江应宿见一木商,自云曾经五月放树,久立风雨湿地,衣服尽濡,患寒热交作,遍身胀痛欲人击打,莫知为何病;

Qi moves simultaneously; logically, there should be wind coming from both east and west at the same time. Why is this not the case? I replied: The heaven and earth constitute a unified qi. If the qi in the east is disharmonious and the qi in the west is also disharmonious, then belching (eructation) might originate from the east, and the disharmonious qi from the west would also discharge along with the eastern qi. Thus, there is only east wind. If it originates from the west, then there is only west wind. However, heaven and earth are the greatest entities. If one side is disharmonious and the other side is disharmonious, although they are extremely far apart and cannot correspond to each other, if belching occurs simultaneously in both places, the winds generated by them will be distant and distinct, which humans cannot perceive. As the proverb says: "The wind differs over a thousand li."
Another question was asked: What about thunder and lightning? I replied: Throughout the year, heat changes to cold, and cold changes to heat; these are all the primordial yang qi of heaven and earth. When ascending, it becomes heat; when descending, it becomes cold. When the solar term Jingzhe (Awakening of Insects) arrives, the qi gradually ascends and reaches the exterior. Because the qi of heaven and earth is disharmonious, and disharmony prevents ascent, the desire to ascend but inability to do so causes the qi to break through obstructions and emerge. Hence, thunder and lightning occur together. Lightning is the primordial yang qi, and qi is fire. Thunder arises because the fire of the primordial yang qi ascends outward; therefore, lightning usually appears before thunder (similar to fireworks, where light is seen before sound). If lightning is intense, thunder erupts violently outward, causing the thunderous sound to shake upwards, forming a thunderbolt. If thunder stops but lightning continues, this is residual qi after breaking through the interior. Thus, lightning flashes briefly without traveling far, and the qi dissipates easily. Therefore, as one flash ends, another begins. In early autumn, yang qi is about to收 (gather/restrain). If the qi of heaven and earth is disharmonious, the qi desires to descend but cannot, instead rebelling upward. Hence, sudden thunder often occurs. As autumn deepens, yang qi gradually descends. By winter, it hides and stores, so thunder also ceases its sound. Humans participate with heaven and earth. In autumn, human qi gradually gathers. Gathering closes the hair follicles (pores), and pathogens contracted in spring and summer become stagnant. When stagnation releases, disease manifests. Thus, diseases often occur in autumn.
Xu Shuwei stated in *Ben Shi Fang*: "I suffered from Yin Pi (accumulation due to fluid retention) for thirty years. It began because when I was young, I sat writing at night, leaning left on the desk, causing food and drink to accumulate on the left. At midnight, I drank several cups of wine and slept on my left side. In my prime, I felt nothing. After three or five years, I noticed the wine made sounds only on the lower left, accompanied by hypochondriac pain, reduced appetite, and gastric discomfort. Drinking half a cup stopped me. Every ten days or so, I vomited several sheng of sour water. In summer, I sweated only on the right side, with absolutely no sweating on the left. I visited many famous doctors, but no treatment worked. I speculated that there must be a pouch in my body like a hollow in the ground holding water; it does not flow until full. Only clear fluids can flow, while turbid fluids stagnate without a path to drain. Thus, after accumulating for five to seven days, vomiting relieves it. Spleen Earth dislikes dampness, and water flows with dampness. The best approach is to dry the spleen to remove dampness and strengthen the earth to fill the hollow. So, I discarded all medicines and used only one jin of Cang Zhu (Atractylodis Rhizoma), peeled, sliced, and powdered. Mixed with half a liang of Sesame Seed (Sesami Semen) and two qian of water, ground and filtered for juice. Fifty Jujubes (Jujubae Fructus) were boiled, peeled, pitted, and mashed to mix into pills the size of Sophora seeds. Taken warm on an empty stomach, fifty pills daily. Increased to one or two hundred pills. Avoided peaches, plums, and sparrow meat. After three months, the disease was cured. Since then, I rarely vomit, spit, or feel pain. My chest and diaphragm are open and smooth, eating and drinking are normal, sweating in summer is uniform all over the body, and I can read small print under a lamp. All this is due to Cang Zhu. Initially, I felt slightly dry, so I took Gardenia powder (Gardeniae Fructus) mixed with boiling water to counteract it. Long-term use naturally eliminated the dryness."
