Treats cough with qi reversal, inability to lie flat relying on support for breathing (Formula see Volume 9, Shanghan Upper).
After administering Xiao Qing Long Tang (Minor Blue Dragon Decoction), if symptoms include excessive salivation, dry mouth, deep pulse at the Cun position, faint pulse at the Chi position, cold extremities, Qi rushing upward from the lower abdomen to the chest and throat, numbness and bi-syndrome in the hands and feet, facial flushing and heat resembling drunkenness, followed by heat flowing downward into the inner thighs, difficult urination, and occasional dizziness, administer Gui Ling Wu Wei Gan Cao Tang (Cinnamon Twig, Poria, Schisandra, and Licorice Decoction) to treat the rising Qi. Formula as follows:
Fuling (Poria, 4 liang) Guixin (Cinnamon Twig Heart) Gancao (Licorice Root, each 3 liang) Wuweizi (Schisandra Berry, half sheng)
The above four ingredients are crushed, boiled in 8 sheng of water until reduced to 3 sheng, strained, and taken warm in three divided doses.
If the rising Qi subsides but is replaced by cough and chest fullness, use Ling Gan Wu Wei Jiang Xin Tang (Poria, Licorice, Schisandra, Dried Ginger, and Asarum Decoction) to treat the cough and fullness. Formula as follows:
Fuling (Poria, 4 liang) Gancao (Licorice Root) Ganjiang (Dried Ginger) Xixin (Asarum, each 3 liang) Wuweizi (Schisandra Berry, half sheng)
The above five ingredients are crushed, boiled in 8 sheng of water until reduced to 3 sheng, strained, and taken warm, half sheng per dose, three times daily.
If the cough and fullness cease but thirst recurs along with the resurgence of rising Qi, this is due to the heating nature of Xixin (Asarum) and Ganjiang (Dried Ginger); taking them should cause thirst. If thirst does not occur, it indicates Zhi Yin (Fluid Retention in the Chest/Diaphragm). Zhi Yin typically causes dizziness, and those who dizzy often vomit; in such cases, Banxia (Pinellia Tuber) is added to eliminate fluid retention. The formula is Gui Ling Wu Wei Gan Cao Qu Gui Jia Jiang Xin Ban Xia Tang (Cinnamon Twig, Poria, Schisandra, Licorice Decoction minus Cinnamon Twig plus Dried Ginger, Asarum, and Pinellia):
Banxia (Pinellia Tuber) Wuweizi (Schisandra Berry, each half sheng) Fuling (Poria, 4 liang) Xixin (Asarum) Ganjiang (Dried Ginger) Gancao (Licorice Root, each 2 liang)
The above six ingredients are crushed, boiled in 1 dou of water until reduced to 3 sheng, strained, and taken warm, half sheng per dose, three times daily.
If fluid retention is eliminated and vomiting stops, but the patient presents with edema, Mahuang (Ephedra) should theoretically be used. However, because the patient already has numbness/bi-syndrome, Mahuang (Ephedra) is contraindicated. Instead, Xingren (Apricot Seed), Fuling (Poria), Gancao (Licorice Root), Wuweizi (Schisandra Berry), Ganjiang (Dried Ginger), Xixin (Asarum), and Banxia (Pinellia Tuber) are used. Formula: Jia Jiang Xin Ban Xia Xing Ren Tang (Decoction Plus Dried Ginger, Asarum, Pinellia, and Apricot Seed):
Xingren (Apricot Seed) Banxia (Pinellia Tuber) Wuweizi (Schisandra Berry, each half sheng) Fuling (Poria, 4 liang) Xixin (Asarum) Ganjiang (Dried Ginger) Gancao (Licorice Root, each 3 liang)
The above seven ingredients are crushed, boiled in 1 dou of water until reduced to 3 sheng, strained, and taken warm, half sheng per dose, three times daily.
If Mahuang (Ephedra) is incorrectly administered against this pattern, the patient will definitely experience cold extremities (Jue). This is because the patient has Blood deficiency, and Mahuang (Ephedra) disperses Yang Qi.
If the face is hot like drunkenness, this is caused by Stomach Heat rushing upward to flush the face; Da Huang (Rhubarb Root) should be added to promote purgation. Formula: Qin Gan Wu Wei Jia Jiang Xin Ban Xing Da Huang Tang (Note: Text says "Qin" likely referring to Huangqin or a typo for another herb, but strictly translating text provided: Huangqin/Ginseng/etc. Contextually usually implies adding Da Huang to the previous base. *Correction based on standard texts*: The text says "Qin Gan...", likely implying Huangqin or just a name variation, but I will translate literally as written "Qin Gan..." or assume standard modification. Given the list below includes Da Huang, Xixin, etc., I will translate the formula name as given). Formula: Qin Gan Wu Wei Jia Jiang Xin Ban Xing Da Huang Tang:
Da Huang (Rhubarb Root) Xixin (Asarum) Ganjiang (Dried Ginger) Gancao (Licorice Root, each 3 liang) Fuling (Poria, 4 liang) Xingren (Apricot Seed) Banxia (Pinellia Tuber) Wuweizi (Schisandra Berry, each half sheng)
The above eight ingredients are crushed, boiled in 1 dou of water until reduced to 3 sheng, strained, and taken warm, half sheng per dose, three times daily.
For cough with Qi reversal, Lung distension (Fei Zhang), floating pulse, fluid retention under the heart, hypochondriac pain radiating to the Qimen acupoint, and if excess pathogen causes irritability, with the patient often leaning or lying prone, Xiao Qing Long Jia Shi Gao Tang (Minor Blue Dragon Decoction plus Gypsum) is the primary treatment. Formula as follows:
Shigao (Gypsum) Ganjiang (Dried Ginger) Guixin (Cinnamon Twig Heart) Xixin (Asarum, each 2 liang) Mahuang (Ephedra, 4 liang) Shaoyao (Peony Root) Gancao (Licorice Root, each 3 liang) Banxia (Pinellia Tuber, half sheng) Wuweizi (Schisandra Berry)
The above nine ingredients are crushed, boiled in 1 dou of water. First boil Mahuang (Ephedra) to reduce volume by 2 sheng, skim off foam, then add remaining herbs. Boil until reduced to 2.5 sheng. Strong patients take 1 sheng. Weak patients take less. Children take 4 he. (Zhang Zhongjing treats Lung distension with cough and Qi reversal, irritability, panting, floating pulse, and fluid retention under the heart. Wai Tai Mi Yao records the same).
