Discussion states: Injuries caused by metal blades (Jinren, metal sharp instruments) vary in severity, determining life or death. The key lies in identifying the affected meridians and collaterals. One must observe the morphological changes of the wound and examine the subtle pulse characteristics. Ancient practitioners held that injuries to Tianchuang (SI16), the corner of the eyebrow, Naohu (DU16), the pulsating vessels in the arm, the inner thigh and groin area, above and below both breasts, the Jiuwei (CV15) point and heart region, the lower abdomen, and the Zang-Fu organs should all be avoided as they are critical areas. This is referred to as "the location of the original meridians." If the brain is ruptured with bleeding, eyes fixed upward staring straight ahead, inability to speak, gurgling sounds in the throat, tight lips with saliva spurting out, hands waving randomly, failure of muscle regeneration, dryness and tension upon palpation, discharge of greenish-yellow fluid, coldness and clarity at the wound margin, muscle wasting with foul odor, initial bright red blood followed by dark black blood, continuous bleeding accompanied by white fluid discharge—these constitute what is called "observing the morphological changes." Diagnosing a deficient, fine, and small pulse indicates survival; a slightly slow and delayed pulse also indicates survival. Reversal of these signs suggests difficulty in cure. This is termed "examining the subtle pulse." When these three aspects (identifying meridians, observing morphology, examining pulse) are integrated, treatment has a reasonable chance of success. One must avoid anger, loud laughter, sexual thoughts, physical exertion, and consumption of sour, salty, hot, alcoholic, or soup-like foods. These are measures within human capability and are hereby recorded together.
It is discussed that blood circulates within the vessels, irrigating the body endlessly. Deep injuries from metal blades cause rapid and turbulent bleeding, akin to a breach in the sea or a rupture of the Yellow River, making control extremely difficult. The crucial step is to curb the surging momentum. Externally observe the form, internally diagnose the pulse. If bleeding is unceasing and the pulse is large but stops flowing, it is difficult to treat. If bleeding continues, starting red and turning black, or yellow or white, with rotting, foul-smelling, cold, stiff, and tense flesh, it is also difficult to treat and requires careful examination.