He also said: "Someone suddenly appeared dead while normally healthy: body motionless, eyes closed, mouth tight, or slightly conscious, dizzy and confused for a while before waking. This is called Blood Jue (Syncope due to Blood Stagnation) or Yu Mao (Depression and Dizziness). It is caused by excessive sweating leading to blood deficiency and direct upward rush of yang qi. Qi obstruction prevents movement, making the body seem dead. When qi passes and blood returns, yin and yang reconnect, so waking occurs after a while. Women suffer this frequently. Bai Wei Tang (Anemarrhenae Decoction) is suitable, using Bai Wei (Anemarrhenae Rhizoma) and Dang Gui (Angelicae Sinensis Radix) one liang each, Ren Shen (Ginseng Radix) half a liang, and Gan Cao (Licorice Radix) one qian and half. Take five qian per dose, decoct in two zhan of water to one zhan, and take warm."
*Meng Xi Bi Tan* records: "Lord Xia Wenzhuang had a luxurious nature and unique constitution. As soon as he fell asleep, his body became cold and stiff, like a corpse. Upon waking, someone had to warm him for a long time before he could move. Someone saw him walking with two carts side by side, carrying a towering object. Asked what it was, it turned out to be a cotton tent made of dozens of jin of cotton. He constantly consumed Xian Mao (Curculiginis Rhizoma), Zhong Ru (Stalactitum), and Liu Huang (Sulfur) beyond count. Every morning he ate Zhong Ru Porridge. A minor official stole some and developed a carbuncle that could not be saved."
*Chui Jian Xu Lu* records: "Ge Kejiu treated a wealthy woman from the same prefecture, aged seventeen or eighteen, suffering from Wei Bi (Flaccidity and Obstruction of the four limbs), unable to feed herself, staring blankly, with no cure found by other physicians. Ge examined her and said: 'Remove all fragrant items like cosmetic boxes and tassels from the bedroom. Dig a pit in the floor, place the girl inside, and close the screen. Warn family members to report to me when her hands and feet move and she cries.' After a long time, her hands and feet moved and she cried. One pill was administered, and she emerged from the pit the next day. This girl had a habit of loving fragrance, which eroded the Spleen."
Puyang Chuan recorded: "In the Dinghai year of the Wanli era, a woman in Jintai sold wool everywhere in the market and then disappeared. Soon, people in the capital developed blistering tumors that grew larger, with many dying of pain. Inside the tumors was only wool. A Daoist transmitted a formula: Powder Black Beans (Phaseoli Semen Nigrum) and Buckwheat (Fagopyri Esculenti Rhizoma) and apply externally. When the wool falls off, it heals. Named Yang Mao Ding (Wool Carbuncle)."
[Shen Yuan's Note]: This condition is common in Jiangnan and Shandong, but differs from the previous case. It starts with fever, resembling Malaria (Nüe Ji) or Cold Damage (Shang Han), heavy body, unbearable distending pain in skin and flesh all over, and red spots like rash. Puncturing with a needle reveals wool inside, either one strand or several, white or red, thick or thin, varying in shape and color. Commonly called Yang Mao Zhen Zi (Wool Rash). Many die. Some do not puncture but take internally Gegen (Puerariae Lobatae Radix), Shengma (Cimicifugae Rhizoma), Chai Hu (Bupleuri Radix), Fang Feng (Saposhnikoviae Radix), Jing Jie (Schizonepetae Spica), Niu Bang Zi (Arctii Fructus), Chan Tui (Cicadae Periostracum), Yin Hua (Lonicerae Japonicae Flos), Lian Qiao (Forsythiae Fructus), Huang Qin (Scutellariae Radix), Ling Yang Jiao (Capricornis Cornu), Xi He Liu (Salicis Tashiroanae Ramulus). Externally apply the previous case's medicine. The wool sticks to the medicine, and pain subsides after rubbing.
