Guyin Ju (Abscess of the Inner Thigh) occurs in the center of the large thigh, presenting with hard swelling and pain on the side of the scrotum. It is caused by disharmony of the Seven Emotions (Joy, Anger, Worry, Pensiveness, Sorrow, Fear, Fright), specifically grief and indignation. After ulceration, the condition lingers and is difficult to heal.
[Note] This syndrome is also known as Chishi (Red Spread). It occurs on the side of the inner thigh near the scrotum, below the Hefeng acupoint. Because it lies predominantly in the Jueyin Meridian (Liver Meridian of Foot-Jueyin), it is called Dagu Ju (Large Thigh Abscess). The area is hard, diffusely swollen, numb, and painful. It results from the condensation of Qi and Blood due to emotional imbalance, including worry, thought, anger, and depression. Since the lesion is in a Yin Meridian, its onset is slow, suppuration is delayed, and after ulceration, it tends to linger; few cases achieve complete healing. The initial treatment is the same as that for Fugu Ju (Abscess adhering to bone). For swelling and ulceration stages, refer to the sections on Swelling Ulcers and Ulcerated Ulcers in the Treatise on Carbuncles and Sores.
Heng Ju (Transverse Abscess) occurs on the left, and Yin Ju (Yin Abscess) occurs on the right. There is hard swelling and pain in the crease of the inner thigh. Pain radiates to the testicles, and the shape resembles a clam. This is caused by stagnation and condensation in the Three Yin Meridians (Spleen, Kidney, Liver) due to emotional depression.
[Note] Both syndromes occur in the creases of the inner thigh. The left is called Heng Ju, and the right is called Yin Ju. They belong to the Three Yin Meridians and result from condensation due to emotional stagnation. The area is diffusely swollen and hard, with intermittent pain. In severe cases, pain radiates to the testicles and extends upward to the lower abdomen. The shape is long like a clam. It takes one to two months to ulcerate, indicating deep pus. If the pus is thick upon rupture, recovery is possible; however, if it is thin and foul (败浆), healing is extremely difficult, often leading to fistula formation over time. Initial treatment is the same as for Fugu Ju. Post-ulceration management follows the section on Ulcerated Ulcers. If pus drains continuously for a long time, worms may appear, resembling roundworms. This is transformed from long-standing stagnation of deep pus and is considered a adverse sign (Ni Zheng).
Ju at the Futu Acupoint is contraindicated. Hard swelling here makes acupuncture and moxibustion unsuitable. Pain is severe enough to penetrate the heart, accompanied by alternating chills and fever. It is caused by stagnation of Stomach Fire and toxic pathogens, making post-ulceration treatment difficult.
[Note] The classics state: "Sores should not arise at Futu." Futu is an acupoint of the Stomach Meridian, located 6 cun directly above the knee cap, where the muscle mass is large and palm-sized when tensed. Acupuncture and moxibustion are strictly prohibited here. Initially, there are alternating chills and fever, and pain penetrating the heart, caused by the stagnation of Stomach Fire and toxins. Healing after ulceration is the most difficult. Initial treatment is the same as for Fugu Ju. Post-ulceration management follows the section on Ulcerated Ulcers.
