Jing Ju (Well Carbuncle) arises from Heart Fire manifesting at the Zhongting (CV17) acupoint. Initially, it resembles a mung bean in size and gradually becomes swollen and painful. It is accompanied by mental restlessness, hot skin, dry lips, and thirst; if the lesion is red and vibrant, it is easy to treat, but if it turns black and sinks inward, the prognosis is ominous.
[Note] This condition occurs in the epigastric region, belonging to the Ren Mai (Conception Vessel) at the Zhongting (CV17) acupoint, caused by the accumulation of Fire-Toxin in the Heart Meridian. Initially resembling a mung bean, the swelling and pain gradually increase. The patient feels mental restlessness as if burning, skin heat like fire, spontaneous sweating, cracked lips, and extreme thirst with a desire to drink cold water. One should urgently administer Neishu Huanglian Tang (Internal Draining Coptis Decoction) or Mailing Dan. If symptoms include irritability, nausea, fever without sweating, Duoming Dan should be used to induce sweating. If the lesion is red, vibrant, and highly swollen, it is a favorable sign (Shun); if it is black, sunken, and flat, it is an unfavorable sign (Ni). All other internal and external treatment methods follow the regulations in the "Carbuncles and Ulcers - Swelling Stage" and "Ulcer Stage" sections. If the ulcer fails to heal after one year, it will inevitably develop into a penetrating cold fistula, which is difficult to treat.
Fengwo Ju (Honeycomb Carbuncle) resembles a honeycomb, occurring on the lateral chest above the breast with multiple sores. It presents with diffuse swelling, purple discoloration, and severe pain due to excessive Heart Fire-Toxin; black sinking indicates an unfavorable sign (Ni), while red vibrancy indicates a favorable sign (Shun).
[Note] This condition occurs on the lateral chest above the breast, and may also occur all over the body, caused by excessive Heart Fire-Toxin. The lesion is purple, diffusely swollen, and painful, accompanied by chills and fever. Initially, there are six or seven pores, which gradually expand to three or five cun, or even six or seven cun. The shape resembles a honeycomb, with dozens of openings, each discharging yellow-white pus. The swelling spreads, and the entire wound surface becomes necrotic. When the necrotic tissue falls off and new flesh grows, appearing red, fresh, and moist, it is a favorable sign (Shun); if black fluid is discharged, the odor is foul, and the lesion is flat, it is an unfavorable sign (Ni). Throughout the course, internal and external treatments follow the regulations in the "Carbuncles and Ulcers - Swelling Stage" and "Ulcer Stage" sections. For patients with Qi-Cold constitution, if the lesion does not suppurate by the eighth or ninth day, Shen Deng Zhao Fa (Divine Lamp Moxibustion) should be applied daily until it suppurates at the expected time.
Du Ju (Maggot Carbuncle) occurs in the Quepen (ST12) acupoint. Initially resembling a mung bean, it gradually enlarges to the size of a plum, becoming hard, purple, and painful. It is accompanied by alternating chills and fever and dysuria. This condition is generated by accumulated Heat in the Gallbladder and Stomach meridians.
[Note] Also known as Quepen Ju or Clavicular Carbuncle, this condition occurs above the chest and below the neck, in the soft depression inside the clavicle at the Quepen (ST12) acupoint. It manifests due to accumulated Heat in the Gallbladder and Stomach meridians. Initial symptoms include alternating chills and fever, stiffness and tension of sinews and bones, loss of appetite, abdominal distension, and scanty, dysuric urine. Initially resembling a mung bean, it gradually enlarges to the size of a plum, becoming purple, hard, and painful. Initially, moxibustion with moxa cones placed on slices of single-head garlic should be performed. Internally, Duoming Dan should be administered to induce sweating, followed by Liuyi San to promote urination. As suppuration approaches, Neituo Huangqi San should be taken. In cases of severe Qi and Blood deficiency, Shiquan Dabu Tang should be used to support the healthy Qi and expel toxins. All other internal and external treatments follow the regulations in the "Carbuncles and Ulcers - Swelling Stage" and "Ulcer Stage" sections. This condition requires urgent supportive treatment; if neglected, leading to internal sinking of rot, the wound will fail to close, inevitably resulting in a fatal stage (Bai Zheng).
