Xue Lizhai stated: According to *Yixue Jiyao* (Essentials of Medical Practice), wind is fundamentally heat; when heat prevails, wind moves. One should use stillness (nourishing blood) to overcome restlessness; this is the function of nourishing blood. The treatment method should involve mild diaphoresis (inducing sweating) and mild purgation (draining downward). Excessive sweating will weaken Wei Qi (Defensive Qi), while excessive purgation will damage Ying Blood (Nutritive Blood). Although there are prohibitions against diaphoresis and purgation, there is a distinction between involvement of the Fu organs and the Zang organs. Involvement of the Fu organs often manifests in the four limbs, presenting with floating pulse, aversion to cold, stiffness, and numbness. Involvement of the Zang organs often affects the nine orifices, presenting with flaccid lips and loss of speech, deafness, nasal congestion, blurred vision, and constipation. Cases involving the Fu organs are suitable for diaphoresis, while those involving the Zang organs are suitable for purgation. Once the exterior and interior are harmonized, treatment should focus on the meridians, using major tonifying herbs to nourish the body. If pregnant women suffer from this condition, treatment should follow these principles, prioritizing herbs that stabilize the fetus, and avoiding excessive use of herbs specifically for treating stroke.
Chen Liangfu stated: During pregnancy, if the constitution is deficient and susceptible to wind pathogenic factors, damaging the Foot-Taiyang meridian, and further encountering a struggle between wind and cold pathogens, symptoms such as locked jaw and back rigidity may occur. In severe cases, the waist may arch backward, a condition named Jing disease (Spasm). The presentation involves dizziness, confusion, and loss of consciousness, followed by spontaneous recovery after a moment, only to recur after a long interval. This is called Wind-Jing, also known as Zi Xian (Eclampsia).
Xue Lizhai stated: For the aforementioned conditions, if caused by Heart-Liver Wind-Heat, use Gouteng Tang (Uncaria Decoction). If due to Liver-Spleen Blood Deficiency, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder). If due to Liver-Spleen Stagnation and Anger, use Jiawei Guipi Tang (Modified Restore the Spleen Decoction). If due to Qi Reversal and Phlegm Stagnation, use Zisu Yin (Perilla Infusion). If due to Liver Fire and Wind-Heat, use Gouteng Tang. If due to Spleen Stagnation and Phlegm Stagnation, use Erchen Tang (Two-Aged Phlegm Decoction) added with Zhuli (Bamboo Sap) and Jiangzhi (Ginger Juice).
Xue Lizhai stated: Chi (muscle contraction) refers to tendons and vessels becoming tight and shortening. Zhang (muscle relaxation) refers to tendons and vessels becoming loose and extending. Alternating extension and contraction, causing the hands and feet to pull and twitch, this syndrome mostly belongs to Wind, as Wind governs tremors. Luo Longji said: The Heart governs the vessels, and the Liver governs the tendons. The Heart belongs to Fire, and the Liver belongs to Wood. Fire governs Heat, and Wood governs Wind. When Wind and Fire blaze together, Chi-Zhang (twitching and spasms) occurs. Treatment methods: If due to Wind-Heat, add Gardenia, Bupleurum, Scutellaria, and Atractylodes to Gouteng Tang to calm Liver Wood, descend Heart Fire, and nourish Qi and Blood; if Wind-Phlegm surges upward, add Pinellia and Arisaema with Bamboo Sap; if Wind Pathogen causes acute twitching, add Scorpio and Bombyx Batryticatus; if Qi and Blood are deficient, use Bazhen Tang (Eight Treasures Decoction) added with Uncaria and Gardenia; if there is weakness with twitching, eyes rolling up, opisthotonos, and sweat like oil, this indicates Liver Qi exhaustion and is incurable.
Shen Zhai commented: The above four sections describe Stroke syndromes before pregnancy, including Wind-Jing, Zi Xian, and Chi-Zhang, which fall into this category. Stroke syndromes are mostly caused by internal Wind, and treatment methods vary. Lizhai, based on Jiegu's theory, argues that the treatment for pre-pregnancy stroke should prioritize stillness overcoming restlessness and nourishing blood; this is the key to treating pre-pregnancy stroke. The fetus relies on Blood for nourishment; when Blood is deficient, it is prone to pathogenic invasion; when Blood flows, Wind extinguishes itself. Danxi focused on clearing Heat and nourishing Blood for pre-pregnancy conditions. Lizhai inherited this view, citing a passage from *Yixue Jiyao* as evidence, which indeed has a solid theoretical foundation.
Shen Zhai further commented: Pre-pregnancy stroke is precisely what Hejian described as improper lifestyle management. Kidney Water declines while Heart Fire flares, leaving the Liver without nourishment. This is not an external Wind Pathogen attack; rather, one must urgently nourish the source of transformation, drain the South (Heart/Fire) and tonify the North (Kidney/Water), strengthen Water to control Fire, so that Liver Wood naturally calms and fetal Qi stabilizes. Merely nourishing Blood might be considered secondary.
