The whites of the eyes appear blue-black, and the pupils are small. Vomiting occurs after taking medicine. Abdominal pain is accompanied by extreme thirst. The shoulders and neck are stiff and difficult to move. The voice is hoarse, and the complexion is sallow and devoid of luster. These are referred to as "Five Rebellions" (Wu Ni). Other symptoms such as fever, thirst, diarrhea, and vomiting are generally caused by toxic pathogens entering the interior, damaging the Zang-Fu organs. (Note by Wang Ji: The above conditions are all incurable because the Qi of the Five Zang organs has already exhausted.)
1. Futu (ST32); 2. Calf region; 3. Back; 4. Shu points of the Five Zang organs; 5. Neck/Upper back region; 6. Brain/Cranium; 7. Mustache area; 8. [Text missing]; 9. Chin/Jaw area.
The first location enters the hairline at Yuzhen (GB17), which is also the root of the tongue. The second is the cervical vertebrae segment. The third vertebra is Chonggu. The fourth Dazhui (GV14) corresponds to the Five Zang organs. The fifth is Feishu (BL13) on the side of the spine. The sixth is on both sides of the spine (Geshu BL17 and Ganshu BL18). The seventh is the two Shenshu (BL23) points on both sides of the spinal column. The eighth is Jiwei (CV15) at the posterior heart. The ninth is Jiwei (CV15). Appendix: The five locations on the front surface should not develop carbuncles or sores.
The first location, the laryngeal bone, is Chuixing. The second location, in the chest, is Shen She. The third is Xinjiwei. The fourth is the point between the two breasts. The fifth is Changqu, located two cun below the umbilicus. Appendix: The three locations on the lateral surface should not develop carbuncles or sores.
Below the ear, near the angle of the jaw behind the ear, in the central depression, is Houmai, one acupoint. Below the arm, there is one point corresponding to the shoulder bone. Three cun above Chengshan (BL57), there is one point belonging to the Intestine meridian.
The brain is the meeting place of all Yang meridians; if an acupoint here ulcerates, marrow will flow out. The neck is close to the throat; if obstructed, neither medicine nor food can be ingested. Shenshu (BL23) opposes the Kidney and is the seat of life; if an acupoint here ulcerates, it penetrates through to empty space. Carbuncles occurring in these three locations are difficult to treat.
If a back carbuncle penetrates the membrane, it is incurable (this refers to areas above Ganshu BL18). If the flesh sinks before ulceration, the face is cyanotic, lips are black, and stool contains stagnant blood, death ensues (this refers to organ failure causing bloody stool). If the sore is on the right side within one cun and three fen, and the toxin is sharp/aggressive, it is incurable. If it ulcerates into the throat, it is incurable. If the toxin enters the abdomen, it is incurable. If it enters the scrotum, death ensues. If the temporal hairline involvement extends deeper than one cun, it is incurable. If the patient suffers from stiffness and spasms of the waist and back due to the sore, it is incurable.
When a carbuncle erupts on the back, if the hands cannot reach upwards to touch it, it is called "Left-Right Da" (Zuo You Da). If the head of the sore resembles a honeycomb, it is easy to treat. If the hands cannot reach downwards to touch it, it is called "Waist Carbuncle" (Yao Ju), which is also easy to treat.
If the hands cannot reach either up or down, it is called "Fa Bei" (Back Carbuncle). This condition is the most severe.
Generally speaking, the locations mentioned above are where meridians converge and internally connect with the Zang-Fu organs. If patients report their condition early, and physicians carefully examine the syndrome and treat according to the principles, they will not die. However, if treatment is delayed or incorrect, even if the sore appears in a location that is usually not fatal, it may still lead to death.
