(There is one treatise containing fourteen clauses, ninety-three formulas, and four moxibustion methods in this chapter.)
**Treatise:**
Biao Ju (Acute Suppurative Infection) refers to small spots suddenly appearing in the flesh, like bean grains. Small ones are like millet seeds, severe cases like plum or cherry pits. They may be red, black, green, or white, with indeterminate shapes. They have a root but do not swell initially. Pain is severe, radiating to the heart. The root extends deep into the muscle. If left untreated for a long time, the surrounding area swells completely, turning dark purple-black, and can rot bones and tendons. If the toxin spreads along the meridians into the zang organs, it can cause death. People in the South call it "Tuo Zhuo Du" (Touching Poison). They cut it away from the thick flesh. Alternatively, they cauterize it with a hot iron until it is charred like carbon. Or they perform moxibustion (100 cones). Or they drink Kui Gen Juice (Kudzu Root Juice), Lan Qing Juice (Indigo Juice), Xijiao Juice (Rhinoceros Horn Juice - *note: modern substitutes used*), Shengma Juice (Cimicifuga Juice), Zhuli (Bamboo Sap), Huanglong Tang, etc., as single-herb remedies to clear heat and achieve cure. This disease tends to occur on the ten fingers, resembling Dai Zhi (Whitlow). People who do not recognize it call it Dai Zhi. If not treated urgently, the toxin travels up the meridians into the zang organs, causing death. People in the South amputate the finger. Initially, the fingertip becomes dark, then swells, turns red, black, and dark, with pain penetrating the heart.
Dai Zhi (Whitlow) begins with swelling, heat, and pain, without darkness of color. Pus forms along the nail edge. In severe cases, the nails fall off. This is called Dai Zhi disease. Applying cold herbal juice compresses is beneficial. If heat is excessive, take Loulu Tang (Euphorbia Decoction) (see this volume, Carbuncle and Fungus section) and apply compresses. Applying Shengma Gao (Cimicifuga Ointment) (see this volume, Erysipelas section) is also acceptable. Pricking to release blood does not hinder subsequent washing, soaking, or applying ointment.
There is also a condition called E Rou Bing (Malignant Flesh Disease), where fleshy growths suddenly appear on the body like red bean grains. They protrude and grow, resembling cow or horse teats when pushed, with a chicken-comb-like top. If untreated, they grow continuously without pain or itching. This is caused by evil wind entering the muscles and meridians during spring and winter. Treatment should involve taking Loulu Tang and externally cauterizing with a hot iron daily until charred, then applying Shengma Gao. It takes many days to heal.
There is also Chi Mai Bing (Red Meridian Disease), where red vascular networks suddenly rise on the body, swollen and convoluted like dead earthworms. It appears as if there is water within the vessels. The length varies according to the meridian distribution. This is caused by evil wind entering the collateral vessels during spring and winter, leading to blood and flesh stasis. Treatment should involve Wuxiang Liaoqiao Tang (Five-Aromatic Forsythia Decoction) (see this volume, Carbuncle and Fungus section) and Zhuli (Bamboo Sap). Puncture to release blood, then apply Danshen Gao (Salvia Miltiorrhiza Ointment) (see this volume, Carbuncle and Fungus section). Applying Bai Jishi (White Chicken Droppings) is also effective.
E He Bing (Malignant Nodule Disease) involves sudden nodules in the flesh, clustered like plum pits or beans. The skin and flesh are painful, accompanied by high fever and aversion to cold. It resembles the root nodules of scrofula from other sores. The nodules arising from sores are slow and non-toxic. E He Bing arises suddenly and is toxic. If untreated and enters the abdomen, it causes oppression and death. This is caused by warm winds in winter colliding with sudden cold in spring and summer, condensing qi into toxicity. Treat primarily with Wuxiang Tang (Five-Aromatic Decoction) (see this volume, Carbuncle and Fungus section). Also apply Xiaodou Powder (Adzuki Bean Powder) externally, and wash frequently with a decoction. After resolution, apply Danshen Gao to dissipate remaining nodules. Initially, E He Bing resembles a bite from a "She Gong" (an ancient mythical poisonous insect), with no fixed location, often causing dull pain or no pain. Lack of pain leads to lack of concern, delayed treatment, and death. Therefore, early prevention is essential. Avoid chicken, fish, pork, beef, horse, and donkey meat. The lesion starts like millet or hemp seeds, hard inside the flesh, growing rapidly. Initially accompanied by aversion to cold, followed quickly by shortness of breath. Take Wu Zhu Yu (Evodia Fruit) 0.5 jin, grind into powder, mix with 1 sheng of water, squeeze out the juice, and take in one dose. Use the dregs... [Text fragment].
Fan ... [Text fragment] disease prefers the limbs. Symptoms include red vascular lines rising like braided ropes, urgent pain, and high fever. When occurring on the feet, it starts from ... [Text fragment] to the ankle, resembling braided ropes, hence the name ... [Text fragment] disease. When affecting the Kidney meridian, it prefers the axilla. This is caused by long-term labor generating excessive heat, which is suppressed by damp-cold, causing qi to knot in the tendons. If not treated urgently, chronic ulceration and pus discharge can lead to tendon contraction. If not resolved, persistent heat often leads to Fu Bing (foot swelling/pain), treated primarily with Loulu Tang. After purgation, puncture multiple times with a Feng Zhen (Sharp Needle) to remove bad blood. Use Qi Zhen (Qi Needle) to drain the root. Apply Xiaodou Powder on the nodule until it resolves. Treat using methods for Erysipelas. Also apply Sanwei Gancao San (Three-Ingredient Licorice Powder) for carbuncles (formula missing). If ulcerated, apply ointment/powder as for carbuncles.
