Nasal sores (Bichuang) mostly occur in children, presenting as macerated and ulcerated skin on both sides below the nose. The etiology is generally attributed to wind-heat pathogens invading the Lung meridian. Symptoms include exudation of pus causing maceration, accompanied by itching but no pain.
[Note] This condition mostly occurs on both sides below the nose in children, with the affected area appearing purple and ulcerated, caused by wind-heat pathogens lodging in the Lung meridian. With pus exuding and macerating, it presents as itching without pain. It is appropriate to take Zexie San (Alisma Powder) internally and apply Qingge San (Green Clam Powder) externally for cure.
Zexie San (Alisma Powder)
Ingredients: Alisma Rhizoma (Zexie), Curcuma Aromatica Rhizome (Yujin), Gardenia Fruit (Zhizi, raw), Licorice Root (Gancao, raw), each 1 qian.
Preparation and Administration: Grind the above ingredients into powder together. Take 1 qian per dose, mixed with decoction of Licorice Root.
[Poem] Zexie San treats nasal sores, addressing pus exudation and Lung fire toxicity; grind Alisma, Curcuma, Gardenia, and Licorice into powder, and administer with Licorice decoction.
Qingge San (Green Clam Powder)
Ingredients: Calcined Tridacna Shell Powder (Ge Fen, 1 liang), Indigo Naturalis (Qingdai, 3 qian), Calcined Gypsum Fibrosum (Shigao, 1 liang), Calomel (Qingfen), Raw Polygonum Cuspidatum Rhizome Powder (Huangbai, each 5 qian).
Preparation and Administration: Grind the above ingredients into fine powder. First mix with sesame oil to form a paste, then add cool water to thin it. Apply thinly to the sore area.
[Poem] Apply Qingge San to eliminate nasal sores; use calcined clam powder, Indigo Naturalis, and calcined gypsum; grind Calomel and Polygonum Cuspidatum rhizome to extreme fineness, mix with sesame oil into a block, and dilute with cool water.
Another type of nasal sore arises from Lung heat, occurring inside the nose, presenting with dryness like fire burning, slight swelling, and pain. Internal administration of Huangqin Tang clears the heat, while external application of Huanglian Gao moistens and relieves pain effectively.
[Note] This condition occurs inside the nasal cavity. Initially, there is a sensation of dryness and pain, resembling millet grains. In severe cases, the external nose becomes red and slightly swollen, with pain feeling like fire roasting. This is due to stagnation of heat in the Lung meridian, which attacks upward to the nasal orifice, gathers, and fails to disperse, resulting in this sore. Internally, Huangqin Tang (Scutellaria Decoction) is suitable to clear the heat. Externally, Chensha Dingtong San (Cinnabar Pain-Stopping Powder) adhered to an oil-paper wick can be inserted into the nasal cavity. If dryness is present, applying Huanglian Gao (Coptis Ointment) yields immediate results.
Huangqin Tang (Scutellaria Decoction)
Ingredients: Scutellaria Root (Huangqin, wine-fried, 2 qian), Licorice Root (Gancao, raw, 5 fen), Ophiopogon Tuber (Maidong, dehearted, 1 qian), Mulberry Root Bark (Sangbaipi, raw, 1 qian), Gardenia Fruit (Zhizi, with skin, wine-fried, 1.5 qian), Forsythia Fruit (Lianqiao, dehearted), Red Peony Root (Chishao), Platycodon Root (Jiegeng), Mint Herb (Bohe), Schizonepeta Spike (Jingshesui), each 1 qian.
Preparation and Administration: Decoct in water and take after meals.
[Poem] Huangqin Tang treats excessive Lung fire, nasal sores with redness, swelling, and pain; contains Scutellaria, Licorice, Ophiopogon, Mulberry root, Gardenia, Forsythia, Red Peony, Platycodon, Mint, and Schizonepeta.
Chensha Dingtong San (Cinnabar Pain-Stopping Powder)

Preparation and Administration: Grind together into fine powder.
[Poem] Chensha Dingtong San treats dry nasal sores; grind Borneol and Picrorhiza into powder; insert into the nasal cavity using an oil-paper wick to relieve pain, dissipate heat, and achieve miraculous efficacy.
Huanglian Gao (Coptis Ointment)
