When a rash appears during pregnancy, the prescription should be based on Siwu Tang (Four Substances Decoction) with modifications, adding Huangqin (Scutellariae Radix, specifically "Tiao Qin") and Aye (Mugwort Leaf), focusing primarily on stabilizing the fetus and clearing heat. In this way, the fetus remains secure, and the measles will naturally erupt. However, heat-toxin steaming the uterus often injures the fetus; although the fetus may be damaged, the mother usually remains unharmed. This is because measles differ from smallpox. Smallpox requires internal fullness because its pathogenic factors should resolve from the exterior; if the fetus is expelled, the toxin attacks inward through the deficiency, leading to the mother's death. Measles, conversely, benefit from relative internal emptiness because their pathogenic factors should resolve from within; if the fetus is expelled, the heat-toxin descends with the fetus, allowing the mother to survive. Nevertheless, rather than losing the fetus while saving the mother, it is far superior for both mother and child to survive. Moreover, ancients only knew to clear heat to stabilize the fetus, failing to consider that clearing heat before the measles have erupted causes the internal heat to deepen even after the rash appears. Thus, intending to protect the fetus inadvertently harms it. Therefore, one should employ light, ascending methods to vent and push out toxins; once the measles erupt, the heat clears naturally. Subsequently, using nourishing yin and clearing/resolving methods ensures neither the measles nor the fetus are hindered. The fetus stabilizes without explicit stabilization efforts. If the eruption is slow, combine Baihu Tang (White Tiger Decoction) with Shengma Gegen Tang (Cimicifugae and Puerariae Decoction), doubling the dosage of Xuanshen (Scrophulariae Radix) and Niubangzi (Arctii Fructus). If fetal qi rushes upward, urgently decoct Zhumagen (Bamboo Shoot Root) and Aye (Mugwort Leaf), grind Binglang (Areca Seed) into the soup, take it, then administer Siwu Tang. If heat is severe causing fetal instability, administer Gutai Yin (Fetus-Stabilizing Drink) for several doses. If there is no improvement, and abdominal pain and lower back pain occur, it indicates an inevitable miscarriage; if the fetus is expelled, treat according to obstetric protocols.
Shengma Gegen Tang (Cimicifugae and Puerariae Decoction)
Ingredients: Gegen (Puerariae Radix), Shengma (Cimicifugae Rhizoma), Baishao (Paeoniae Radix Alba), Gancao (Glycyrrhizae Radix et Rhizoma) in equal parts.
Preparation: Decoct one cup of water until reduced to 70%, take warm.
Analysis: This is a formula for resolving the exterior and inducing sweating. It applies to cases where exterior heat is vigorous and pathogens are excess at the exterior. The Classic states: "Light substances can remove excess." Therefore, Shengma and Gegen are used to disperse the exterior. The rationale is that Shengma resolves epidemic toxins and raises yang qi from beneath extreme yin, assisting the generating qi; Gegen resolves heat toxins and simultaneously disperses Ying and Wei, guiding the mechanism of eruption. Besides these two, Gancao is added to harmonize exterior qi, and Baishao is added to harmonize interior Ying blood. By removing excess pathogens and harmonizing Ying and Wei, wind-cold resolves naturally, and measles erupt spontaneously.
Gutai Yin (Fetus-Stabilizing Drink)
Function: Relieves pain and stabilizes the fetus.
Ingredients: Dihuang (Rehmanniae Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Guishen (Angelicae Sinensis Radix - body part), Renshen (Ginseng Radix), Baishao (Paeoniae Radix Alba), Chenpi (Citri Reticulatae Pericarpium) each 1 qian; Baizhu (Atractylodis Macrocephalae Rhizoma, earth-fried), Huangqin (Scutellariae Radix) each 1.5 qian; Gancao (Glycyrrhizae Radix et Rhizoma) 3 fen; Huanglian (Coptidis Rhizoma), Huangbai (Phellodendri Cortex) each 1 fen; Sangshangyangerteng (Celastrus Ramulus growing on mulberry, selecting those with seven round leaves).
Preparation: Finely cut all ingredients. Take 2-3 qian per dose, add 20 grains of glutinous rice, one cup of water, decoct to 70%, take warm. Add Renchen (Amomi Fructus) if pain is prominent; add Ejiao (Colla Corii Asini) if blood deficiency exists.
Cuisheng Wanquan Tang (Labor-Inducing Ultimate Safety Decoction)
Ingredients: Renshen (Ginseng Radix) 3-5 qian (greatly tonifies primal qi as Sovereign herb); Danggui (Angelicae Sinensis Radix) 3 qian (greatly tonifies Ying blood as Minister herb, remove root tip); Chuanxiong (Ligustici Chuanxiong Rhizoma) 1 qian (enters Liver channel to disperse stagnation, providing slight ascending nature so descending drugs gain efficacy); Taoren (Persicae Semen) 13 kernels (do not remove skin or tips, crush; bitterness removes old stasis, sweetness generates new blood, slipperiness promotes downward movement); Ganjiang (Zingiberis Rhizoma) 1 qian (warmth circulates blood fraction; stir-fry to black-yellow color so charred nature directs downward and suppresses upward movement); Zhigancao (Honey-fried Glycyrrhizae Radix et Rhizoma) 6 fen (slows drug action, allows Middle Jiao to receive tonification, prevents immediate downward collapse); Niuxi Shao (Achyranthis Bidentatae Radix - tip) 2 qian (descends to abdomen, traverses twelve channels, unblocks meridians so Qi and Blood efficacy can operate and push out); Honghua (Carthami Flos) 3 fen (large dose breaks blood, small dose invigorates blood and generates new; wine-fried); Rougui (Cinnamomi Cortex) 6 fen (8 fen in winter; peel and chop before decoction; acts as guide drug to lead herbs directly into blood fraction; warmth circulates and dissipates stasis, making labor easier); Jiaozao (Jujubae Fructus processed with glue) 2 pieces.
Preparation: Decoct in water, take warm before meals. If the parturient is robust or unable to tolerate Renshen, omit Renshen; the labor-inducing effect remains more than double that of Foshou San. Commentary: Women at labor face the life and death of mother and child in an instant. Hence, many formulas were created by ancients. Childbirth greatly damages Qi and Blood; difficult labor often stems from Qi and Blood deficiency. Postpartum diseases belong to severe Qi and Blood depletion. However, postpartum deficiencies originate from prenatal conditions. Now, establishing Wanquan Tang embodies the intent of Dasheng and Shenghua formulas, combining them into one to ensure ultimate safety. It has been frequently used with great efficacy, hence named "Ultimate Safety." First, regulate and tonify Qi and Blood; assist with dispersing stasis, descending, and warming the middle. When Qi and Blood are strong, they naturally facilitate labor induction—this is inducing labor without explicitly inducing it. Thus, Renshen and Danggui are Sovereigns to tonify Qi and Blood and strengthen the main force; slightly add Taoren, Chuanxiong, Heijiang (Charred Ginger), Zhigancao, and Jiuhonghua (Wine-fried Safflower) to warm the middle and dissipate stasis; Niuxi Shao and Guixin (Cinnamon Heart) warm and guide downward, preventing upward rushing to the heart. Not only does it induce labor divinely, but postpartum also avoids blood stasis stagnation, preventing hundred diseases. Tonifying with warmth prevents stagnation; warming with tonification prevents flooding. Less ascending, more descending allows Qi to be lifted and easily descend; descending with ascending allows stasis to go and new blood to return. More tonifying, less purging removes pathogens without injuring Yuan Qi; less bitter, more sweet drives away stasis while maintaining harmony. Is this not the ultimate labor-inducing formula?
