Mr. Sanqi: Regarding the issue of "Open, Close, and Pivot" (Kai He Shu), I have explained it clearly online; here I will elaborate further.
"Open" (Kai) and "Close" (He); in these two characters, "Close" can also be referred to as "Shut" (Guan). Thus, "Open-Close" is equivalent to "Switch." Whether it is a machine or an electric light, there is always a switch. What object best embodies the function of a switch? It is the door, which represents the concept of Open-Close. To enter a room, one must open the door; after entering, one closes it. These are two actions: one is pushing inward (Open), and the other is closing outward (Close). For a door, first there must be a door panel to block the outside. For this door to open and close, it requires an axis. This "Pivot" (Shu) is essentially the axis; without an axis, rotation would be difficult. As the saying goes, "Flowing water does not rot, the pivot of a door does not get worm-eaten." Here, "household pivot" refers to the door's axis. The so-called "Open-Close" refers to the opening and closing of this door. The Qi mechanism of the human body is the same: there is Open and Close, with a Pivot in between. The characteristic of the Pivot is its ability to rotate; it is dynamic. The door itself does not move; only the axis moves. That is to say, through the rotation of the axis, the door is pushed to achieve Open and Close. Inward is Close, outward is Open; or inward is Open, outward is Close—these are two directions. Why can the axis move? Movement involves dynamics. What is the driving force? The movement of all things is driven by Fire (Huo); only Fire can serve as the driving force. Therefore, this axis and Pivot are essentially Fire, the location of Fire. So, what is Fire? This relates to the issue we mentioned earlier: "Ruler Fire illuminates, Minister Fire takes its position." What is Ruler Fire? It is Shaoyin Ruler Fire. What is Minister Fire? It is Shaoyang Minister Fire. Thus, in the theory of Open-Close-Pivot, Shaoyin serves as the Pivot, and Shaoyang also serves as the Pivot. One is Yin Pivot, the other is Yang Pivot. Then there arise issues of Open and Close. The Open of Yang is governed by Taiyang, while Yangming governs Close. The Open of Yin is governed by Taiyin, while Jueyin governs Close. The Pivot lies between Shaoyin and Shaoyang, effectively in the middle, corresponding to the Kan (Water) and Li (Fire) trigrams—one within Kan, the other within Li, representing Water and Fire. Water and Fire actually govern ascending and descending, functioning as this axis.
Regarding "Open," Taiyang governs Open. Taiyang primarily governs the exterior, controlling the exterior of the whole body, where Qi reaches the surface. When Taiyang opens to its extreme, reaching the state of "Two Yangs Combined Brightness" (Liang Yang He Ming) at the Yangming stage, it must then begin to Close. When Yang reaches its extreme externally, Yin begins to generate from within, and Yin returns. Things reverse when they reach their extreme. When Yang reaches its peak, Yin is born; at this time, Yin descends from above, transforming into Yangming. Yangming governs Close; Closing means shutting. If Open reaches its extreme, it naturally returns; this return process is a "Close" process. Yangming closes downward, and the Yin below must Open. Actually, the sequence and direction of Open and Close are consistent: as Yang Closes, Yin Opens, and Yang Qi enters internally. After Yang Qi reaches the body surface, it must converge inward and enter. Entering internally is a process of Yang Closing and Yin Opening. Thus, Yangming and Taiyin, one governing Close and the other Open, reflect the relationship between the Spleen and Stomach. At this point, Yang has withdrawn, entered internally, and reached the Zang organs. Upon reaching the extreme of entering internally, Yin has already opened but must now Close. Closing signifies Yin reaching its extreme internally, analogous to the Winter Solstice, when one Yang begins to move. When Yin reaches its extreme, it transforms into Yang; thus, when Yin Closes, Yang simultaneously Opens. Although Yang has not opened to its extreme, the direction is outward, so it Opens. Therefore, the Closing of Jueyin and the Opening of Taiyang occur simultaneously. One may consider formulas such as Guizhi Tang (Cinnamon Twig Decoction) and Danggui Sini Tang (Angelica Sinensis Cold Extremities Decoction); their directional nature ascends from below, from the lower left towards the upper part, representing a process of Closing and Opening. If Closing cannot occur, Yin cannot transform into Yang, leading to failure to ascend and subsequent sinking. Why are warming herbs commonly used to treat patients with essence depletion? Essentially, this indicates that Yin fails to transform into Yang, and Yang fails to enter Yin initially. Why is Danggui Sini Lizhong Granules effective for a wide range of conditions? It simultaneously Opens Taiyin and Closes Jueyin, promoting both descent and ascent. After Yang enters Yin, it emerges again from Jueyin and Taiyang, completing the cycle of ascending and descending. If one views only the local aspect, misinterpreting this failure to ascend as Heat, and insisting it is Damp-Heat pouring downward, constantly clearing Heat, it will never rise. Eventually, the Yuan Yang in Kan is damaged, and Minister Fire is extinguished, leaving no source of power, and Minister Fire is no longer in its proper position. This is the general meaning of Open-Close-Pivot. Every character in this concept corresponds to a specific formula.