However, it may recur after one day, two days, or half a day. Rubbing with medicine brings back the wool, which gradually flattens. This may last one or two months, or three to five months, until white blisters appear all over and severe itching occurs, then recovery happens. Without this treatment, few survive. Another type presents only with fever, heavy body, and distending pain, without red spots. The same treatment applies: puncture to remove wool and release blood, curing the disease immediately. This is Yi Du (Epidemic Toxin); releasing the toxin cures it.
Li Dongyuan treated Lady Li Zheng. Her six pulses were all wiry (Xian), combined with surging and slow qualities, weak upon pressure, with wiriness above. This indicated Wind-Heat sinking into Yin, blocking the Yang pathway. Symptoms: closing eyes caused whole-body numbness, worse at night; opening eyes made numbness recede, eventually stopping. Keeping eyes open prevented symptoms; fearing numbness, she dared not close them, causing insomnia. Body heavy, occasional phlegm cough, feeling of stuck phlegm in chest, irritability, shortness of breath, robust skin, normal diet and excretion. Fear of numbness and inability to sleep was the greatest suffering. Color and pulse matched the disease without contradiction. *Nei Jing* states: "Yang excess leads to eye closure with light movement; Yin disease leads to eye closure with heavy stillness." Also: "All pulses relate to the eyes." *Ling Shu* states: "Opening eyes allows Yang pathway to function, circulating yang throughout the body; closing eyes blocks the Yang pathway, like day and night separation." Knowing yang is deficient and yin is abundant, and numbness is associated with Wind—even children believe this—careful analysis shows otherwise. Sitting long and rising causes numbness; binding with rope causes numbness and immobility, releasing restores movement. Thus, it is not Wind pathogen, but Qi stagnation. Do not treat Wind; tonify Middle Qi, and numbness will vanish. Knowing the meridians, Yin Fire attacks Yang division, moving internally causing numbness. Must clear Yin Fire. Phlegm cough is due to Autumn coolness outside and Dampness above; solidify exterior with warm agents. Heavy body and slow pulse indicate hidden Dampness; raise Yang, assist Qi, benefit Blood, mildly purge Yin Fire, remove Dampness, unblock meridians, regulate Yin and Yang. Not an original pathogen of the Five Zang. Main formula: Bu Qi Sheng Yang He Zhong Tang (Tonify Qi, Raise Yang, Harmonize Middle Decoction). Eight doses cured.
Treated another person who had a son in middle age. After one year, red-thread tumors appeared on his body. His four subsequent sons died of similar tumors. Asked why. Next month, he thought and said: "You have latent fire in Kidney. Essence and Qi must contain red threads, transmitted by Qi, causing this disease in offspring. Commonly named Tai Liu (Fetal Tumor). Observe carefully." He confirmed. Prescribed Zi Shen Wan (Nourish Kidney Pills) several doses to purge Kidney fire and supplement True Yin deficiency. Avoid alcohol and spicy-hot foods. Wife used Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia) to nourish Blood. After five months of pregnancy, used Huang Qin (Scutellariae Radix) and Bai Zhu (Atractylodis Macrocephalae Rhizoma) as powder, taken five to seven doses. Subsequent sons did not develop the condition.
Wang Shishan treated a man, tall and thin, pale and fragile, over thirty years old, with a strange disease. Whole body tingling, moving like insects, sometimes starting from left leg, ascending to head, descending to right leg, hearing insect sounds. Doctors did not know the disease. Wang palpated pulse: floating, small, and soft, insufficient upon pressure. Observing form and color indicated deficiency. *Shang Han Lun* states: "Like insects moving on body, indicates loss of Yang from excessive sweating." Following this example, used Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction), doubling Ren Shen (Ginseng Radix) and Huang Qi (Astragali Radix), assisted by five fen of Huang Bo (Phellodendri Cortex) fried in wine. Thirty doses cured.
