Hemorrhoids and fistulas, although within the scope of surgical physicians, I discuss them here. However, treating diseases based on their location—refusing to treat those in hidden areas—is not the mindset of a benevolent healer. In my study of the *Neijing* (Inner Canon), it states: "Due to overeating, meridians become distended and loose, leading to intestinal dysentery which forms hemorrhoids." Generally, overeating damages Pi Tu (Spleen Earth). When Pi Tu is damaged, it fails to nourish Fei Jin (Lung Metal). When Fei Jin lacks nourishment, Gan Mu (Liver Wood) loses control, generating Xin Huo (Heart Fire). Xin Huo insults Fei Jin and controls Pi Tu. Thus, there is control over what should be controlled and insult against what should not be insulted. However, hemorrhoids resulting from overeating must have specific triggers. What are these triggers? They include, after overeating, sudden anger, strenuous effort, prolonged sitting, or indulgence in alcohol and sex; women may suffer from difficult labor; children may cry excessively at night. These factors cause Qi and Blood to flow chaotically and meridians to intersect and block, flowing into the anus to form hemorrhoids. Swelling indicates Dampness; pain indicates Fire; itching indicates Wind; constipation indicates Dryness. Treatment should suppress the dominant pathogenic factor. I fear students may not grasp the details, so I append the family secret recipe of specialist Zhao Zhenzi, followed by my own insights, to supplement any omissions.
