Qian Yi (courtesy name Zhongyang) stated: In the onset of convulsions due to fright and epilepsy, in boys, if the eyes look to the left, there is no sound; if they look to the right, there is a sound. In girls, if the eyes look to the right, there is no sound; if they look to the left, there is a sound. This is due to the principle of mutual overcoming among the Five Elements. Convulsions occurring in the morning are caused by tidal fever. During the Yin, Mao, and Chen hours (3:00 AM to 9:00 AM), the body has high fever, the eyes roll upward, and the limbs tremble. Saliva is produced during the day, and the neck and nape are stiff. This indicates hyperactivity of the Liver Qi. Treatment should tonify the Kidney and treat the Liver. To tonify the Kidney, use Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill); to treat the Liver, use Xieqing Wan (Drain the Green Pill) as the primary formula. Convulsions occurring at noon are caused by tidal fever. During the Si, Wu, and Wei hours (9:00 AM to 3:00 PM), the patient experiences fright and palpitations of the Heart Spirit, eyes rolling upward, redness in the whites of the eyes, tight jaw, saliva production, and limb tremors. This indicates hyperactivity of the Heart Qi. Treatment should tonify the Liver and treat the Heart. To treat the Heart, use Daochi San (Guide Out Red Powder) and Liangjing Wan (Cool the Fright Pill); to tonify the Liver, use Dihuang Wan (Rehmannia Pill) as the primary formula. Convulsions occurring in the evening are caused by tidal fever. During the Shen, You, and Xu hours (3:00 PM to 9:00 PM), convulsions are not severe but accompanied by panting. The eyes deviate slightly, the body feels hot, the eyes remain open while sleeping revealing the pupils, the hands and feet are cold, and the stool is pale yellow water. This indicates hyperactivity of the Lung Qi. Treatment should tonify the Spleen and treat the Heart and Liver. To tonify the Spleen, use Yihuang San (Benefit the Yellow Powder); to treat the Liver, use Xieqing Wan; to treat the Heart, use Daochi San as the primary formula. Convulsions occurring at night are caused by tidal fever. During the Hai, Zi, and Chou hours (9:00 PM to 3:00 AM), convulsions are not severe but sleep is unstable. The body is warm with a sensation of heat, the eyes are tightly fixed or deviated, there is phlegm rattling in the throat, the stool is white, milk and food are undigested, the infant is lethargic and unresponsive, and cannot absorb fluids. Treatment should tonify the Spleen and treat the Heart. To tonify the Spleen, use Yihuang San; to treat the Heart, use Daochi San and Liangjing Wan as the primary formulas. Whenever acute wind-stroke (Ji Jing Feng) is about to occur, first observe the child's clenched fist. If the fingers curl inward, it is Yin-shun (favorable); if outward, it is Yang-shun. For boys with left-hand convulsions and girls with right-hand convulsions, one must first use Tongguan medicine to open the orifices. It is strictly forbidden for saliva to obstruct the Lungs or逆流 into the Heart Meridian causing aphasia. In such cases, immediately administer medicines that clear the Heart, calm fright, and resolve phlegm to stabilize the condition. If the convulsions are Ni-chou (adverse), they are incurable. If these symptoms arise as a transformation after vomiting and diarrhea, they are difficult to regulate and treat. Generally, for this condition, observe the favorability of the fist clenching. If the convulsions are Shun (favorable), regulation is easy; if Ni (adverse), there is fear of recurrent tidal fever. First use Tongguan San to stimulate the infant, then Baoming Dan and Shengfeng Tang, followed by medicines that disperse wind, resolve phlegm, and reduce fever, which will resolve the issue. There is also fright-wind with convulsions, accompanied by vomiting, diarrhea, and frequent crying. In acute convulsions, if the eyes have slight tears and the face is red, recovery is difficult. Frequent crying