[Discussion on Various Sores]
The term "various sores" refers to various types of minor skin lesions. Their names and clinical manifestations differ, but their etiology generally involves the failure of Heart and Kidney interaction (the non-interaction of Heart Fire and Kidney Water), irregular diet leading to food stagnation in the intestines and stomach, and the binding of Wind-Toxin with Qi and Blood, which coagulates and stagnates between the muscles, causing eruption. Whenever the human body is deficient and contracts Wind, Heat, Dampness, or Toxin pathogens, sores and ulcers erupt. Symptoms such as itching, pain, swelling, fever, and excessive sweating characterize a malignant sore (E Chuang). If a sore appears between the hands and feet, resembling a new cornel fruit, with itching, pain, and fissures, oozing yellow fluid when scratched, forming holes like insect erosion, and eventually producing worms, it is called Gan Chuang (a specific type of ulcerative sore). Alternatively, if it begins small like a grain of rice, gradually enlarges with a root, breaks open bleeding and suppurating, with flesh everted like a blooming flower, it is called Fan Hua Chuang (Flower-Everting Sore). If it starts very small, first itching then painful, with exudate spreading and macerating the muscle tissue until covering the whole body, it is named Jin Yin Chuang (Spreading Eczema-like Sore). If it occurs on both ears, nose, and face, rotting and extending to the lower orifices, invading the sinews and collaterals, worsening at the beginning of the lunar month and subsiding at the end because it follows the moon's phases, it is called Yue Shi Chuang (Moon-Eclipse Sore). If toxic qi attacks the fingers or toes, with flesh enveloping the nail, causing pain and bleeding, and worms present in the sore, it is called Jia Qu Chuang (Nail-Worm Sore). If the fingertip swells first, burning and pulling pain ensues, followed by pus formation at the nail edge, and in severe cases, total nail loss, it is called Dai Zhi (Paronychia). Some individuals have a constitution sensitive to lacquer; upon exposure, they suffer poisoning, facial itching and swelling, scratching leads to sores spreading over the body with purulent discharge, itching, and pain, known as Qi Chuang (Lacquer Dermatitis). During midsummer, pores open easily, allowing Wind-Heat-Toxin to bind to the skin; mild cases resemble scattered millet grains, while severe cases involve heat-sweat maceration forming clustered sores, known as Fei Chuang (Prickly Heat). If mental restlessness accompanies full-body eruptions that are red, raw, and fiery, it is named Re Chuang (Heat Sore). If contact with Wind-Cold causes blood stasis and obstruction, resulting in stubborn, insensate sores difficult to heal, it is called Leng Chuang (Cold Sore). If body sores swell but are neither carbuncles nor gangrenous ulcers, nor tinea nor scabies, yet resemble malignant sores with fluctuating severity, it is called Wu Ming Chuang (Unnamed Sore). If white patches appear on the head, scaling like tinea, with white scales turning into crusts, leading to widespread scalp sores with holes, pus, internal worms, minimal pain but slight itching, and chronic non-healing, it is called Tu Chuang (Tinea Capitis/Alopecia). If a woman's vulva develops chronic sores that spread, it is called Yin Shi Chuang (Yin Eroding Sore). If winter cold causes fissures on hands and feet, it is called Dong Chuang (Frostbite). Injuries from knives or axes are called Jin Chuang (Metal Wounds), and burns from fire or hot liquids are called Huo Chuang (Fire Burns). Although these sores have different names, their treatments are listed below; clinicians should examine carefully.
