The younger brother of my clan member Yingzhang contracted San Yin Malaria (a type of malaria with deep-seated pathology) in the tenth month, which persisted until mid-spring of the following year. Attacks occurred on Chen, Xu, Chou, and Wei days, characterized by more chills than fever in the afternoon, and night sweats (sweating during sleep). The left pulse was weak, while the right Guan position (corresponding to the Spleen and Stomach) was wiry, rapid, and forceful. A decoction was prescribed containing Bai Shao Yao (Radix Paeoniae Alba), Dang Gui (Radix Angelicae Sinensis) each 1 qian; Bai Zhu (Rhizoma Atractylodis Macrocephalae) 2 qian; Chai Hu (Radix Bupleuri), Chuan Xiong (Rhizoma Chuanxiong), Fen Cao (Radix Glycyrrhizae Preparata), Sha Ren (Fructus Amomi), Gui Zhi (Ramulus Cinnamomi), and Jiu Qin (Radix Scutellariae Praeparata cum Vino) each 3 fen; and 3 slices of Sheng Jiang (Rhizoma Zingiberis Recens). This was taken after water decoction. Subsequently, a second prescription was prepared with He Shou Wu (Caulis Polygoni Multiflori), Bai Zhu, and Bie Jia (Carapax et Plastrum Testudinis) each 3 qian; Chai Hu 1 qian; Qing Pi (Pericarpium Citri Reticulatae Viride), Jiu Qin, and Gan Cao (Radix Glycyrrhizae) each 5 fen; one Mei Zi (Fructus Mume); and 3 slices of Sheng Jiang. This was decocted in water and administered at Wugeng (approx. 3-5 AM) on the day of the next attack. After two doses, the attacks ceased. Half a month later, large boils and sores appeared on the lower body, which were resolved through regulation with Dongpo Sishen Dan (Dongpo's Four Divine Pills).
Madam Wu Zhongyue first contracted Wind Pathogen (Feng Xie), followed by dietary injury, resulting in fever, headache, and abdominal distension. A physician administered Ba Dou Wan (Croton Seed Pill) for purgation, but the fever did not subside; another physician then used Da Huang (Radix et Rhizoma Rhei) for severe purgation, yet the fever remained unchanged. A subsequent physician reasoned that fever persisting after purgation indicated deficiency, and thus heavily supplemented with Ren Shen (Radix Ginseng), [text missing, likely Qi-tonifying herb], and Bai Zhu. After four days of supplementation, her spirit became lethargic and she lost consciousness. I was then consulted. The left pulse was wiry and rapid; the right Guan and Chi positions were deep, rapid, and forceful. The tip of the tongue was sedge-brown, while the root was scorched black with prickles. Speech was slurred due to tongue stiffness. Inquiry into previous treatments revealed erroneous purgation and supplementation. Considering this occurred amidst famine and epidemic prevalence, the mismanagement led to a Huai Zheng (complicated critical condition), which was dangerous and nearly fatal. Initially, I prescribed Zhi Mu (Rhizoma Anemarrhenae), Chai Hu each 3 qian; Shi Gao (Gypsum Fibrosum) 6 qian; Zhi Shi (Fructus Aurantii Immaturus), Tian Hua Fen (Radix Trichosanthis) each 5 fen; Fen Cao, Huang Qin (Radix Scutellariae), Mai Dong (Radix Ophiopogonis) each 1 qian; Shan Zhi Zi (Fructus Gardeniae), Sheng Di Huang (Radix Rehmanniae Preparata) each 7 fen; Ren Shen 6 fen; Zhu Ye (Leaf of Bambusa) 30 pieces; and Sheng Jiang 3 slices. Decocted in water and administered. After midnight, her consciousness slightly cleared, she had slight sweating, the tongue became softer, speech was no longer slurred, indicating the presence of vital energy. The next day, Sheng Di Huang was removed from the formula and Bai Shao Yao was added. The scorched black coating on the center of the tongue completely disappeared, and seven-tenths of the symptoms subsided. However, there had been no bowel movement for five days, the whole body still hurt, and she was coughing. Two doses of Qizhi Huatan Wan (Seven-Processed Phlegm-Resolving Pill) were given, followed by a decoction of Shi Gao 2 qian; Mai Dong, Bei Mu (Bulbus Fritillariae Cirrhosae) each 1 qian; Qian Hu (Radix Peucedani), Zhi Shi, Huang Qin, Zhi Zi (Fructus Gardeniae) each 6 fen; Gan Cao 3 fen; Sang Bai Pi (Cortex Mori Radicis) 8 fen. With this, the condition stabilized.
