Scrotal Wind (Scrotal Eczema) is caused by the liver meridian contracting wind-dampness. The condition presents with itching, and a preference for bathing in hot water; in severe cases, nodules appear with numb skin, which may rupture and exude serous fluid. It is advisable to fumigate and wash the area twice with Shechuangzi Decoction (Cnidium Monier Decoction), after which it will heal.
Shechuangzi Decoction (Cnidium Monier Decoction)
Shechuangzi Decoction consists of Dangguiwei (Tail of Angelica Sinensis Root), Kushen (Sophora Flavescens Root), and Weilingxian (Symplocos Glauca Stem).
Decoct them together in river water, first fumigating the affected area, then soaking and washing when the temperature is suitable, twice to achieve relief from itching.
This formula treats conditions caused by wind-damp-heat in the scrotum, presenting with nodules and itching that hurts upon scratching, suitable for external washing.
Herbs: Shechuangzi (Cnidii Fructus), Dangguiwei (Radix Angelicae Sinensis Cauda), Weilingxian (Stem Symplocos Glaucae), Kushen (Radix Sophorae Flavescentis) (each 5 qian).
Use five bowls of water, boil several times until boiling, pour into a basin, fumigate first, then soak and wash when warm; two treatments are sufficient for cure.
Yelangdu Gao (Wild Wolf Poison Ointment)
Yelangdu Gao contains Fengziron (Fengzi Meat), Binglang (Areca Nut), and Wubeizi (Galla Chinensis) in equal amounts.
Add Chuanjiao (Sichuan Pepper), Chuangzi (Cnidium Fruit), Liuhuang (Sulfur), and Zhudannao (Pig Gallbladder Bile); add Xiaokun (Niter) to the boiling oil mixture.
Indications are the same as above.
Herbs: Yelangdu (Daphne Genkwa Stem), Binglang (Arecae Semen), Liuhuang (Sulfur), Wubeizi (Galla Chinensis), Chuanjiao (Zanthoxyli Pericarpium), Fengziron (Semend Feroniae Limoneae), Shechuangzi (Cnidii Fructus) (each 3 qian).
Grind the above herbs into powder. Use one large cup of Xiangyou (Sesame Oil), boil until bubbling, add three qian of Xiaokun (Natrii Sulfas), boil again; then add one Gongzhudannao (Male Pig Gallbladder Bile), mix evenly with the herbal powder, and apply to the affected area. This ointment is effective for various itchy sores.
Jie (Scabies) is a minor disease. Its onset causes incessant scratching, yet its origin is unknown. Scabies says: "I arise without root, form without mother, nurtured by the qi of yin and yang, taking shape through damp-heat. I often appear on the palms of nobles, why not on the bodies of scholars and commoners? I can make writers put down their pens and embroideresses stop their needles, regardless of status, affecting all equally." The physician asks: "Why do you attack only the hands and not other parts of the body?" Scabies replies: "The palm is governed by Taiyin Damp Earth, and the center of the palm is controlled by Shaoyang Ministerial Fire. Earth generates me, and Fire transforms me; my growth relies on the transformation of Damp Earth and Yang Fire, thus I necessarily emerge from the palms." The physician says: "Indeed! But your nature is dynamic rather than static, advancing but not retreating, implying you are not formed by clear qi. Furthermore, the spleen governs digestion and absorption, and the stomach governs transmission and transformation. Human diet rarely avoids rich flavors; within these rich flavors, damp-heat arises together, causing this sore. Clear qi circulates with the meridians, while turbid qi stagnates and does not disperse, remaining in the skin and muscles. Over days, it fails to resolve, subsequently generating heat and itching. Thus, within the itch and heat, dampness and fire mix to form insects (parasites). The form changes with dampness, movement changes with fire; this is the transformation of the lifeless into the living. After formation, they hide in the skin, moving around, causing itching and piercing sensations, reproducing endlessly, spreading throughout the limbs. Recently, they become Jie-Xuan (Scabies and Tinea); if prolonged, they become Tanfeng (Stubborn Wind), often leading to withered skin, infiltrating the blood vessels, causing extreme itching relieved only by hot water. Externally, Xiuyuan Wan (Embroidery Ball Pill) can be applied to stop itching and kill parasites; internally, Xiaofeng San (Wind-Dispersing Powder) can disperse wind and cool the blood. One must strictly avoid certain foods, refrain from spicy and hot items, and avoid washing with hot water; the irritation will naturally subside. This is a trivial ailment, not worth detailed exposition; these are idle words written casually."
