When treating a carbuncle of the stomach (Wei Wan Yong), one should observe the Stomach pulse. The Renying point's pulse reflects the Stomach meridian. If the pulse is deep and fine, Qi rebellion exacerbates the condition; when severe, heat accumulates at the gastric opening, forming a carbuncle. If the pulse is slippery and rapid, pus has already formed; if the pulse is slow and tight, although pus has not yet matured, blood stasis is already present, requiring urgent treatment. Otherwise, pathogenic toxins will attack internally, causing rotting of the intestines and stomach, which would be incurable. Use Shegan Tang (Belamcanda Decoction): Belamcandae Rhizoma (Shegan, 射干) [removed hairs], Gardeniae Fructus (Zhizi, 栀子), Poria Cocos (Chifuling, 赤茯苓), Cimicifugae Rhizoma (Shengma, 升麻) (each 1 liang), Paeoniae Radix Rubra (Chishao, 赤芍) (1.5 liang), Atractylodis Macrocephalae Rhizoma (Baizhu, 白术) (0.5 liang). Cut the herbs, decoct in water, add Rehmanniae Radix Preparata juice (Dihuang Zhi, 地黄汁) 1 he, and honey 0.5 he, take warm. Shaoyao Gancao Tang (Peony and Licorice Decoction) or Shengma Tang may also be selected as appropriate.
An infant suffered from white sores on the head, with shiny and tight skin, unresponsive to various medicines. This is called Brain Gangrene Sore (Nao Gan Chuang), caused by fetal toxin combined with wind-heat. It was cured by administering Longdan Wan (Gentian Pill) and Aloe Vera powder into the nose, along with external application of Jiedu San (Detoxification Powder). In severe cases, hair matts into tufts, the head feels hot like fire, the whole body sweats, the fontanelle swells and eyelids bulge high, which requires particular attention.
An infant presented with cough, wheezing and upward reversal of Qi, high fever with aversion to cold, skin resembling millet seeds, itchy nose with runny discharge, throat discomfort, cheek ulceration with hematemesis, abdominal distension, dry and brittle hair, and diarrhea. This is called Lung Gangrene (Fei Gan), treated and cured with Dihuang Qingfei Yin (Rehmannia Lung-Clearing Decoction) and Huachong Wan (Worm-Transforming Pill).
An infant cried frequently with sparse eyebrows, vomited clear foam, had abdominal pain and distension, visible blue veins, purple-black lips and mouth, and anal itching. This is called Roundworm Gangrene (Hui Gan), cured by taking Dalmaluoya Wan (Large Aloe Pill).
Parasites erode the spine and back, causing postprandial heat, yellow complexion, emaciation, irritability, warmth, and dysentery. Patting the back produces a drum-like sound, the spinal bones protrude like saw teeth, and ulcers appear on the ten fingers with frequent nail-biting. This is Spinal Gangrene (Ji Gan), also suitable for treatment with Dalmaluoya Wan (Large Aloe Pill).
A ten-year-old child suffered from scabies that did not heal for a long time, with emaciation, alternating chills and fever, hot head and cold feet, loose stools, abdominal pain, gum ulceration, foul breath, dry peeling skin, black nails, and sallow complexion. This is Kidney Gangrene (Shen Gan), cured by taking Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) and external application of Jiedu San (Detoxification Powder).
A person suffered from "Ghost Strike" (sudden severe pain with bruise-like marks), experiencing pain with green marks on the body. Drinking decoction of Honeysuckle Flower (Jinyinhua, 金银花) resulted in immediate cure.
A woman suffered from Lijie Feng (Joint-Wandering Wind), presenting with chills, fever, and joint pain. After taking Renshen Baidu San (Ginseng Detoxification Powder), she experienced generalized body pain the next day, unable to turn over. She stated she had this chronic condition before, which subsided spontaneously after more than a month, and medication was ineffective. Due to her deficient constitution, wind-pathogen qi entered, traveled with the blood, overflowed into the skin, and suddenly caused pulling pain that moved unpredictably. This is named Lijie Feng (Joint-Wandering Wind), cured by treatment with Sisheng Wan (Four Freshnesses Pill). (This diagnosis was based on pulse.)
Another noble lady first felt pain in both knees, which later spread to all bone joints throughout the body. Her pulse was slow and moderate. Qianghuo Shengshi Tang (Notopterygium Decoction for Overcoming Dampness) and Jingfang Baidu San (Cyclamen and Saposhnikovia Detoxification Powder) plus damp-draining herbs were ineffective. Subsequently, Fuzi Bawu Tang (Aconite Eight-Ingredient Decoction) administered in one dose resolved all symptoms, and continued use led to recovery. (This diagnosis was based on pulse.)
If the pulse is surging and rapid with pain, Renshen Baidu San (Ginseng Detoxification Powder) is indicated. If toxic pathogens cause pain starting from hands and feet spreading to the whole body, or cervical lymph nodes swell like beads, this is a syndrome of Wind-Dampness Flowing Qi. Jiawei Xiao Xuming Tang (Modified Small Xu Ming Decoction) and Duhuo Jisheng Tang (Angelica Pubescens and Taxillus Decoction) are indicated.
A person over fifty years old suffered from flaccidity of sinews and bones, bedridden for five years, with generalized pruritus worse in the afternoon. Treatment with blood-nourishing herbs gradually relieved the itching and slightly improved the flaccidity. Further treatment with Jiawei Sijin Wan (Modified Four Jin Pill) and careful regulation led to recovery after more than a year. (This diagnosis was based on symptoms.)
Someone asked: Why use warm tonification when heat is rampant internally? The reason is that blood deficiency fails to nourish the sinews, leading to laxity and inability to support the body. Yang hyperactivity and internal heat require nourishing Yang and Yin. When Yang becomes substantial, Water ascends and Fire descends, achieving balance.