Netizen: Teacher, why didn't you answer my question?
Mr. Sanqi: Do not ask why I do not answer questions. If everyone thinks this way, it becomes a major problem. I hope that when I lecture, you think carefully and do not look at other things. If you can understand these concepts clearly, Traditional Chinese Medicine (TCM) simplifies greatly. Do not be absent-minded; if you understand one point, you can deduce the entire system. Thus, whatever is discussed ultimately boils down to these few issues. If you never touch upon these core issues, you do not know what they are, and your understanding remains inconsistent. No matter how thick your textbooks are, if you do not know what "Fire" is, what "Qi" is, and what is being treated, how can healing occur?
For example, this afternoon, I treated a patient with acupuncture. Acupuncture is essentially about dredging the meridians and regulating the body to expel pathogenic Qi. She felt coldness at the needle insertion sites. Finally, I performed some Dao Yin (guiding and pulling exercises) to guide the Qi, and she felt cold air escaping. For decades, her lower abdomen had been ice-cold. This patient suffered from infertility, married for over 20 years without conceiving, with a consistently cold lower abdomen. After the cold Qi emerged from her right arm bit by bit, her lower abdomen began to warm up. The heat increased, eventually rising from the lower abdomen to the Middle Jiao. The lower area became hot. Why was she infertile? Essentially, the Earth Qi of the Lower Jiao was not warm. This is like the ground in northern China during winter: nothing grows, lacking vitality, due to insufficient temperature. Why is Earth suitable for life? Because the temperature is appropriate. Between 0°C and 100°C, water exists. Below is ice, above is vapor, and in the middle is liquid water. Room temperature below 30°C is suitable for human survival. If a person is a place of extreme Yin, lacking Yang Qi, being purely Yin and ice-cold, they lack vitality. This patient underwent many examinations with no findings. No matter how she was checked, no disease was found, and various treatments were ineffective. After this regulation, she perceived what "Qi" in the body truly is. Cold places are cold, hot places are hot. Heat indicates warmth, and warmth brings vitality. Cold places lack vitality. Spring governs growth; spring generates, summer grows. Why do people love spring? Because vitality is abundant, making spring suitable for the growth of all things.
Temperature is crucial. It should be moderate, not too hot. Temperatures of 30–40°C are unsuitable, as everything would turn to vapor. Complete stagnation is also bad; without any Yang Qi, it becomes a dead object. The moon has no life because the temperature is unsuitable. Essentially, treating illness is about regulating body temperature—treating the body's internal season and climate. Do not obsess over physical forms or rely solely on laboratory tests for microbes. Microbes are products of climate, not the cause of climate. Without a climate suitable for life, life cannot exist; focusing on microbes first is putting the cart before the horse. However, observing biological existence allows us to infer the climate. Knowing life exists implies a suitable climate exists.
