(Applied to Jue-Ni / Cold Extremities)
[From *Jin Gui Yao Lue*] Question: There are three diseases in women shortly after childbirth: one is Jing disease (spasm), the second is Yu Mao (depression and dizziness), and the third is difficult defecation. What does this mean? The Master said: After childbirth, blood deficiency occurs, with profuse sweating and a tendency to contract wind pathogens, thus causing Jing disease (spasm). Loss of blood followed by sweating leads to excessive cold, thus causing Yu Mao (depression and dizziness). Depletion of body fluids results in dryness of the Stomach, thus causing difficult defecation. Note: Yu Mao in postpartum women is what is currently referred to as Xue Yun (blood syncope).
[From *Da Quan Liang Fang*] Postpartum Xue Yun (blood syncope) is caused by敗血 (stagnant blood) flowing into the Liver Meridian, presenting with blacking out of vision, visual floaters, vertigo, inability to sit up, and even coma with loss of consciousness, termed Xue Yun (blood syncope). It is best treated with Hei Shen San (Black Divine Powder) mixed with fine wine. Quack doctors sometimes treat it as An Feng (dark wind) or Zhong Feng (stroke). In all cases of fainting, heat of blood rises counterflow due to deficiency, attacking the Heart, causing coma and loss of consciousness, with Qi blockage nearing extinction. However, there are three causes: fainting due to excessive mental or physical exertion; fainting due to excessive bleeding; and fainting due to insufficient bleeding. Although the symptom of fainting is the same, the treatment differs, so careful differentiation is required. For fainting due to excessive bleeding, characterized only by coma, oppression, and irritability, use herbs that tonify blood and clear the Heart. For fainting due to insufficient bleeding, where lochia fails to discharge and rushes upward to attack the Heart, causing epigastric fullness and urgency, coma, clenched jaw, and complete loss of consciousness, use herbs that break stasis and promote blood circulation. Ancient methods state: Immediately after delivery, pre-heat a weight stone, yellow stone, or hard charcoal until red-hot, place it in a vessel, and urgently pour vinegar over it near the bed; the vinegar fumes can eliminate Xue Yun (blood syncope). Doing this occasionally within one month postpartum is excellent. Cui Shi said: All fainting is due to deficient heat, where Qi and Blood rush to the emptiness in the abdomen. For those about to deliver, first apply strong vinegar to the mouth and nose, keep vinegar nearby for inhalation, and drink it slowly; this is the superior method. If feeling faint, apply vinegar to the face; once recovered, drink vinegar diluted slightly. (Another version says dilute with water.) Another method is to burn dried lacquer to produce thick smoke and fumigate the mother's face to awaken her. (If dried lacquer is unavailable, burning an old lacquerware vessel to produce smoke is also effective.)
Guo Jizhong discussed: What is postpartum Xue Yun (blood syncope)? Answer: After childbirth, Qi and Blood suddenly become deficient. Without rest, Blood follows Qi rising counterflow, confusing the Spirit of the Heart, causing visual floaters. In severe cases, it causes suffocation, loss of consciousness, clenched jaw, coma, and cold breath. If physicians do not recognize this and call it An Feng (dark wind), treating it as such will make recovery difficult. Simply taking Qing Hun San (Clear Soul Powder) will restore consciousness.
