有积,经行为血虚有热也。大抵多内因忧思忿怒,外因饮冷形寒。盖人之气血周流,忽因忧思忿怒所触,则郁结不行;
妇人以血为主,经行与产后一般最宜谨慎。其实若有瘀血一点未净,或被内寒湿热暑邪,或内伤生冷,或浣濯入冷;

Generally, women with strong congenital endowment (Bingfu) menstruate as early as age thirteen; those with weak endowment may not menstruate until after the age of fourteen (two sevens). If a woman is constitutionally frail and often suffers from yin deficiency with night fever, and has not yet menstruated by eighteen or nineteen, she must be nourished according to the season and marriage should be postponed. If intercourse occurs before the yin qi is fully matured, it often leads to consumptive weakness (Laoqie).
If a woman's Tian Gui (the substance promoting reproductive maturity) has arrived, but she has had no intercourse for over ten years, her menses will become irregular. If less than ten years have passed but she desires intercourse, her menses will also be irregular. Forced intercourse before the appropriate age also causes irregularity. Irregular menses result in old blood failing to discharge and new blood flowing recklessly, potentially infiltrating bones or causing swelling, or leading to infertility even if conception occurs. Excessive intercourse depletes essence, damaging the body; frequent childbirth and lactation exhaust blood, endangering life. "Jing" (Menses) refers to the regular physiological cycle. Excess yang causes early menstruation; insufficient yin causes delayed menstruation. Fluctuating amounts, sudden cessation, metrorrhagia, and leukorrhea are all caused by the excess or deficiency of yin and yang. When yin attacks yang, the uterus becomes cold, and blood circulation is impaired. As stated in the *Classic*, "When heaven is cold and earth frozen, water condenses into ice," leading to scanty menses that arrive late. When yang attacks yin, menstrual blood boils, leading to heavy menses arriving early. One must distinguish yin and yang, harmonize qi and blood, prevent mutual attack, and aim for balance.
Women's irregular menses may be caused by wind-cold invading the uterus due to deficiency, damaging the Chong and Ren vessels. These vessels originate in the uterus; if maintained properly, qi and blood harmonize, protecting against the Six Evils (Wind, Cold, Summer-heat, Dampness, Dryness, Fire). However, labor injury to qi and blood allows wind-cold to invade, damaging the Spleen and Stomach, reducing appetite, weakening Ying and Wei qi, causing sallow emaciation—all due to Chong and Ren damage. Thus, caution is essential during menstruation, lest complications resemble postpartum disorders.
Women rely on blood as their foundation. Worry, thought, anger, and stagnation affect blood: when qi moves, blood moves; when qi stops, blood stops. Excessive worry causes qi stagnation and blood stasis; excessive anger causes qi rebellion and blood rebellion. Early menstruation indicates fire; treat with Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) to calm fire. Early and heavy menstruation requires adding Hai Piao Xiao (Cuttlefish Bone), Chai Hu (Bupleurum Root), Bai Shao (White Peony Root), and Wu Wei Zi (Schisandra Berry) to the base formula. Menstruation occurring every half month or ten days with prolonged bleeding indicates qi deficiency; use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). Delayed menstruation indicates deficient fire, manifesting as cold, deficiency, stagnation, or phlegm; add Ai Ye (Mugwort Leaf), Xiang Fu (Cyperus Rhizome), and Ban Xia (Pinellia Tuber). Late menstruation with pale color requires adding Gui Zhi (Cinnamon Twig). This is a general overview. Some cases show early onset without fire, or late onset with fire. Purple-black blood usually indicates extreme fire heat, though some cold-deficiency cases also present this way. Pale white blood clearly indicates absence of fire. However, damp-phlegm can cause pale white blood, and damp-heat phlegm can cause yellow turbid blood. Therefore, pulse speed (Chi/Shuo) and constitutional strength must be considered.