The human body is 70% water, sometimes more. This water, or Jin Ye (Body Fluids), is essential; without it, the body becomes dry. With fluids, there must be Fire to drive transformation into Qi. If Fire is insufficient, water stagnates, failing to transform into Qi, resulting in Dampness. The tongue coating appears damp, and severe cases show edema overflowing onto the skin surface or forming Phlegm-Dampness internally. This is due to insufficient Qi transformation and weak Fire. At this stage, supporting Yang is necessary; once Yang Qi is sufficient, it automatically resolves the stagnation. If body fluids are scarce and the body is extremely dry inside and out, it indicates excessive Fire and insufficient Water, requiring supplementation of Water. Thus, neither Water nor Fire can be lacking; deficiency leads to pathology. Life depends on Water and Fire. All things grow relying on the sun, yet rain and dew nourish crops. Without timely rain and dew, plants wither (drought). Excessive water without sunlight causes flooding, also lethal. Therefore, Yin and Yang are essentially Water and Fire. Water and Fire are the manifestations of Yin and Yang. Yang transforms Qi, Yin forms shape. Water's nature is Cold, Fire's nature is Hot. Extreme heat leads to combustion, transforming Qi, which destroys formed shapes, releasing form. This can be seen in water: heating turns it to vapor (invisible, dispersed); cooling condenses it to droplets, and further cooling freezes it into solid ice. Similarly in humans, Yin excess causes coagulation, Yang excess causes dispersion. Dispersion outward manifests as sweat (evaporation of fluids/blood). Yin excess causes coagulation, lowering overall temperature, leading to Qi and Blood stagnation, obstruction, cessation of sweating, and accumulation of waste impeding Qi movement.
These principles apply to all diseases. Regardless of the specific diagnosis, applying these principles identifies the root issue, much like using a multimeter to test electrical circuits. Check for current; if none, restore the connection.
Netizen: What is diabetes?
Mr. Sanqi: What is diabetes? Why does sugar fail to metabolize? Diabetes is not merely about excessive sugar intake; rather, it is a problem of sugar transformation. Sugar cannot convert into energy, meaning Yin fails to transform into Qi. The reason for this failure to transform is insufficient Fire—a simple issue. Fire requires fuel. What is the fuel? Fuel is Yin. Thus, one must find the substance that generates Fire and allows Fire to adhere to it. Merely having Fire is insufficient without fuel to sustain it.
Mr. Sanqi: Any disease, regardless of type, will eventually reach a stage where it cannot be cured; this is inevitable. Do not assume every disease is curable. Even with correct methods, if Yuan Qi (Original Qi) is exhausted, treatment is useless.
Netizen: Teacher, could you discuss Tan Yin (Phlegm-Fluids) and Shui Qi (Water-Qi) diseases?
Mr. Sanqi: Tan Yin and Shui Qi diseases involve fluid retention in different locations. If Qi transformation is insufficient and cannot function in a specific area, disease arises. Tan Yin resides in the Spleen and Lung; Shui Qi affects the Exterior. Both result from Yang's inability to transform. Exterior involvement leans toward Taiyang. Do not be confined by terminology. Instead of asking how to treat a disease named X, transform the name into the image of Yin-Yang or Water-Fire. Is there too much Water or too little? Once the fundamental imbalance is regulated, the specific label becomes irrelevant. Zheng Qin'an's *Yi Li Zhen Chuan* (True Transmission of Medical Principles) emphasizes distinguishing Yin and Yang deficiency/excess to encompass all diseases. "Those who grasp the essentials need but one word to conclude; those who miss the essentials scatter infinitely."
Netizen: How should liver cancer be treated?
Mr. Sanqi: Liver cancer at this stage is difficult to manage. The Liver stores Blood. Now transformed into cancer, it presents as a large, hard mass, like stone, with blocked blood vessels. The Liver is a hub between Water and Fire. Dysfunction here disrupts the interaction of Water and Fire. Treatment must unblock the obstruction. Cold causes coagulation, implying significant Cold pathogens internally. Treatment must also address the Blood level. Understanding this principle, herbal medication may still struggle. At this stage, management is challenging. It is easier said than done. Patients should not reach this stage. Yin forms shape; creating such a large, solid shape requires immense heat to dissolve. During dissolution, the mass may enlarge as it consumes energy. Since it is hard, it must be softened, like a willow branch in spring possessing generative Qi, unlike brittle branches in autumn/winter. Currently, generative Qi is lost. First, preserve the warm Qi within the Water. Warm water generates Wood (Liver), nourishing it and preventing withering. Cancer blocks this hub, severing the Water-Fire pathway, halting ascent. As long as a passage remains, death is not immediate. Completely eliminating the tumor is impossible; one must first leave a pathway for gradual resolution before addressing the mass.
