Sitting Board Sores occur on the buttocks and thighs, resembling millet grains in size, accompanied by itching and pain. This is caused by the condensation of summer heat, dampness, and toxic heat within the muscles. Initially, it is appropriate to use scalding washes and oil-soaked moxa fumigation.
[Note] This condition is also known as Wind Chancre (Feng Gan). It arises between the buttocks and thighs, resembling millet grains, with redness and itching; in severe cases, it becomes painful and spreads to the anal region, feeling like a burning sensation. It results from sitting on furniture or stools heated by the sun during summer, or from prolonged sitting in cold and damp places, leading to the stagnation of summer-heat, dampness, and toxic heat in the muscles. Initially, one should decoct Genkwa Flower (Yuan Hua), Sichuan Pepper (Chuan Jiao), and Phellodendron Bark (Huang Bo) for a scalding wash, which can eliminate the condition. If the toxicity is severe and itching and pain persist, take a strip of blue cloth three fingers wide from an oil vat, mix one qian of Realgar powder (Xiong Huang) with sesame oil, spread it on the cloth, roll it up, ignite it, blow out the flame, and fumigate the sore with the smoke. The itching and pain will stop immediately, with excellent efficacy.
Bone-Attached Abscess (Fu Gu Ju) occurs on the lateral side of the thigh, while Bone-Biting Abscess (Yao Gu Ju) occurs on the medial side. When the body is deficient, cold-dampness invades through the deficiency, causing alternating chills and fever without redness at the site. The pain is severe, penetrating to the bone, making flexion and rotation difficult. If cold-dampness transforms into heat, swelling and puffiness appear. Garlic-moxa therapy can be effective if there are no adverse signs; after ulceration, the discharge of foul, watery pus must be avoided.
[Note] These two conditions arise on the inner and outer sides of the thigh. The lateral side belongs to the Foot Yangming, Foot Taiyang, and Foot Shaoyang meridians (Foot Three Yang Meridians), while the medial side belongs to the Foot Taiyin, Foot Jueyin, and Foot Shaoyin meridians (Foot Three Yin Meridians). Bone-Attached Abscess arises on the lateral thigh, and Bone-Biting Abscess arises on the medial thigh. In individuals with weak constitutions, sleeping exposed to wind and cold, cooling off after bathing, or having insufficient covering after sexual intercourse allows cold pathogens to invade the interior through deficiency, resulting in this disease. Initially, there is alternating chills and fever, similar to a wind-cold common cold, followed by bone and tendon pain without heat or redness. In severe cases, the pain feels like needle pricks, and the bones and tendons cannot flex or extend. After a long duration, extreme yin generates yang, and stagnant cold transforms into heat. Severe heat corrupts the flesh into pus. Externally, there is swelling and puffiness without a head, skin color remains normal, gradually revealing a bright red spot, indicating internal pus formation. For treating this condition, when alternating chills and fever occur and pain is felt initially: mild cases should take Wan Ling Dan immediately; severe cases should take Wu Ji San (containing Achyranthes Root/Niu Xi and Safflower/Hong Hua); the painful area should be needled with Thunder-Fire Needles (Lei Huo Zhen) to induce sweating, dispel cold, and unblock the meridians; once pus forms, it should be incised. Post-ulceration treatment follows the Ulcer Section.
For diffuse swelling and pain occurring in the sacral and gluteal region, Nei Tuo Qiang Huo Tang is appropriate. For those occurring on the medial thigh near the knee, belonging to the Foot Taiyin Spleen and Foot Jueyin Liver meridians, Nei Tuo Huang Qi Tang is appropriate. For those occurring on the lateral thigh, belonging to the Foot Shaoyang Gallbladder meridian, Nei Tuo Jiu Jian Tang is appropriate. For those occurring on the anterior thigh, belonging to the Yangming Stomach meridian, presenting with dizziness, vomiting, inability to eat, chest oppression, irritability, and heat, Fu Ling Zuo Jing Tang is appropriate. For those occurring on the medial thigh, belonging to the Taiyin Spleen meridian, presenting with joint pain, limb stiffness, spontaneous sweating, shortness of breath, difficult urination, and edema of hands and feet, Fu Zi Liu Wu Tang is appropriate. For those occurring on the posterior thigh, belonging to the Foot Taiyang Bladder meridian, presenting with leg and foot spasms and numbness, heavy and painful joints, aversion to cold with fever, absence of sweat, aversion to wind, or headache, Ma Huang Zuo Jing Tang is appropriate. For cases of deficiency in the Three Yin meridians with excessive external pathogens, where the entire thigh is swollen, skin color is unchanged, pain increases daily, and neither resolution nor suppuration occurs, this belongs to deficiency-cold and cold bones. Urgently take Da Fang Feng Tang to tonify deficiency and drive out cold. If resolution fails over time and suppuration is imminent, apply garlic-slice moxibustion externally; blistering is auspicious. If no blisters form and no heat is felt in the bone, it is an adverse sign. After moxibustion, take Shi Quan Da Bu Tang added with Niu Xi, Qiang Huo, and Fang Ji, or Ba Zhen Tang added with Fu Zi for tonification and support. When pus forms with distending pain, puncturing yields sticky white pus, which is favorable; if white serous fluid or bean-water-like discharge occurs, it is foul pus, indicating a critical prognosis. Internal and external treatments for these ulcerated conditions follow the Carbuncle and Ulcer Section.
All the above conditions stem from deep-seated cold and chronic cold. External applications and internal medications must not use bitter-cold herbs that damage the spleen and drain qi. Violating this leads to ice-like coagulation of qi and blood, internal corruption of flesh, turning into sewage over time, becoming incurable. According to books such as *Zhun Sheng*: After sweating in Shanghan (Cold Damage), residual pathogens form Liuzhu (Flowing Deposits); bad outcomes of Liuzhu become Bone-Attached Abscess. However, post-sweating Liuzhu is easy to cure; only mismanagement leads to bad outcomes that cannot recover, seemingly unable to transform into Bone-Attached Abscess. Moreover, Bone-Attached Abscess is curable with proper regulation. If it undergoes multiple transformations, it becomes worse and worse! How could it then achieve success? Therefore, the theory that bad outcomes of Liuzhu transform into Bone-Attached Abscess should be retained but not discussed.
