The lesion occurs behind the ear, also known as "post-auricular hair abscess" (Er Hou Fa). It is necessary to differentiate between excess syndrome of Yang and deficiency syndrome of Yin; treatment must be error-free. If the lesion color is white, administer Yanghe Wan (Yanghe Pill) decocted together with Erchen Tang (Two-Cured Decoction), or take Xiaojin Dan (Xiaojin Pill) to resolve it. If the lesion color is red, take Xingxiao Wan (Xingxiao Pill) to resolve it. Various medical texts, regardless of red or white color, uniformly treat with seven herbs: Xuanshen (Scrophulariae Radix), Niubangzi (Arctii Fructus), Lianqiao (Forsythiae Fructus), Canwei (Bombyx Batryticatus), Chishao (Paeoniae Radix Rubra), and Yinhua (Flos Lonicerae Japonicae). Even for red lesions, such treatment fails to resolve them, let alone white lesions, which would surely suffer harm.
Ma stated: Post-auricular sharp toxin (Er Hou Rui Du), in its initial stage, invariably presents with a red root and a white head. This is caused by the accumulation and knotting of pathogenic fire in the Liver and Lung meridians, and belongs to the region of the Shaoyang Sanjiao (Triple Burner). This meridian has abundant Qi but scarce Blood; therefore, Yanghe Wan must absolutely not be administered. If the condition progresses to ulceration, both Qi and Blood become deficient, and pathogens sink inward. In this case, Renshen (Ginseng Radix et Rhizoma) and Rougui (Cinnamomi Cortex) should be used in small amounts to expel toxins from the interior and tonify deficiency.
It is further stated: In the initial stage, dispersing wind and resolving phlegm is appropriate. Since sharp toxin is synonymous with post-auricular hair abscess, it is similar to Nao Ju (Brain Carbuncle).
A lesion occurring on the foot bone is commonly called "Jia Gun Ju" (Clamping Stick Carbuncle). If un-ulcerated and white in color, treat as Ju (Carbuncle); if red and swollen, treat as Yong (Abscess). If ulcerated for a long time with a shape resembling a donkey's eye, do not treat it as a common sore. One must inquire about the initial color (red or white) to differentiate between Ju and Yong and apply corresponding treatments.
Ma stated: For this condition, treating it as an Abscess (Yong) is the correct approach.
A blister forms inside the skin of the sole, causing pain so severe that walking is difficult. Slightly remove the old skin, grind Sheng Caowu (Radix Aconiti Kusnezoffii) in wine, and apply it topically for rapid recovery. If the lesion occurs at the center of the sole, it is named Yong Quan Ju (Yongquan Carbuncle), and treatment should be differentiated based on red or white color.
Ma stated: It should be treated as Damp-Heat syndrome. Red color indicates severe Heat; white color indicates heavy Dampness.
The lesion occurs at the junction of the front of the anus and the back of the genital root. Initially, it appears as fine granules, gradually enlarging to the size of a lotus seed, and after several days, to the size of a plum or peach. It is commonly called "Tou Fen Lao Shu" (Stealing Feces Mouse). Upon ulceration and leakage of pus, a fistula tract forms. Prolonged fistula leads to Qie Zheng (Debility Syndrome). When a patient with Debility Syndrome contracts this, it is akin to a life-threatening demon. While most fistulas are curable, this one is particularly difficult to treat; improper treatment may cause the fistula tract to enlarge, eventually resulting in Hai Di Lou (Sea-Bottom Fistula), which is incurable. Before suppuration occurs, use Sheng Gancao (Raw Glycyrrhizae Radix et Rhizoma) and Zhi Jun (Prepared Rheum Radix et Rhizoma), each 3 qian, decocted in wine and taken on an empty stomach; one dose results in cure. If suppuration has occurred, treat with Xingxiao Wan (Xingxiao Pill). If the lesion color is white, treat with Xiaojin Dan (Xiaojin Pill).
Ma stated: This type of fistula also has specific treatment methods; those without mixed internal symptoms are curable. However, if cough and fever appear, it becomes incurable.
It is further stated: Formulas such as Shu Jun (Cooked Rheum), Xingxiao Wan, and Xiaojin Dan must not be administered when Yin-deficiency cough is present.
