You Yong (Deep Abscess) arises seven cun above the umbilicus. Initially, the swelling is small but gradually increases in size, becoming swollen, hard, and painful. If caused by excessive worry or consumption of rich and greasy foods leading to the eruption of fire-toxin, symptoms such as teeth grinding and chills may occur, indicating toxin penetration and internal attack.
[Note] This condition occurs seven cun above the umbilicus. At onset, it resembles a millet grain, gradually developing into a diffuse swelling with pain, shaped like a goose egg. In severe cases, it becomes hard, and the pain radiates to the chest and ribs. It results from overconsumption of rich, fatty, and flavorful foods, combined with emotional stress causing Qi stagnation and intestinal blockage. Fire accumulates and transforms into toxin, erupting from the interior to the exterior. Initially, there is swelling and pain, but the skin color remains unchanged. If accompanied by mental restlessness, vomiting, and belching, with a deep and solid pulse, one should dissipate fire-toxin to eliminate the source. The appropriate prescription is Neishu Huanglian Tang (Internal Draining Coptis Decoction). If swelling and pain are severe, indicating excess pathogenic factors, Tuoli San (Support the Interior Powder) should be taken, and moxibustion with moxa cones separated by garlic slices should be applied externally. If the pulse is deep and slow, indicating that pus has not yet formed, Buyi Yiqi Tang (Tonify Middle and Augment Qi Decoction) should be used to support the body and expel toxins. If the pulse is surging and rapid, indicating that pus has formed, Tuoli Tou Nong Tang (Support the Interior and Penetrate Pus Decoction) should be used. If the mature pus causes distending pain without rupturing, it indicates deficiency of Qi and Blood. Urgently use Shiquan Dabu Tang (Ten Great Tonifying Decoction) for warming and tonification. Externally, employ horizontal needle incision (horizontal needle refers to oblique insertion and withdrawal to prevent damage to the inner membrane). Alternatively, if sweating or purgative methods were mistakenly used, or cold-natured herbs were mistakenly applied topically, resulting in swelling without rupture or rupture without healing, urgently use Shiquan Dabu Tang supplemented with Ganjiang (Dried Ginger) and Fuzi (Prepared Aconite Root) to rescue the condition. After rupture, morning chills and evening fever indicate Qi and Blood deficiency; reduced appetite and diarrhea indicate Spleen and Stomach deficiency; chest stuffiness and profuse phlegm indicate Spleen and Lung deficiency. All these conditions should be treated with Liujunzi Tang (Six Gentlemen Decoction). After symptoms subside, switch to Shiquan Dabu Tang for conditioning and recovery. For external treatment, refer to the sections on abscesses and ulcers. Regardless of whether the lesion is ruptured or not, sudden teeth grinding and chills indicate Qi deficiency failing to overcome the toxin, signaling toxin penetration and internal attack. If after rupture the pus flow fluctuates significantly, or the wound opening resembles crab bubbles, it indicates penetration of the inner membrane. Both are adverse signs.
Tuoli San (Support the Interior Powder)
Cistanche Stem (Zaocis), Honeysuckle Flower (Jinyinhua), Scutellaria Root (Huangqin), Oyster Shell (Muli), Angelica Sinensis Root (Danggui), Red Peony Root (Chishao), Mirabilite (Puxiao), Rhubarb (Dahuang), Trichosanthes Root (Huafen), Forsythia Fruit (Lianqiao) (heart removed, equal parts).
Grind into coarse powder. Take 5 qian per dose, decoct with one cup of wine and one cup of water until reduced to 0.8 cups. Strain and take orally.
[Prescription Song] Tuoli San treats various sores and toxins; decoct and take when swelling and pain are severe to eliminate them. Contains Zaocis, Jinyinhua, Huangqin, Muli, Danggui, Chishao, Puxiao, Dahuang, Huafen, Lianqiao.