Treated a woman who occasionally experienced whole-body numbness leading to confusion and unconsciousness, recovering after a long time. Doctors treated as Wind, using Wu Yao Shun Qi San (May Chang Qi-Regulating Powder) and Xiao Xu Ming Tang (Minor Extinction Brightening Decoction), worsening the disease. Wang palpated: pulses floating, soft, slow, and weak. Said: "This is Qi deficiency. Numbness is Qi failing to move; slowness fails to connect. Like sitting long with bent knees, Qi pathway blocked, standing up causes numbness. Heart nourishes Blood and houses Spirit. If Qi transport is poor, Blood rarely arrives. Heart loses nourishment, causing confusion." Used Ren Shen (Ginseng Radix) and Huang Qi (Astragali Radix) two qian each, Dang Gui (Angelicae Sinensis Radix), Fu Ling (Poria), Mai Dong (Ophiopogonis Radix) one qian each, Huang Qin (Scutellariae Radix), Chen Pi (Citri Reticulatae Pericarpium), Gan Cao (Licorice Radix) five fen each. Decocted and taken, cured.
Zhu Zhongning treated a noble lady suffering from aversion to cold, covering her head with heavy furs day and night, jumping into boiling water without feeling it. Doctors thought cold was severe. Zhu said: "This is Phlegm-Fire rushing upward, known as Extreme Yang resembling Yin." Must purge downward to kill fire. Overnight removed furs, called for water to drink. Ten days later, Qi calmed and recovered.
Danxi treated a woman near sixty, often shivering with cold in summer, preferring fierce fire and cotton, sweating like rain, fat and muscular, having taken Fu Zi (Aconiti Lateralis Radix Praeparata) over ten doses. Whole body very itchy, pulses deep and choppy, slightly large when pressed heavily. Indicated extreme heat and Blood deficiency. Used Si Wu Tang (Four Substance Decoction), removing Chuan Xiong (Ligustici Chuanxiong Rhizoma), doubling Shu Di Huang (Rehmanniae Radix Praeparata), adding Bai Zhu (Atractylodis Macrocephalae Rhizoma), Huang Qi (Astragali Radix), fried Huang Bo (Phellodendri Cortex), raw Gan Cao (Licorice Radix), one liang per dose. First decoction drunk. Severe diarrhea, blind, mute. Realized disease was deep and medicine lacked counter-agent guidance. Gave same medicine well-fried, borrowing fire power as guide. One dose stopped diarrhea, four doses restored spirit, ten doses fully healed.
Treated Jiang's wife, over thirty, thin and dark-faced, preferring heat and aversion to cold in June, pulses deep and choppy, rapid when pressed heavily. Purged with San Huang Wan (Three Yellow Pills), taken with Ginger Decoction, thirty pills per dose. Twenty doses, slight sweat, cured.
Treated a girl over fifteen, irritable and stout, coughing and reversing qi due to great anger in summer. Each sound caused whole-body trembling and fainting, occurring every three to five breaths. Pulse unpalpable, form and Qi indicated excess. Decocted two liang of Ren Shen Lu (Ginseng Stem) and drank. Vomited several bowls of sticky phlegm, profuse sweat, slept for a day, then calm.
Treated a man with lumps all over body, which are Phlegm. Er Chen Tang (Two-Cured Decoction) plus Bai Jie Zi (Sinapis Semen), fried Huang Lian (Coptidis Rhizoma) decocted and taken.
A man from Wu Prefecture had lumps on shoulder and back. Leaning left moved lump to left, leaning right moved to right. Treated as Phlegm-Fluid flowing and pooling. Removed Huang Lian from previous formula, added Jie Geng (Platycodonis Radix), cured.
