The two eyes of the human body are like the sun and moon in the high sky. Although floating clouds may temporarily rise, they do not hinder the function of illumination; however, if fine opacities (cataracts) arise, clarity is lost by even a hair's breadth. Therefore, once formed, immediate treatment is required.
There are many morphological manifestations of internal cataract diseases, varying in severity. Some present as "ice-like," some as "astringent," some as "slippery," and some as "scattered." Ice-like opacities do not sink when manipulated; slippery opacities do not deepen when closed.
In such cases, acupuncture should be directed towards the area below the patient's head. Acupuncture performed through the nose is suboptimal due to nasal obstruction of hand movement. The patient should sit facing forward, holding the physician's belt firmly without letting go. First, use a blunt needle to stabilize the acupoint, allowing the eye to adapt and preventing rotation. Adjust breathing for fifty breaths (xi), then advance the needle slowly. The force should not be too heavy nor too light. Initially, apply gentle pressure; before entering the point, slightly increase the force. If the needle tip deviates, it may cause injury, leading to bleeding that follows the needle. Do not forcibly stop the bleeding, nor press heavily, as this may exacerbate hemorrhage. Gently wipe the area. Slow down breathing and apply steady pressure to expel blood stasis, which will stop naturally. If bleeding persists, severe damage has occurred; wait for coagulation to seal the puncture. Resume care according to standard protocols using medicinal therapy. Rotate the needle no more than the Zi-Wu meridian axis. If resistance feels rigid or urgent, the membrane layer has been entered. If the needle feels slippery upon release and has not fully penetrated, proceed with caution. Once deeply inserted, pain may occur; pause briefly if painful, then advance gradually. As the needle approaches piercing the membrane, pain intensifies; skillfully guide the needle through. Once pain subsides slightly, invert the needle tip towards the pupil (Tongren), aligning it level with the pupil, and gently manipulate downwards without excessive force. When near the pupil, vision may return. Upon seeing objects, close the eyelids by rolling them, and slowly withdraw the needle. Rest for fifty breaths. Prolonged rest leads to clearer vision. Once vision is distinct, seal the eye with cotton and follow dietary and lifestyle regulations to maintain optimal light exposure, allowing the opacity to re-ascend. Re-treatment can follow previous methods. If movement occurs after manipulation, stop according to symptoms. For localized pain, massage gently to alleviate. For severe unrelenting pain, apply heat therapy (fire moxibustion/warming). Before acupuncture, inform the patient to prevent sudden shock and vomiting. Prepare analgesic herbs: powder Da Huang (Rhei Radix et Rhizoma) and Mu Xiang (Aucklandiae Radix), mix with vinegar and water into a paste, form into cakes, and apply to the site to stabilize. If vomiting persists, hold dried plum (Bai Mei) in the mouth to promote saliva swallowing. Severe vomiting is difficult to control. All medications must be prepared in advance. Unbearable prolonged pain indicates tissue damage. After surgery, seal the eye with cotton for seven days. Consume fermented soybean porridge while lying supine; avoid turning or side-lying. Constant supervision is required. Avoid loud calls. Assist the patient gently during urination and defecation; avoid straining. Wash face is prohibited; avoid wind and rest properly. Open the seal on the seventh day. If vision remains white or frosty, it is due to ocular tenderness. The seal need not be completely removed; open briefly for viewing. If viewing causes eye pain, further damage may have occurred. After fourteen days, the seal may be fully removed. Sensations resembling flying insects or ribbons, or trembling like a suspended needle, indicate instability of the Shui Lun (Water Wheel/Pupil/Lens); oral medication will gradually resolve this. Itching after twenty-one days is normal. During surgery, the patient should neither be overly full nor hungry. Post-surgery, if pain occurs at specific locations, acupuncture can be applied bilaterally along the Du Mai (Governing Vessel), including Xi Feng and Feng Fu points. Needle-site pain typically resolves within two to three days. Avoid washing the face for one month to prevent water entry into the puncture wound. Use cotton soaked in saline solution to gently wipe. No solid food for seven days to avoid jaw movement affecting the Shui Lun; consume porridge and soft foods. Regardless of the type of cataract or complex symptoms, patients must avoid wind, direct sunlight, emotional extremes, sexual activity, pungent spices (Wu Xin), alcohol, wheat products, and roasted/fried toxic foods. Maintain a calm temperament and careful self-care for recovery. Indulgence and violation of taboos lead to self-inflicted harm.