Menstrual blood is yin blood, which follows yang, hence its red color. It corresponds to the moon; full moon wanes, waning moon waxes. Its movement is regular, hence "Menstruation." It accompanies qi: qi heat makes blood hot, qi cold makes blood cold, qi stagnation makes blood stagnant. Clots indicate qi凝 (stagnation); pain before menstruation indicates qi stagnation; pain after menstruation indicates qi and blood deficiency; erratic flow indicates qi disorder; pale color indicates deficiency mixed with water-dampness; purple indicates qi heat; black indicates extreme heat. Modern practitioners often mistake purple-black clots with pain for wind-cold, using warm formulas, which brings disaster. The *Classic* states: "Excess causes harm, restraint brings order." Extreme heat transforms into water-like characteristics, hence purple (heat) and black (extreme heat). The *Yu Ji* says: "Cold causes stagnation and non-movement." Since blood flows and is purple-black, it is not cold. Women are prone to anger and depression, and visceral Jue-yang fire is always present; thus, it is rarely not heat. Pulse diagnosis clarifies this.
Initial external cold pathogens entering meridians cause pain; long-term stagnation transforms into heat. Blood cold causes coagulation; although purple-black blood flows, it is not cold, similar to Shanghan transforming into heat disease.
Some women experience severe colic-like abdominal pain before menstruation, alternating chills and fever, and discharge resembling black bean juice. The Chi pulses are deep and choppy, while others are wiry and urgent. This is due to lower jiao cold-damp pathogenic factors binding with the Chong and Ren vessels. Extreme pain generates heat, allowing flow, but cold-damp creates turbidity, resulting in bean-juice-like discharge. Treat the lower jiao with acrid-dispersing bitter-cold and blood-invigorating herbs. For blood deficiency and涩 (roughness), use blood-nourishing herbs with qi-regulating agents.
Body pain during menstruation occurs when wei qi nourishment is externally lacking and ying blood irrigation is internally deficient, despite abundant qi and blood. Some say: if the Sea of Blood is sufficient, it overflows at the proper time; if insufficient, systemic blood is damaged when the time comes, causing body pain before flow. Post-menstrual abdominal pain indicates severe qi and blood deficiency; use Bazhen Tang (Eight-Treasure Decoction). For deficiency with heat, use Xiaoyao San (Free and Easy Wanderer Powder). For qi stagnation with incomplete flow, use Siwu Tang (Four-Substance Decoction) plus Mu Xiang (Aucklandia Root).
Post-menstrual fever, fatigue, and blurred vision indicate Spleen yin deficiency. The Spleen is the head of yin organs; eyes are the gathering of blood vessels. Spleen yin loss affects all five zang organs, preventing clear vision. Use Buzhong Yiqi Tang and Guipi Tang to regulate Spleen and Stomach, tonify qi and blood. Eyes see when nourished by blood. Mistakenly using cooling eye remedies causes serious illness.
Premenstrual diarrhea for two to three days followed by menstruation, with soft and weak pulses, indicates Spleen and Kidney deficiency. The Spleen controls blood; during menstruation, Spleen qi transports, but the Sea of Blood fails to control dampness. The Kidney controls retention; menstrual flow should be timely, but Gui Yuan (Kidney Essence) is depleted, losing retention power. Use modified Guipi Tang to warm and tonify Spleen and Kidney. Excessive blood loss with leukorrhea, worse at night, and constant diarrhea indicates Yang deficiency sinking, called "Tuoyang" (Loss of Yang). Use Shiquan Dabu Tang or Buzhong Yiqi Tang.
Amenorrhea may result from chronic Spleen-Stomach deficiency, emaciation, and declining qi/blood; excessive mental labor causing Heart fire to rise without descending to fu organs; middle-xiao (consumption) stomach heat causing thirst, hunger, emaciation, fluid depletion, and empty Sea of Blood (Xueku Jingjue); cold pathogens in the uterus causing blood cold and stagnation; obesity with phlegm obstructing the Sea of Blood; cold or heat mixing with dirty blood causing stagnation; or food/damp-phlegm blocking the Taiyin meridian causing amenorrhea with pain. Differentiating cold/heat/excess/deficiency relies on pulse speed and strength.