Jiang Rujie treated an old woman with weakness, asthma, left half of body from head to foot hot with spontaneous sweating, unable to endure single clothes. Right half from head to foot cold, unable to be warmed by thick clothes. Three years, medicine ineffective. Wang palpated six pulses: lifting revealed all subtle and slightly wiry; pressing revealed slightly surging and weak. Two Guan pulses slightly stronger than two Cun pulses. Classics state: "Subtle indicates deficiency." "All wiry indicates Fluid retention." "Surging indicates Yang/Heat." "Weak indicates deficiency." Thus, Wind pathogen entered Spleen, exterior-interior Yin-Yang Qi-Blood all deficient. Classics state: "Treating disease must seek root. Wind pathogen means Wood invades Earth." Wind enters Spleen from Taiyang. First, vent Taiyang to secure exterior. Then nourish Spleen and suppress Wood, preventing Spleen from thief pathogen. Qi-Blood gradually calm, left-heat/right-cold disease removed. Shi Gao (Gypsum Fibrosum) and Kuan Dong Hua (Farfarae Flos) three qian each, Guan Gui (Cinnamomi Cortex), half amount of Gan Cao (Licorice Radix), powdered fine, blown into throat with tube, taken with strong tea three to four qian. Cough and asthma stopped. Next day, tonic agent: Bai Zhu (Atractylodis Macrocephalae Rhizoma) two qian half, Bai Shao (Paeoniae Radix Alba), Xiang Fu (Cyperi Rhizoma) one qian half each, Huang Qi (Astragali Radix), Chen Pi (Citri Reticulatae Pericarpium) one qian each, Gan Cao (Licorice Radix) three fen. Decocted and taken. Later removed Bai Shao, added Ren Shen (Ginseng Radix) three qian. Several doses cured.
Shizhen treated a man. Onset: distending pain at one point in right waist, radiating to right hypochondrium, up to chest, requiring lying down. Bowel urgency with heaviness, frequent defecation urge, long and frequent urination. Or acid regurgitation, or vomiting water, or diarrhea, or impotence, or relieved slightly by wine, or reduced slightly by food. Triggered immediately by cold exposure, cold food, labor, intercourse, anger, or hunger. Once stopped, all symptoms vanished as if healthy. Sometimes several times a day. Warm-tonifying damp-resolving, nourishing, and digesting medicines gave slight temporary relief. Shizhen thought: Hunger/fullness and labor injured Original Qi, Clear Yang trapped, unable to ascend. Used Sheng Ma Ge Gen Tang (Kudzu and Cimicifuga Decoction) combined with Si Jun Zi Tang (Four Gentlemen Decoction), adding Chai Hu (Bupleuri Radix), Cang Zhu (Atractylodis Rhizoma), Huang Qi (Astragali Radix). Decocted and taken. After taking, drank one or two cups of wine to assist. Medicine entering abdomen felt Clear Yang ascending, chest/diaphragm refreshing, hands/feet warm, head/eyes clear, spirit vigorous, symptoms swept away. One dose stopped each onset, incredibly effective. Reducing Sheng Ma/Ge Gen or not drinking wine delayed effect. Generally, after fifty, Qi consumption exceeds growth, descent exceeds ascent, Autumn/Winter commands exceed Spring/Summer. Those with weak constitution and these symptoms should use this active method. *Su Wen* states: "Where Yin Essence is offered, life is long; where Yang Essence descends, life is short."
Sun Dongsu treated a noble lady in Shizhou. Whole body migrating pain, worse at night, like insects biting muscle, often attributed to ghosts. Sun said: "This is exactly Li Jie Feng (Joint-Wandering Wind)." Three doses of She Xiang Wan (Musk Pill) cured.
Jiang Yingsu saw a timber merchant who claimed spending five months felling trees, standing long in wind/rain/wetland, clothes soaked. Suffered alternating chills and fever, whole-body distending pain wanting to be beaten, unknown disease. Medicines ineffective. Suddenly craving rice wine, drank several cups hot, felt abdominal comfort. Drank to drunkenness, then cured. This was Cold-Dampness, unrecognized by doctors.
