Commentary states: The Five Elements and the Five Zang organs mutually generate each other. Although the Liver is situated centrally (in terms of specific textual context or emphasis), it is the head of all Zang organs, located in the directions corresponding to Jia and Yi (Wood), harboring nurturing and benevolent virtues, thus it should flourish in accordance with Spring. The Liver is the mother of the Heart; the remaining Zang organs follow in sequence of generation. The Heart acts as the monarch, governing the Spirit (Shen) of the body, perceiving everything without omission; the other four Zang organs serve as ministers assisting the monarch. If the four ministers have worries, the monarch's heart will inevitably be affected. The Lung is referred to as the "House of Wind," positioned at the highest point (Tai Qiong), serving as the gateway for respiration and the pathway for Qi movement. When the functions of various Zang organs are disordered, changes in Qi will manifest; who can avoid passing through this gateway? If wind-heat pathogenic factors are excessive, worry in the heart leads to headache; if worry is excessive, it leads to vexation. Extreme heat must transform into cold, and extreme cold must transform into heat; this is the principle of mutual dependence and transformation, which clarifies that these conditions are due to wind and manic heat pathogens. By pacifying wind-heat and suppressing manic pathogens, Ying (Nutritive) and Wei (Defensive) Qi will naturally become smooth and harmonious.

Commentary states: Itching in the body indicates deficiency, while pain indicates excess. Pathogens in the exterior indicate deficiency, while pathogens in the interior indicate excess. Deficient heat in the Heart and Lung rushing to the chest causes dry mouth; prolonged dryness leads to sores and pus formation, resulting in itching. This is naturally due to extreme deficiency, not excess. "Zang" means storage; because they cannot contain and store Yang Qi, allowing Yang Qi to easily emerge, the body becomes increasingly deficient. If only the Liver has wind pathogen, Wood Qi attacks the Heart causing pain, but this is not true "Zhen Xin Tong" (True Heart Pain). If it were true Heart Pain, death would occur within half a day, because the Heart does not accept pathogenic invasion directly.
