating sores and carbuncles on the back, if the swelling is high and hard, with thick, abundant, red-colored pus, cupping therapy is suitable. If the area is depressed, flesh color unchanged, soft, slow, and sparse, cupping therapy is not used... Pit two... down... waist region... affected by water... every... side... shock... sprout... rupture... precious to... to...