E He Bing, ... [Text fragment], and Biao Ju often originate in Lingbiao (Southern regions) and are rare in Zhongtu (Central Plains). Southerners eat diverse foods, leading to varied diseases. Officials traveling there must prevent these illnesses rigorously. Without prevention, one will suffer the toxicity. Wuxiang Tang, Xiaodou San (Hemp Seed Adzuki Bean Powder, see this volume, Carbuncle and Fungus section), and Wu Zhu Yu are key medicines.
Fan Gu Ju (Bone-Attached Abscess) gets its name because it attaches to the bone and suppurates without initial surface rupture (*Waitai Mi Yao* reads "gu"). It prefers large joint spaces. Men and postpartum women prefer ... [Text fragment]. Children often get it on the back. Acute cases in adults begin with pain preventing movement; pressing elicits bone pain. After a day, skin and flesh become tight, swollen, and obese-looking. Children cry loudly when touched near the limb, indicating joint pain. Chronic cases in adults begin with a burning sensation in the muscle. After a day, pain and numbness prevent free movement. Children's limbs cannot move, appearing paralyzed. Examining joint spaces, if there is a burning sensation, it may be Fan Gu Ju. Generalized swelling without ulceration, with bluish-dark discoloration, is often mistaken by adults for Zei Feng (Thief Wind) or Wind Swelling, not recognizing it as Ju (Abscess).
... [Text fragment] Also, sleeping outdoors in summer and autumn exposes one to cold, while wind-heat lingers and knots, causing this disease. Acute cases have more heat than wind; chronic cases have more wind than heat. Children do not know how to avoid wind and cold; why do they get this? Because their blood is abundant and skin tender, wind attacks and causes condensation. Upon initial onset of Fan Gu Ju, take Loulu Tang to purge. Apply Xiaodou San to resolve. Can take Wuxiang Liaoqiao Tang (see this volume, Carbuncle and Fungus section).
Zei Feng (Thief Wind): The person has no sudden fever. After exposure to sudden wind-cold, deep bone/joint pain occurs but rotation is not impaired. Pressing elicits bone pain. Over time, it knots into pain or scrofula. Unlike Fan Gu Ju, which eventually swells and suppurates. Treating Fan Gu Ju as Zei Feng worsens the depth and pus. Treating Zei Feng as Fan Gu Ju exacerbates wind-cold, leading to scrofula, hemiplegia, and contractures. Treatment efficacy depends on accurate differentiation. Only the most skilled physicians can do this. To differentiate: Fan Gu Ju starts with pain before swelling. Zei Feng has pain without heat. Fan Gu Ju has high heat on the surface, alternating chills and fever in the limbs, red urine, constipation, and anhidrosis. Purging reduces heat and opens pores, leading to resolution. Even if not fully resolved, it becomes superficial. Zei Feng causes night pain only. Bones cannot be pressed or rotated. The painful area is not hot, nor does it have alternating chills/fever. The body feels shivery, preferring heat application. Pain slightly relieves. Occasional sweating indicates Zei Feng syndrome. Acupuncture, moxibustion, and hot compresses are suitable. Take ... Wind-Heat Toxin conflict causes swelling. Initially, blisters form on the swelling, resembling burns. This is Wind-Heat Toxin. Treat as Erysipelas.
Hong Zhu Chu (Blazing Candle Sore): Sudden blisters on the body, like boiling water splashed. Severe cases cover the head and face, or cause swelling in the chest, ribs, waist, and abdomen. The whole body feels like boiling water burning and blistering. Treat urgently with Loulu Tang to purge. Externally apply Shengma Gao. If ineffective, apply Dan Fang (Erysipelas Formula).
Re Chu (Heat Sore): Starts with white pus and yellow decay. The sore is shallow. Discharging yellow fluid is called Fei Chu (Fatty Sore).
Jin Yin Chu (Oozing Sore): Superficial. Scratching causes spreading and extension. Itching starts like Scabies. Scratching produces fluid, connecting lesions.
... [Text fragment] Chu: Starts like Fei Chu. Prefers hands and feet, often symmetrical. Fluctuates with the moon cycle. Pain, itching, and cracking vary by season.
Chronic abscess sores degenerating into deep Ju: Preferably occurs between the tibia and fibula. Discharges watery foul exudate. Cold prevents healing. Becomes Bone Ju over years, also called ... [Text fragment] Chu. Deeply rotten, dark green/black. Borders are hard. Center discharges pus and blood. Unresponsive to hundred medicines. Healthy tissue around the discharge is swollen and empty. Fragmented bones may排出. Soak in warm Chilong Pi Tang (Dragon Skin Decoction) (Vol 23, Intestinal Abscess). Wash daily in summer, every four days in winter. If granulation tissue is excessive, apply Bai ... [Text fragment] Ru San (White ... Radish Powder) (this volume, Carbuncle section). Remove necrotic tissue, apply every three days until stopped. Then apply Jia Zhushi San (Domestic Pig Manure Powder) (this volume, Carbuncle section) until healed. Burn pig manure to ash, grind to powder, fill the wound. Suck out white discharge, reapply until healed. If green granulation returns, apply Bai ... [Text fragment] Ru San again, then Jia Zhushi San until flat.
For Bone Ju unresponsive to all medicines, moxibustion on the sore sequentially for three days and nights heals it. If it recurs and bone protrudes from the hole, it is named Bone Ju. Take one dead female black chicken, remove meat, keep bones, roast until charred. Scrape wood屑 from ... [Text fragment] trees from three families. ... [Text fragment]
