Discussion: Fast-acting purgatives should not be used for constipation in the elderly, debilitated individuals, or those with wind disorders.
Chu Yushi stated: "I have observed that ancient practitioners generally used Qi-descending medicines for bowel unblocking. Since Lung Qi failing to descend prevents the Large Intestine from transporting waste, herbs such as Xing Ren (Apricot Seed), Zhi Ke (Aurantium Fruit), and He Zi (Chebula Fruit) are employed. Furthermore, the elderly, debilitated persons, and those with wind disorders lack body fluids, leading to constipation. The Classics state: 'For obstruction, use lubricating methods.' Therefore, herbs like Hu Ma (Sesame), Xing Ren (Apricot Seed), Ma Zi Ren (Hemp Seed), and E Jiao (Donkey-hide Gelatin) are used. Modern practitioners fail to follow ancient wisdom, making arbitrary adjustments. Whenever constipation occurs, they immediately employ fast-acting purgatives to scour the bowels. This depletes body fluids and Qi and Blood; although the bowels may clear temporarily, they subsequently become more obstructed, and other diseases arise. Previously, when I was in Lushan, there was a young man claiming to be an Imperial Physician. A resident named Wang Shi'an in Cao Town, originally from the capital, initially suffered from limb ailments caused by wind pathogenic factors. This physician repeatedly administered purgatives to him. Later, he developed Fei Wei (Lung Atrophy), expectorated pus and blood, and ultimately died due to inability to pass stool. Ancient practitioners were extremely cautious with medication, unlike modern people who disregard life. I cite this case to warn future generations."
The Classics state: "Wind injury in spring leads to undigested food diarrhea in summer." This is because Wood Qi overacts on Earth Qi; if Earth Qi fails to secrete properly, Wood Qi becomes excessive, causing diarrhea. Wind pathogen circulation dries up body fluids, causing constipation. Thus, one cannot solely attribute constipation and dryness to wind. My relative Zhang Gongdu's mother, aged seventy, suffered from dozens of diarrheal episodes daily. Despite numerous treatments, she remained ill and also suffered from stiffness and spasms in her waist and legs. Zhang Gongdu treated her spasms with Ji Li (Tribulus Fruit) and Suan Zao Ren (Zizyphus Seed), which cured both the spasms and the diarrhea. Huang Luzhi's mother, the Lady of Ankang Commandery, suffered severely from constipation. After administering Zhang Gongdu's prescription, her bowels moved freely. Thus, we know that both constipation and diarrhea stem from wind. Zhang Gongdu was from Luzhou, named [Name Missing in Original Text], from a family renowned for virtue. Inheriting his father's medical expertise, he upheld the family honor, practiced self-cultivation, and never took what was not his. He is truly a virtuous gentleman, as seen here.
Formula suitable for patients with wind disorders or Hong Kong Foot (Gouty Arthritis/Charcot Joint) experiencing either constipation or diarrhea, especially appropriate for debilitated individuals.
Zao Jia Zi (300 pods, broken into two halves, stir-fried slowly until very dry, then add Su You (Butter/Ghee) the size of a jujube pit, stir-fry until dry again, add more Su You, stir-fry until charred black, then grind into a fine powder)
Combine with refined honey to form pills the size of Sophora japonica seeds. Take 30 pills per dose, administered with a decoction of Ji Li (Tribulus Fruit) and Suan Zao Ren (Zizyphus Seed). Take on an empty stomach. If no bowel movement occurs after two hours (Shi Chen), administer another dose. The dosage can gradually increase to 100 pills without harm, stopping once bowel movement is achieved.
