The diseases of Ben (sudden heavy uterine bleeding) and Lin (dysuria with dribbling) are similar in appearance but different in essence. Ben is like a landslide, with massive bleeding that cannot be controlled; this is due to blood heat combined with Qi deficiency, resulting in the failure of Qi and Blood to consolidate and retain. Lin resembles dripping water, difficult and unsmooth, caused by internal stagnation of heat and stagnant Qi. However, Ben involves pure blood, whereas Lin presents differences in red, white, sand, and stone characteristics. Red indicates involvement of Blood, white indicates involvement of Qi, and sand/stone represents the most turbid aspects of Qi and Blood. To treat these conditions, one only needs to regulate Qi and Blood and clear internal heat.
Zheng Wenkang stated: Sudden heavy bleeding in women is caused by Kidney Water Yin deficiency, which fails to control the Minister Fire in the Bao Liao (uterine collaterals), causing blood to rush and bleed heavily. Liangxue Dihuang Tang (Cooling Blood Rehmannia Decoction) should be used as the primary treatment. However, this condition often arises from internal injury. If there is no lower abdominal pain, this formula alone is suitable, or Bawu Tang (Eight Substance Decoction) can be modified with Huangqin (Scutellariae Radix) and Huanglian (Coptidis Rhizoma). If there is pain, a large dose of Siwu Tang (Four Substance Decoction) should be used first, doubling the dosage of Danggui Shen (Angelicae Sinensis Radix), Baishao (Paeoniae Radix Alba), and Chuanxiong (Ligustici Chuanxiong Rhizoma), and adding Xiangfu (Cyperi Rhizoma) processed with vinegar. If using tonifying herbs, Bugong Tang (Uterus-Tonifying Decoction) with Huangqin and Huanglian is appropriate. Furthermore, there are two theories regarding Ben: one is stasis of Blood, the other is "empty pain." Those with stasis of Blood must experience alternating chills and fever; those with empty pain do not. Stasis of Blood requires removal, while empty pain requires tonification. There are also cases where the Sea of Blood is deficient and cold, external wind-cold invades and binds without dispersing, causing Qi and Blood to coagulate into masses; Shenshen Jubao Dan (Divine Immortal Treasure-Gathering Pill) is indicated. In cases caused by damage to Qi and Blood, coagulation forms masses known as Seven Zha and Eight Jia (abdominal masses); upward, it causes rebellious Qi and vomiting; downward, it causes dysentery with five types of bloody stools; Neizhi San (Internal Moxibustion Powder) is indicated. For patients with Ben who do not respond to decoctions, the condition is attributed to Fire; Sanhuang Tang (Three Yellows Decoction) is used, removing Dahuang (Rhei Radix et Rhizoma) and adding Huangbai (Phellodendri Cortex). If a woman's bleeding does not stop, it is due to weakness of the Chong and Ren vessels and cold deficiency of the Zang-Fu organs. This may also present with urgent lower abdominal pain and discharge of red and white leukorrhea; Jiajian Wuzhu Yu Tang (Modified Evodia Decoction) or Aijian Wan (Mugwort Decoction Pill) is indicated. If excessive blood loss leads to Qi and Blood deficiency, manifesting as limb fatigue, Zengsun Siwu Tang (Modified Four Substance Decoction) is suitable. Generally, for women suffering from blood leakage and depletion, consolidating Qi is essential; this is the core method of ancient sages. First, tonify Stomach Qi to aid the source of generation; sweet-flavored herbs are prioritized, such as in Yiwei Shengyang Tang (Stomach-Tonifying and Yang-Raising Decoction). People often mistake this for merely tonifying Qi, not realizing that sweet flavors generate Blood, following the principle of Yang generating Yin. Moreover, the human body treasures grain Qi; herbal dosages must be adjusted according to food intake, ensuring the medicinal Qi does not overpower the grain Qi. If abdominal pain occurs, add Wuyao (Linderae Radix) 3 fen and Guangui (Cinnamomi Ramulus) a little. If thirsty, add Gegen (Puerariae Lobatae Radix) 3 fen. For elderly women with Ben, Bawu Tang with Huangqin and Huanglian is used. This is an emergency rescue formula; protecting Stomach Qi is paramount. If decoctions fail to stop Ben, powders can be used, such as Zonghui (Palmae Fructus Cinchona Ash), Goudimo (Pot Bottom Ink), Chao Hei Shanzhi (Fried Gardeniae Fructus), Huaihua (Sophorae Flos), Cebaiye (Platycladi Cacumen), Renshen (Ginseng Radix et Rhizoma), Huangqi (Astragali Radix), and Gancao (Glycyrrhizae Radix et Rhizoma), ground into powder and taken with Tongbian (Child's Urine). Pills are even better, or they can be taken with Xiaojizhi (Leonuri Herba juice) or Ouji (Nelumbinis Rhizoma juice). The Classic states: For leukorrhea and Ben, floating, deficient, and slow pulses indicate survival, while excess and rapid pulses indicate severe disease. The Aphorism states: Shengdi (Rehmanniae Radix) combined with Puhuang (Typhae Pollen), Huangqin and Huangbai cool, Renshen and Wuweizi (Schisandrae Fructus) detoxify; finely decocted and taken on an empty stomach, it treats Ben and leukorrhea well. Acute cases belong to blood heat; nourish Blood and clear fire; use Wenqing San (Warming and Cooling Powder). Chronic Ben belongs to deficiency-cold; use warming and tonification; use Yimu Tang (Motherwort Decoction). If the Cun Kou pulse is wiry and large, wiry indicates reduction, large indicates Kuo (hollow); reduction implies cold, hollow implies deficiency; cold and deficiency interacting is named Ge (leather-like) pulse.
