Wind-phlegm in women arises from wind-cold affecting the zang-fu organs, causing fluid retention and accumulation in the chest and diaphragm. Everyone has phlegm; a small amount causes no hindrance, but an excessive amount becomes a pathology. When phlegm-fluid accumulates in the chest and diaphragm and permeates the five zang organs, it causes dizziness of the eyes, as well as vertigo and headache.
Commentary: The harm caused by phlegm often results from external contraction of the five pathogenic factors (the five factors are: cold, summerheat, dryness, dampness, wind) and internal injury from the seven emotions (the seven emotions are: joy, anger, worry, thought, fright, fear, resentment). Those acquired through the five pathogenic factors become wind-phlegm upon contracting wind, cold-phlegm upon contracting cold, cold-phlegm upon contracting chill, heat-phlegm upon contracting heat, and summerheat-phlegm upon contracting summerheat. Those injured by the seven emotions often arise because women have stubborn temperaments and lack tolerance, leading to this syndrome. Is it solely due to wind-cold? Thus, external contraction of the five pathogenic factors and internal injury from the seven emotions cause the body's fluids—blood, yin fluids, tears, sweat, nasal mucus, and saliva—to become turbid when dampness is prevalent, transforming into phlegm and taking advantage of opportunities to cause harm. The Classics state: "Clear substances transform into essence and nutrients; turbid substances coagulate into phlegm and fluid retention." This is what is meant. However, phlegm, salivary secretion, fluid retention, and foam are four similar conditions that require careful differentiation and treatment. For phlegm generated by wind, San Sheng Yin (Three-Life Decoction), Xing Xiang Yin (Aucklandia and Amber Decoction), Qingzhou Bai Wan Zi (Qingzhou White Pills), and Hua Tan Wan (Phlegm-Resolving Pill) [formula see *Tai Ping Hui Min He Ji Ju Fang*] are suitable. For those caused by cold, Jiang Qi Tang (Qi-Descending Decoction), Hei Xi Dan (Black Tin Pellet), and Yang Zheng Dan (Upright-Nourishing Pellet) [all see *Tai Ping Hui Min He Ji Ju Fang*] are suitable. For those caused by heat, Jin Fei Cao San (Boilweed Powder) and Chai Hu Ban Xia Tang (Bupleurum and Pinellia Decoction) are suitable. For those caused by summerheat, Xiao Shu Wan (Summerheat-Dispelling Pill) [see *Tai Ping Hui Min He Ji Ju Fang*] is suitable. For those caused by qi stagnation and constraint, Shen Su Yin (Ginseng and Perilla Decoction), Si Qi Tang (Four-Qi Decoction), and Er Chen Tang (Two-Aged Decoction) are suitable. If phlegm is located in the upper part, induce vomiting with Gua Di San (Melon Seed Powder); if in the lower part, purge with Kong Xian Dan (Controlling Fluid Retention Pellet). Although purging and inducing vomiting are indicated, one must observe the patient's physical strength and examine the pulse for deficiency or excess before administering medication. It is crucial to remember that violent purgation should be avoided; hence, Chu Yushi's warnings regarding Jin Hu (Golden Tiger) and Bi Xia (Jade Cloud) refer to this principle.
