The emergence of a small tongue beneath the main tongue is called *Chong She* (Double Tongue). It is appropriate to apply *Bingpeng San* (Ice and Borax Powder) topically.
If a nodule forms on the tongue, characterized by stiffness and pain, it is attributed to Phlegm-Qi stagnation. The nodule should be pricked with a clothing needle, followed by topical application of *Bingpeng San* and oral administration of herbs that clear heat, reduce fire, and resolve phlegm.
If sores form on the tongue, resembling bayberries, without external symptoms of chills or fever but accompanied by internal irritability, it is referred to as severe mouth sore (likely *Zhong Gan*). Pricking with needles is contraindicated. It is appropriate to treat with *Huanglian Jiedu Tang* (Coptis Decoction for Resolving Toxicity) supplemented with *Jiegeng* (Platycodon Root) and *Gancao* (Licorice Root), combined with the topical insufflation of medicated powder.
If a sore forms on the tongue, shaped like a bean, hard, accompanied by chills and fever, and the pain radiates to the heart region, it is a *Ding Chuang* (Carbuncle/Furuncle). It is appropriate to hold *Chansu Wan* (Toad Venom Pill) under the tongue, swallowing it as it dissolves, or take three additional pills to resolve internal toxicity. In severe cases, acupuncture may be used, or moxibustion applied to the Shaocong acupoint (*Shaochong*, HT9).
*Nong She* (Playing Tongue) refers to the tongue protruding slightly and immediately retracting. This belongs to deficiency in the Heart and Spleen meridians. It is appropriate to tonify with *Wenpi San* (Warm the Spleen Powder). Cases occurring after major illnesses are difficult to treat.
*Shu She* (Extended Tongue) refers to the tongue extending outward and taking a long time to retract. This is caused by accumulated heat in the Heart and Spleen. It is primarily treated with *Xiehuang San* (Drain the Yellow Powder).
*Liangge San* (Cool the Diaphragm Powder) (see Mouth section); *Huanglian Jiedu Tang* (Coptis Decoction for Resolving Toxicity); *Chansu Wan* (Toad Venom Pill) (see Carbuncle section).
Regarding meridians: The front teeth are called *Chi*, belonging superiorly to the Du Meridian and inferiorly to the Ren Meridian. The side teeth are called *Ya*, belonging superiorly to the Foot-Yangming Stomach Meridian and inferiorly to the Hand-Yangming Large Intestine Meridian. Teeth are the exterior manifestation of bones and belong to the Kidney. The growth of teeth is like planting trees in soil. If damp-heat exists internally in the stomach and intestines and rises to the gums, and is simultaneously constrained by wind-cold or forced by cold drinks, the damp-heat cannot diffuse outward, resulting in pain. This is a Yangming disease, not a Shaoyin disease. Since cold is the manifest aspect (biao), external rinsing and wiping medications should be pungent and warm to disperse cold and open stagnation. Since heat is the root aspect (ben), internal medications should be pungent and cool to dissipate heat and clear the middle jiao, such as *Lianqiao Tang* (Forsythia Decoction). If fire pathogen is excessive, pain will radiate to the cheeks and zygomatic arches; add *Shigao* (Gypsum Fibrosum). If wind pathogen is excessive, swelling occurs; add *Fangfeng* (Saposhnikovia Root). If qi is excessive, abnormal growth occurs; add *Zhizi* (Gardenia Fruit). If qi stagnation causes distension, also add *Zhizi*. If phlegm is excessive, discoloration occurs; add *Beimu* (Fritillaria Bulb), rinse externally with vinegar to remove phlegm-saliva and drain wind-heat. If caries cause blackening, add *Shijunzi Rou* (Seed of Quisqualis); if worms cause cavities, add *Binglang* (Areca Seed), rinse externally with *Wulingzhi Tang* (Trogopterus Dung Decoction) to kill parasites. If blood deficiency causes tooth itching, stomach-intestine deficiency causes gum recession and exposed teeth, overwork causes loose teeth, and kidney deficiency causes sparse teeth. Only when essence is secure will teeth be firm. Treatment should further involve tonification according to specific symptoms.
