Dingyong (carbuncle following smallpox) occurs after the scabs of smallpox have fallen off. It is caused by toxic factors remaining and hiding in the deficient areas of the meridians or at joint movements. In mild cases, only nodular swellings and boils appear; in severe cases, Liuzhu (flowing abscesses where toxins spread causing multiple abscesses) occurs all over the head, neck, chest, and back, making it difficult to treat. For excess-heat patterns, use Chishao (Radix Paeoniae Rubra), Gancaojie (Node of Radix Glycyrrhizae), Lianqiao (Fructus Forsythiae), Jiegeng (Radix Platycodonis), etc. Add Shengma (Rhizoma Cimicifugae) and Gegen (Radix Puerariae) to guide the medicine upward; add Binglang (Semen Arecae) and Niuxi (Radix Achyranthis Bidentatae) to guide it downward. Further assist with Beimu (Bulbus Fritillariae Cirrhosae), Rendong (Flos Lonicerae), Gualou (Fructus Trichosanthis), and Baizhi (Radix Angelicae Dahuricae). For alternating chills and fever, add Huangqin (Radix Scutellariae) and Huangbai (Cortex Phellodendri). For blood stasis, add Sumu (Lignum Sappan) and Honghua (Flos Carthami). For constipation, add Dahuang (Radix et Rhizoma Rhei). For Qi and Blood deficiency, characterized by a cool body and no thirst, bitter-cold herbs must be used cautiously to avoid damaging Zhongqi (Middle Jiao Qi/Spleen-Stomach Qi). Taking Baoyuan Tang (Preserve the Origin Decoction) as an adjunct is essential. Externally, apply a paste made by crushing Machixian (Herba Portulacae), pig fat (Zhi), and rock sugar (Shimi). Alternatively, use Tianmaziren (Semen Cannabis) flesh mixed with Songxiang (Resina Pini) powder as a plaster. If pus has formed, puncture with a needle. If ulcerated, apply Haibarer San (Haibarer Powder). If dry, mix with oil for application.
Shegan Niuzi Tang (Belamcanda and Burdock Seed Decoction): Treats carbuncles, sores, and toxic sores after smallpox/rash.
Niuzi (Niu Bang Zi / Semen Arctii) 2 qian, Shegan (Rhizoma Belamcandae), Shengma (Rhizoma Cimicifugae), Gancao (Radix Glycyrrhizae) each 0.5 qian.
Decoct with one zhong of water until reduced to 0.6 zhong, then take orally.
Doufengchuang (Wind-induced rash after smallpox) initially presents as fine sores with itching, which then spread into patches with exudation of serum. It is caused by exposure to wind after smallpox. It is suitable to apply Maijian San externally while avoiding wind and observing dietary restrictions. In severe cases, intense itching and exudation occur, leaving no intact skin, resulting in Doulai (smallpox-derived dermatitis/scarring). Urgently use Shiquan Dabu Tang (Ten Substance Great Supplement Decoction) to greatly tonify Qi and Blood, combined with Kushen Wan (Sophora Pill) to clear heat and detoxify. Take both formulas together. Externally apply Miecan Jiuku San (Scar-Extinguishing and Suffering-Saving Powder).
Maijian San: Xiaomai (Frumentum Tritici) 1 he, stir-fried until dark yellow. While hot, place in a mortar, add Liuhuang (Sulfur) 4 qian and Baipi (Arsenic Trioxide) 1 qian, grind into powder, mix well, and let cool. Then add Yanjiao (Smoke Glue/Resin) 8 qian, Chuanjiao (Pericarpium Zanthoxyli) 3 qian, and Kufan (Alumen Exsiccatum) 2 qian, grind into powder, and mix uniformly. First wash the lesion with Congtang (Scallion decoction), mix with Mayou (Sesame Oil) for application, cover with oiled paper, change once every three days. Three applications will cure it.
Kushen Wan: Kushen (Radix Sophorae Flavescentis) 1 liang, Baijili (Fructus Tribuli), Huma (Semen Sesami Nigrum), Niubuzi (Semen Arctii) each 5 qian, Gancao (Radix Glycyrrhizae) 2.5 qian.
Grind into powder, make pills with wine paste. Take with Zhuyetang (Bamboo Leaf Decoction).
Miecan Jiuku San: Mituoseng (Litharge/Oxide of Lead), Huashi (Talcum) each 2 liang, Baizhi (Radix Angelicae Dahuricae) 5 qian.
Grind all into powder. If wet, sprinkle dry powder; if dry, mix with Baimi (White Honey) for application.
Douhou Ganshichuang (Post-smallpox gan erosion sore): Toxic factors stagnate in muscles, penetrate inward to bones and sinews, and connect outward to the skin. It causes intermittent pain and bleeding, and does not heal for a long time. Use Shiquan Dabu Tang internally and Mianjian San externally.
