Chushi Erchen Tang (Dampness-Expelling Two-Aged Decoction) is indicated for treating Buttock Carbuncle (Shentong), Shangma Abscess, and Xiaoma Abscess.

Ganzhou Dan, Guijin Wan, Ganzhou Gao, Weisheng San, Tuoli Xiaodu San, Liujunzi Tang, Liuwei Dihuang Wan (all described in the first volume of this book).
Shangma Abscess arises on the buttock, near the right side of the anus.
Xiaoma Abscess arises on the buttock, near the left side of the anus.
The treatment for these two conditions is the same as that for Buttock Carbuncle.
Fenggan arises on the buttocks and thighs, commonly known as "Sitting Board Sore." It resembles scabies or tinea, breaks open to discharge yellow fluid, and presents with itching preceding pain. It is caused by wind-cold pathogenic factors lodging in the anal region. It is appropriate to apply Xiangbai San or similar medicaments externally to allow the toxic fluid to drain completely, leading to scabbing and healing.
Xiangbai San: Kubai Fan (Calcined Alum), Qingfen (Calomel, each 2 qian), Zhangnao (Borneol, 1 qian).
Grind all into powder. If the lesion is moist, sprinkle the dry powder; if dry, mix with raw Tung Oil for topical application.
Alternative methods: Roast Loofah Skin to dryness, grind into powder, and apply mixed with Sha Jiu (Baijiu); or roast Garlic Clove until carbonized (preserving its nature), grind into powder, and sprinkle; or decoct Yuanhua Root in water to wash the affected area.
Xuanyong arises at the Huiyin Acupoint, located posterior to the scrotum and anterior to the anus. Initially, it is like a pine cone; subsequently, it enlarges to the size of a lotus seed. After several days, it erupts with redness, heat, swelling, and pain, eventually becoming as large as a plum or peach.
Yongquan Ju arises anterior to the sacrum. Medical texts state: Swelling manifests along the Taiyin Meridian, resembling a crouching mouse. It may be punctured after ten days. Discharge of white pus indicates a favorable prognosis; green pus indicates danger; red-black pus indicates an unfavorable prognosis. Failure to puncture leads to death.
Chuandang Fa arises at the Qiongu Acupoint, which is the base of the genitalia. The area is red and swollen, with pain radiating to the genitals, lower back, and anus. Moxibustion should be applied to Sanyinjiao Acupoint for thirty-seven moxa cones.
Kuma Abscess arises between the legs, near the pubic hair line.
Note: All the above conditions arise posterior to the anterior yin (genitals) and anterior to the posterior yin (anus). They are named differently due to their distinct morphologies. These conditions belong to the collateral branches of the Ren Mai (Conception Vessel) and are located at the convergence of the Du Mai (Governor Vessel) and Bo Mai vessels. They result from deficiency of the Three Yin Meridians of the Foot, often compounded by unfulfilled sexual desires. Mild cases may develop into fistulas; severe cases may lead to death. Therefore, symptoms such as numbness, black depression, diarrhea, vomiting, hiccups, and fatigue, or prolonged progression leading to internal organ ulceration via the anal canal, are considered incurable. Ancient texts state: For those with hyperactive Yang Qi, Kanli Wan is suitable, utilizing its bitter-cold property to drain fire within water, thereby reducing Yang Qi so that Yin naturally generates. For those with deficient Yang Qi, Liuwei Dihuang Wan is suitable, utilizing its sour-warm property to generate water within fire, thereby strengthening Yang Qi so that Yin naturally generates. However, nine out of ten cases of this condition involve Yin Essence deficiency, while only one in a hundred involves Yang Qi hyperactivity.
In the initial stage, if there is swelling, pain, and dark, scanty urination, first administer Zhi Gancao (Processed Licorice Root) for one to two doses, combined with Suan Jiu (Garlic Moxibustion). Subsequently, administer Longdan Xiegan Tang (Gentian Liver-Draining Decoction). If the abscess has matured and cannot be resolved internally, use Tuoli Xiaodu San (Interior-Supporting Toxin-Resolving Decoction) added with Chuanshanjia (Pangolin Scale) and Zaojiaoci (Gleditsia Spine). If it ruptures spontaneously or causes distending pain due to pus, perform needle drainage. In the morning, administer Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia); at noon, administer Shiquan Dabu Tang (Ten-Ingredient Great Tonifying Decoction) added with Mudanpi (Tree Peony Bark), Zexie (Alisma Rhizome), or additionally Fuzi (Aconite Root). If it becomes a chronic fistula, dissolve Guolao Gao (Honey Confection) in decoction and swallow with Lafan Wan (Beeswax-Alum Pill). Intermittently apply Douchi Bing Jiu (Fermented Soybean Cake Moxibustion). For long-standing cases, apply Da Fuzi Bing Jiu (Large Aconite Cake Moxibustion). If evening fever, irritability, thirst, panting, body fatigue, poor appetite, loose stools, and frequent urination occur, this indicates Spleen Deficiency. It is appropriate to administer Buzhong Yiqi Tang (Middle-Warming Qi-Boosting Decoction) added with Fuling (Poria) and Banxia (Pinellia Rhizome). Once appetite improves and other symptoms subside, add Maidong (Ophiopogon Tuber) and Wuweizi (Schisandra Berry) for natural recovery. The Classics state: This is Yin Deficiency combined with Spleen Deficiency. The Spleen is Yin Earth and governs blood generation. If one mistakenly uses bitter-cold agents such as Zimu (Anemarrhena Rhizome) and Huangbai (Phellodendron Bark), further damaging the Spleen and Lung, it severs the source of Qi and Blood production.
