(Also treats chronic spleen diarrhea that persists for a long time.)
Renshen (Ginseng Radix) [2 qian], Baishao (White Peony Root, stir-fried with wine) [8 fen], Baizhu (Atractylodis Macrocephalae Rhizoma) [2 qian], Danggui (Angelicae Sinensis Radix) [1.5 qian], Lianzi (Nelumbinis Semen, heart removed) [12 grains], Shengjiang (Fresh Ginger, 2 slices). Decoct and take. Strictly forbid sexual exertion and internal fire agitation. For chronic spleen diarrhea lasting many years, more than one hundred doses are required. If the condition is severe with abdominal pain, add Jiangtan (Charred Ginger) [5 fen]; if deficiency is extreme, add Renshen (Ginseng Radix) [2-3 qian]. For symptoms of uterine bleeding accompanied by spleen diarrhea, taking this formula has saved many lives. However, even if there is heat, it is contraindicated to use Zhizi (Gardeniae Fructus), Baipi (Phellodendri Cortex), Huangqin (Scutellariae Radix), and Lianlian (Coptidis Rhizoma).
Postpartum spleen and stomach deficiency, with diarrhea starting immediately after delivery, requires first taking one dose of Shenghua Tang (Decoction for Generating New Blood). In the second dose, add Fuling (Poria) [1 qian], Rouguo (Myristicae Semen, wrapped in flour and baked until ripe, flour removed) [1 fruit], Hezipi (Pericarpium Arecae Seedae, oil removed) [1 qian], Lianzi (Nelumbinis Semen) [10 grains], Shengjiang (Fresh Ginger, 1 slice). Decoct and take two doses; if diarrhea does not stop, add Renshen (Ginseng Radix) [1-2 qian]. If urinary retention occurs due to fluid loss from diarrhea, absolutely do not use diuretic herbs.
For chronic diarrhea before pregnancy that continues postpartum, leading to maternal collapse, use the following formula on an emergency basis to support deficiency. If mass-related pain persists, wait until the collapse subsides slightly, then remove Renshen (Ginseng Radix) and Rouguo (Myristicae Semen) to eliminate the mass pain.
Chuanxiong (Ligustici Chuanxiong Rhizoma) [1 qian], Danggui (Angelicae Sinensis Radix) [2 qian], Ganjiang (Zingiberis Rhizoma), Gancao (Licorice Root) [each 5 fen], Fuling (Poria) [1.5 qian], Shanyao (Dioscoreae Rhizoma) [1 qian], Rouguo (Myristicae Semen, baked in flour until ripe) [1 fruit], Hezipi (Pericarpium Arecae Seedae, oil removed) [1 qian], Lianzi (Nelumbinis Semen) [7 grains], Renshen (Ginseng Radix) [2 qian], Nuomi (Glutinous Rice) [a large pinch]. If deficiency is extreme, increase the amount of Renshen (Ginseng Radix). After seven days postpartum, if blood masses still cause pain, also take this formula. If blood masses no longer cause pain, add Baizhu (Atractylodis Macrocephalae Rhizoma) [2 qian] and Chenpi (Citri Reticulatae Pericarpium) [3 fen]. If diarrhea is accompanied by heat, do not use Zhizi (Gardeniae Fructus), Baipi (Phellodendri Cortex), Huangqin (Scutellariae Radix), or Lianlian (Coptidis Rhizoma). If there is phlegm, do not use Banxia (Pinelliae Rhizoma) or Gualou (Trichosanthis Fructus). If diarrhea is accompanied by thirst, use Shengmai San (Generating Pulse Powder) to restore fluids.
Within seven days postpartum, suffering from dysentery with red and white discharge, frequent tenesmus is the most difficult to treat. One wishes to regulate qi and move blood to push out the dysentery pathogen, yet fears the weakness of postpartury yuanqi (vital energy); one wishes to nourish ying (nutritive qi) and tonify qi to greatly supplement postpartum deficiency, yet fears aiding the pathogen and exacerbating the initial surge of dysentery. Only a method that neither damages yuanqi nor aids the pathogen is suitable. This aligns with modifying Shenghua Tang (Decoction for Generating New Blood) by removing Paoyang (Roasted Ginger) and adding Muxiang (Aucklandiae Radix) to smooth qi, thereby treating both without contradiction. Subsequently, take Jiawei Xianglian Wan (Modified Almond and Coptis Pill) and observe for one or two days, adjusting according to the disease trend, to ensure safety. If brown dysentery with frequent tenesmus occurs, there are relevant discussions in Danxi Zuanyao (Essentials of Zhu Danxi). If the mother was originally robust and it has been a month postpartum, purgative formulas and cold-natured herbs like Huangqin (Scutellariae Radix) and Lianlian (Coptidis Rhizoma) can be used. If the mother was originally weak, even if more than a month postpartum, harsh purgatives should not be used to move stagnation. Regarding anorexic dysentery (Jinkou Li), there are specific treatments in Zhai Kan Qi Fang (Selected Miraculous Formulas). Be careful not to use Houpo (Magnoliae Officinalis Cortex) and Zhike (Aurantii Fructus Immaturus) based on ancient formulas to treat postpartum dysentery; instead, substitute with Xianglian Wan (Almond and Coptis Pill). (The formula for Xianglian Wan is in the continuation volume.)