Menstrual disorders include irregularity and obstruction. Irregularity includes early (heat) and late (deficiency). Obstruction includes blood stasis (needs moving) and blood exhaustion (needs tonifying). Pain includes constant/pre-menstrual (blood accumulation) and post-menstrual (blood deficiency). Fever includes constant (deficiency with accumulation) and pre-menstrual (deficiency with heat). Causes are internal emotional stress (worry/anger) and external cold/damp. Emotional stress causes stagnation; cold/damp prevents lochia discharge. Treatment regulates qi, moves blood, opens stagnation, tonifies deficiency, cools blood, clears heat. Warm herbs like Rou Gui (Cinnamon Bark) assist blood movement. Post-illness amenorrhea due to qi/blood deficiency requires Spleen tonification and blood nourishment; natural recovery follows.
Menstruation and postpartum periods require utmost caution. Residual blood stasis, internal cold/damp/heat/summer-heat, raw/cold food injury, cold water exposure, or sour/salty foods can cause Qi Qing (Seven Emotions) stagnation, named "Blood Stasis." Overexertion, excessive sex, or hot foods after menstruation cause fire movement, pathogen excess, and fluid depletion, named "Blood Exhaustion." Shock during menstruation causes chaotic blood flow: upward causes nose/mouth bleeding; inward causes edema. Anger causes qi/blood rebellion in waist/legs/abdomen/back/limbs, worsening during menstruation. Extreme anger damages Liver, causing dizziness, hematemesis, scrofula, or skin itching. Leakage causes sores and continuous dripping; damp-heat causes metrorrhagia and leukorrhea. Internal blood knots form masses (Jia). All variations stem from Blood Stasis or Blood Exhaustion. Severe cases lead to amenorrhea and consumption. Thus, treating women requires asking about menstruation first.
Blood Exhaustion (Ku) and Blood Blockage (Ge) both cause amenorrhea. Ku means depletion/extreme deficiency; Ge means obstruction with normal blood volume but blocked by qi, cold, or accumulation. Ge is acute, painful/full, cured by unblocking. Ku is gradual, empty, requiring yin tonification. Using harsh purgatives on Ku worsens it and risks death.
Blood Stasis amenorrhea requiring breaking stasis is due to toxic heat, rage-induced stasis/phlegm. Use Da Huang (Rhubarb) and Gan Qi (Drylac) to eliminate old and generate new. Qi-excess/Blood-exhaustion from labor/worry needs gentle tonification. With phlegm-fire/damp-heat, cool and clear, aided by Rou Gui to promote flow. Do not use harsh drugs to damage yin; nourish Ying/Wei and harmonize diet. Seeking quick fixes with harsh drugs is futile.
A fifteen-year-old girl presented with absent menses since age fourteen. This is "Bi Nian" (Avoidance Year), where menses stop temporarily and resume later. It stems from Tian Yi (Source of One). Accumulation fills monthly and overflows. Chong/Ren originate in the uterus; Kidney Fire governs color: strong=red, weak=pale, very strong=purple, very weak=white. Nourishing water requires nurturing fire. Even dryness/blockage from extreme fire is rooted in water deficiency. Do not use bitter-cold to suppress fire; greatly tonify water from the source. Fullness leads to natural flow. Toxic purgatives are contraindicated.
Always ask about pregnancy before treating women. Sick pulses mimic pregnancy pulses. Avoid harsh qi-breaking/blood-moving drugs if pregnancy is suspected.
Women's diseases are ten times harder to treat than men's. After fourteen, floating yin, excessive thoughts, and internal organ damage disrupt menses, causing stasis and blockage. Desire exceeds men's; emotions like jealousy deepen pathology. Nuns and widows, lacking yang, suffer heart/Spleen damage from depression, hard to cure with herbs alone.
Disease preceding irregular menses requires treating the disease first. Irregular menses preceding disease requires regulating menses first.
*Mai Jing* states: Slippery Chi pulse indicates abundant blood/qi and difficult menses. Interrupted Chi pulse indicates difficult menses. Normal Cun/Guan but absent Chi pulse indicates difficult menses and lower abdominal pain. Wiry Liver pulse indicates difficult menses and waist/abdominal pain.