with phlegm accumulation in the heart and red veins on the face indicates epilepsy. Random blue patterns on the fingers indicate disease transmission. This results from excessive sedative treatment for lower fright-wind, leaving wind pathogens unresolved, opposing Qi dynamics, causing uncontrollable convulsions. One should administer medicines that move Qi downward and disperse wind, take Wendan Tang with Suanzaoren (Sour Jujube Seed), and subsequently use Shengfeng Tang dissolved with Suqing Wan. In acute wind-stroke with unilateral convulsions, if the face is green and the back is cold, convulsions in the boy's left hand or girl's right hand are manageable; convulsions in the boy's right hand or girl's left hand are difficult to treat. If the palms have cold sweat, the eyebrows twitch, the abdomen convulses, and the condition persists day and night without relief, it is named Zhen-chou (True Convulsion). If both eyebrows twitch, it is Che-zheng (Tugging Syndrome). Not only must convulsions be stopped, but Qi must also be smoothed. Administer Renshen Tang (Ginseng Decoction) to level Stomach Qi, then proceed with Chuanwu (Aconite Root) and Quanxie (Scorpion) and other medicines, carefully adjusting the Qi-smoothing agents. Qian Yi discussed convulsions following wind-cold invasion: If acquired after wind-cold invasion, with hot breath, yawning, irritability, and limb tremors, one should disperse the exterior using Daqing Gao (Great Blue Ointment) as the primary formula. Convulsions following food injury: If acquired after food stagnation, with warm body, excessive sleep and salivation, vomiting, loss of appetite, and convulsions, one should first stop convulsions. After convulsions subside, use Baibingzi (White Cake Pill) to purge, then administer Anshen Wan (Calm the Spirit Pill). Convulsions in infants within 100 days: True cases die after two or three occurrences; false cases occurring frequently are not severe. True cases stem from internal fright-epilepsy; false cases from external wind-cold invasion. Because Qi and Blood are not yet solid, they cannot withstand the pathogen, leading to convulsions. To identify false cases, check if the breath is hot. Treatment should disperse the exterior using Daqing Gao as the primary formula, still employing the Fontanelle Smearing and Body Bathing methods. Use Zhenzhu Wan (Pearl Pill) to resolve phlegm and stabilize. If there is heat and constipation, use Qibao Xixin San (Seven-Treasure Heart-Washing Powder) and Kuanye San (Widen Heat Powder) to purge, which will be effective. Subsequently, use Tianma Fangfeng Wan (Gastrodia and Saposhnikovia Pill), Longchi San (Dragon Tooth Powder), and Gouteng San (Uncaria Powder) to regulate. If there are hot sores or scabies, or aphasia, Fangfeng Tongsheng San (Saposhnikovia Universal Rescue Powder) and Dalianqiao Tang (Large Forsythia Decoction) can be taken.
Xieqing Wan See Question 8.
Daochi San See Question 8.
Liangjing Wan (Symptoms: Tidal fever, excess saliva in the mouth, limb tremors, indicating Heart Qi hyperactivity.) Treats convulsions due to heat in fright-gan (malnutrition with heat), fright and palpitations, upward eye deviation, red face, and tight jaw.
Ingredients: Huanglian (Coptis Rhizoma, powdered, 5 fen), Longnao (Borneolum Syntheticum, 1 qian), Longdan (Gentiana Radix et Rhizoma, 3 qian), Fangfeng (Saposhnikovia Radix, 3 qian), Qingdai (Indigo Naturalis, 3 qian), Gouteng (Uncariae Ramulus cum Uncis, 2 qian), Niuhuang (Calculus Bovis, 1 zi), Shexiang (Moschus, 1 zi).
Grind all together uniformly, bind with wheat paste into pills the size of millet seeds. Take 3–5 pills up to 10–20 pills per dose, swallowed with decocted gold and silver water.
Dihuang Wan See Question 8.
Yihuang San See Question 8.
Tongguan San Treats various wind-induced convulsions and spasms where orifices are blocked, caused by phlegm blocking the Middle Jiao and stagnating in the joints. The reason phlegm floods like a tide is that Qi makes it substantial.