[Xue Ji] When Yuan Qi (Original Qi) is depleted postpartum, lochia attacks upward due to deficiency, causing blurred vision, dizziness, epigastric fullness, coma, clenched jaw, or phlegm obstruction. Urgently use Shi Xiao San (Pain Relieving Powder). If fainting occurs due to excessive bleeding, or if there is coma with irritability, use a large dose of Xiong Gui Tang (Ligusticum and Angelica Decoction) to tonify, or Yun Tai Zi San (Rapeseed Seed Powder), or Tong Zi Xiao Bian (Child's Urine). Add Er Chen Tang (Two-Cured Decoction) if there is phlegm. If caused by labor and mental exertion, use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) with Xiang Fu (Cyperus Rhizome). If unconsciousness is due to extreme deficiency of Qi and Blood, use Qing Hun San (Clear Soul Powder), followed by Xiong Gui Tang (Ligusticum and Angelica Decoction) and other formulas to strongly tonify Qi and Blood. Vinegar or lacquerware fumigation can be used postpartum, or Ban Xia (Pinellia Rhizome) powder mixed with cold water into pills inserted into the nostrils, preventing these ailments. Dan Xi stated: Xue Yun (blood syncope) is due to joint deficiency of Qi and Blood, with Phlegm-Fire floating upward. Use Er Chen Tang (Two-Cured Decoction) to guide out phlegm, or Jia Jian Zhu Sha An Shen Wan (Modified Cinnabar Calming the Spirit Pill), taken with Mai Men Dong Tang (Ophiopogon Decoction). Generally, postpartum conditions like facial paralysis should primarily be treated by strongly tonifying Qi and Blood, while addressing phlegm. If Spleen and Stomach deficiency fails to secure, use Liu Jun Zi Tang (Six-Gentlemen Decoction). At five to seven months, take An Tai Yin (Fetus-Calming Infusion). At eight to nine months, add Da Fu Pi (Areca Peel) and Huang Yang Nao (Buxus Brain). Near delivery, take Bao Sheng Wu You San (Protect Life Without Worry Powder) to prevent these issues. A family member had postpartum lower abdominal pain and sudden clenched jaw; administered Shi Xiao San (Pain Relieving Powder), revived after a long time, then cured with Si Wu Tang (Four Substances Decoction) plus Pao Jiang (Roasted Ginger), Bai Zhu (Atractylodes Rhizome), and Chen Pi (Tangerine Peel). One patient had numbness in both hands; after taking Yu Feng Dan (Wind-Dispelling Pill) and Tian Ma Wan (Gastrodia Pill), whole-body numbness, mental fatigue, afternoon fever with thirst, spontaneous and night sweats occurred, indicating joint Qi and Blood deficiency. Used Shi Quan Da Bu Tang (Ten Complete Great Tonifying Decoction) plus Pao Jiang (Roasted Ginger) for several doses, symptoms subsided; removing Pao Jiang (Roasted Ginger) and continuing for several doses led to recovery. Only internal heat remained, indicating Blood deficiency; cured with Xiao Yao San (Free and Easy Wanderer Powder). A woman at eight months felt fetal descent or movement, with sallow complexion, fatigue, and poor appetite, indicating Spleen Qi deficiency. Used Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) with doubled Bai Zhu (Atractylodes Rhizome) plus Su Geng (Perilla Stem) for over thirty doses, resulting in stability. Postpartum vertigo, chest fullness, and cough were cured with Si Wu Tang (Four Substances Decoction) plus Fu Ling (Poria), Ban Xia (Pinellia Rhizome), and Jie Geng (Platycodon Root). A patient drank alcohol during labor, had excessive lochia, suffered from Xue Yun (blood syncope), and exhaled alcohol odor; this was Blood moving recklessly due to alcohol heat, causing faintness from deficiency. Used Fo Shou San (Hand Buddha Powder) plus Gan Ge (Kudzu Root) 2 qian, cured in one dose. Alcohol is fierce; it should not be consumed during pregnancy or postpartum to avoid such conditions. Crowded, noisy, and hot delivery rooms can also cause this condition.
Dan Xi treated a pale, tall woman who was emotionally depressed and gave birth at midnight, fainting at dawn. Urgently moxa Qihai (CV6) 15 cones to revive. Later, she took Ren Shen (Ginseng) and Bai Zhu (Atractylodes Rhizome) etc. for two months to stabilize. This was Yang deficiency.
Lu Yan from Fenghua treated a woman from Xinchang who died suddenly postpartum, with only slight warmth in the chest and diaphragm. Lu diagnosed: This is Blood stasis. Used dozens of jin of Hong Hua (Safflower), boiled in a large pot. When boiling, placed the patient on a wooden barrel filled with the decoction to fumigate, adding more as needed. Soon, the woman's fingers moved, and she revived after half a day. This aligns with Xu Yinzong's treatment of Empress Dowager Wang.

Zhong Jing said: Postpartum Yu Mao (depression and dizziness), pulse is weak and thin, unable to eat, stool is hard, but only head sweats. This is because Blood deficiency leads to Jue (cold extremities/reversal), and Jue leads to Mao (dizziness). For Mao to resolve, profuse sweating must occur. Due to Blood deficiency and lower reversal, solitary Yang emerges upward, hence head sweat. Therefore, postpartum women welcome sweating; loss of Yin Blood leads to excess Yang, so sweating occurs to restore Yin-Yang balance. For hard stool, vomiting, and inability to eat, Xiao Chai Hu Tang (Minor Bupleurum Decoction) governs. If illness resolves and appetite returns, but fever recurs after 7-8 days, this is Stomach excess, governed by Da Cheng Qi Tang (Major Order the Qi Decoction). Now note: Yu Mao (depression and dizziness) is synonymous with Yun (fainting/dizziness). Observing this, postpartum Xue Yun (blood syncope) has three methods: sweating, purging, and harmonizing. Differentiate exterior-interior and deficiency-excess precisely.