Yao Yingfeng saw a man, around sixty, whole body small boils like millet. Yingfeng said: "Named Jing Hai Chuang (Pure Sea Sore). In the Jiazi year nearing Zhouhai, immortal assessment true. Born with this sore: untreated lives, treated dies." Man did not believe. Treated sore, sore healed, man died.
One person had strange carbuncle, like abscess but small, strange pus like mung bean soup, gone today, full tomorrow. Use Yun Tai Cai (Rapeseed Vegetable), disperse blood and detoxify. Must mash cooked, put in cloth bag, bake in hot ash, alternate compressing. Two or three times cured.
Wu Huangchi treated a woman, over thirty, postpartum deficiency syndrome, aversion to cold, unable to speak, unable to move hands/feet, good appetite, normal excretion, profuse sweating. Treatment: Ren Shen (Ginseng Radix) large dose, add Gui Zhi (Cinnamomi Ramulus), one to three qian per dose depending on severity. Taken for half a year. In summer, aversion to wind/cold worsened. Mouth could speak, hands/feet could move, but cold aversion remained. Forced to sit in cool breeze, immersed hands/feet in cold water, held cold water in mouth. Initially very uncomfortable, later endured, felt safe. Gradually warm to hot, increasingly hot, desired frozen drinks. Drank cold water suddenly, removed clothes suddenly, returned to normal and healed.
Qin Jianquan, Zhuangyuan in Ren Shen year, nocturnal sweating for three years, wetting clothes/quilt every waking. Diet/lifestyle normal. Dozens of doctors ineffective. I was in capital, came to consult. Palpated: six pulses like silk, yet leisurely without illness, only Liver pulse medium-pressure wiry and solid. Said: "Your pulse is Six Yin pulses. No other abnormality, only left Guan wiry. Liver/Gallbladder has Fire." Zhongjing states: "Nocturnal sweating is half-exterior half-interior, Gallbladder has Heat." Used Xiao Chai Hu Tang (Minor Bupleurum Decoction), adding Dang Gui (Angelicae Sinensis Radix), Sheng Di Huang (Rehmanniae Radix), Dan Pi (Moutan Cortex), Frost-fallen Sang Ye (Mori Folium). Few doses cured.
Zhou Nanxi from Wulin, over thirty, strong constitution, feared heat. Avoiding heat in West Lake, sat in damp shade daily, ate fruits/melons. Mid-Autumn suddenly legs tendons pulled painfully, days later arms. Days later, finger movement caused whole-body tendon pulling pain and cessation. Treated with wind-dispersing/phlegm-draining agents, ineffective, critical. Pulses all wiry and urgent. Wiry indicates Fluid retention, urgent indicates Cold. Phlegm entering bones/tendons causes pulling pain. Disease due to Cold-Damp generating Phlegm flowing into tendons. Ban Xia (Pinelliae Rhizoma), Fu Ling (Poria) three qian each, Bai Jie Zi (Sinapis Semen) two qian, Chen Pi (Citri Reticulatae Pericarpium), Mu Gua (Chaenomelis Fructus) one qian half each, Gan Jiang (Zingiberis Rhizoma), Gan Cao (Licorice Radix) one qian each, Sheng Jiang (Zingiberis Rhizoma Recens) three slices. Delivered Qi Ying Wan (Miraculous Response Pill) seven Sophora-seed-sized pills. Half day later, fingers moved. Further doses stopped pulling. Changed to Liu Jun Zi Tang (Six Gentlemen Decoction), regulated half month, spirit recovered.
Official surnamed Wang, mid-Spring body very hot, sensation of object moving on body, dry mouth/lips, yellow-red urine. Said: "This is Re Bi (Heat Bi)." *Tong Zhi* states: "Heat Bi transfers heat from Zang-Fu, meets external pathogen, lodges in meridians, stays not moving. Yang encounters Yin, thus Bi obstructed and stifled, muscles extremely hot, body like mouse running, lips/mouth cracked, skin color changed. Treat with Sheng Ma Tang (Cimicifuga Decoction)." Gave this decoction, cured.