Ingredients: Nanxing (Arisaematis Rhizoma Praeparatum, processed, 1 qian), Shexiang (Moschus, 1 zi), Zhuyazao (Fructus Gleditsiae Sinensis, one pod, slightly burnt to retain properties), Chiwugong (Centipeda, red variety, one piece, roasted), Zhijiangcan (Bombyx Batryticatus, 1 qian).
Grind into powder. Dip finger in ginger juice, dip into a small amount of powder, and rub against teeth. Or use an instrument to guide, dripping 2–3 drops of medicine into the mouth. When saliva flows out, the blockage opens. This is generally used interchangeably with Kaiya San in Question 19.
Tiguan San
Ingredients: Shengbanxia (Pinelliae Rhizoma, raw, 1 qian), Zaojiao (Fructus Gleditsiae Sinensis, half qian).
Grind into powder. Take a bean-sized amount, blow into the nose with a tube to induce immediate awakening.
Kaiguan Rusheng San
Ingredients: Chiwugong (Centipeda, red variety, one piece, split in two, soaked in scallion juice overnight and dried), Quanxie (Scorpio, one piece, split in two, marking left and right sides separately).
Grind each into powder. If the left eye rolls up and left hand convulses, apply the following powder into the left nostril; if the right eye rolls up and right hand convulses, apply the above powder into the right nostril; if both eyes roll up and both hands convulse, use both.
Zhisheng Baoming Dan Treats infantile fetal fright and internal hooking, abdominal tightness and hardness, restless sleep, and frequent crying at night. Also treats acute and chronic wind-stroke, upward eye deviation, limb convulsions, and unconsciousness. All are indicated. Used for cold syndromes.
Ingredients: Quanxie (Scorpio, fourteen pieces, tails removed), Fangfeng (Saposhnikovia Radix, 2 qian), Baifuzi (Typhonii Rhizoma, 1 qian), Paonanxing (Arisaematis Rhizoma Praeparatum, 1 qian; use Dansheng/Dried Gallbladder-treated Arisaema if heat is present), Chantui (Cicadae Periostracum, 1 qian), Jiangcan (Bombyx Batryticatus, silk removed and stir-fried), Tianma (Gastrodiae Rhizoma, 2 qian), Zhusha (Cinnabaris, 1 qian), Shexiang (Moschus, half qian), Jinbo (Folium Aurum, ten leaves).
If heat syndrome is present, add Niuhuang (Calculus Bovis), Bingpian (Borneolum Syntheticum), and Pengsha (Borax) in small amounts.
Grind into powder, bind with glutinous rice paste into pills. Each liang of powder makes forty pills. Regularly taken to calm the Heart Spirit, resolve phlegm, and eliminate all wind-stroke syndromes. Can be made into decoctions for temporary changes. Another prescription adds Renshen (Ginseng Radix et Rhizoma) and Baifuling (Poria, 2 qian each).
Shengfeng Tang Treats wind-stroke with locked jaw, muscle spasms, convulsive pain, excess wind and phlegm, dizziness and sudden collapse, vertigo, chest and diaphragm fullness, mental confusion, and unconsciousness.
Ingredients: Tiannanxing (Arisaematis Rhizoma, raw), Fangfeng (Saposhnikovia Radix, de-rubbed, 4 liang each), Gancao (Licorice Radix et Rhizoma, raw), Banxia (Pinelliae Rhizoma, soaked in rice wash overnight), Huangqin (Scutellariae Radix, coarse bark removed, 1 liang each).
Cut into small pieces. Take 2 qian per dose, add one slice of fresh ginger, decoct, remove dregs, and take at any time.
Wendan Tang Treats fright, palpitations, irritability, and insomnia.
Ingredients: Banxia (Pinelliae Rhizoma Praeparatum), Zhishi (Aurantii Fructus Immaturus, 2.5 qian each), Fuling (Poria, half liang), Juhong (Citri Reticulatae Pericarpium Rubrum), Gancao (Licorice Radix et Rhizoma, 1.5 qian each), Suanzaoren (Zizyphi Semen, shelled, 2.5 qian).
