Within half a month after childbirth, there is alternating chills and fever (alternating cold and heat), occurring in the afternoon, at dusk (3-5 PM), or at night. The fever has a regular schedule and resembles malaria. This is caused by the exhaustion of both Qi and Blood; Yang deficiency generates cold, while Yin deficiency generates heat. Do not treat it as malaria. Even Chai Hu (Bupleurum) should not be used. Only by regulating and tonifying Qi and Blood will the chills and fever naturally subside. Do not use Huang Qin (Scutellaria), Huang Lian (Coptis), Huang Bo (Phellodendron), or Zhi Zi (Gardenia) to reduce fever, nor Cao Guo (Tsaoko) or Bing Lang (Areca Seed) to arrest malaria. If there is sweating and shortness of breath, add Ren Shen (Ginseng); if heat is prominent, add Dang Gui (Angelica Sinensis) and Ren Shen (Ginseng). If one month has passed since delivery, Ren Shen Yang Wei Tang (Ginseng Stomach-Nourishing Decoction) may be used with modifications for treatment, along with separately decocting Ren Shen Zhu Gan Gao (Ginseng and Atractylodes Paste). Zhang Zhongjing stated: "In Shang Han (Cold Damage) disease, alternating chills and fever, occurring once, twice, or three times, indicates that both Yin and Yang are deficient. One must not use diaphoresis again, nor purgation or emesis." This principle is identical to the current condition.
Jia Wei Sheng Hua Tang (Modified Birth-Expelling Decoction): Treats malaria-like symptoms within half a month before and after childbirth.
Chuan Xiong (Ligusticum), Ren Shen (Ginseng), Bai Zhu (Atractylodes Macrocephala) (each 1 qian), Dang Gui (Angelica Sinensis) (2 qian), Gan Cao (Licorice) (0.3 qian), Bai Fu Ling (Poria) (8 fen), Huo Xiang (Patchouli) (8 fen), Qing Pi (Citrus Reticulata Green Peel) (2 qian), Wu Mei (Mume Fruit) (1 piece).
If thirsty, add Mai Dong (Ophiopogon) 1 qian and Wu Wei Zi (Schisandra) 10 grains; if phlegm is present, add Ban Xia (Pinellia) 0.7 qian and Sheng Jiang (Fresh Ginger) 3 slices; if sweating is excessive, add Huang Qi (Astragalus) and Suan Zao Ren (Ziziphus) each 1 qian.
Jia Wei Ren Shen Yang Wei Tang (Modified Ginseng Stomach-Nourishing Decoction): Treats malaria during and after the first month postpartum, combined with the use of Ren Shen Zhu Gan Gao (Ginseng and Atractylodes Paste).
Ren Shen (Ginseng) (1.5 qian), Bai Zhu (Atractylodes Macrocephala) and Dang Gui (Angelica Sinensis) (each 2 qian), Fu Ling (Poria) and Ban Xia (Pinellia) (each 0.8 qian), Cao Guo (Tsaoko) (0.3 qian), Gan Cao (Licorice) and Qing Pi (Citrus Reticulata Green Peel) (each 0.4 qian), Huo Xiang (Patchouli) (0.5 qian), Wu Mei (Mume Fruit) (3 pieces).
Additionally, take 4 liang of Bai Zhu (Atractylodes Macrocephala), wash, chop, and dry it; take 4 liang of Ren Shen (Ginseng). Decoct each in 6 bowls of water until half a bowl remains. Repeat this process three times according to the method. Combine the dregs and juices to obtain 9 bowls total. Further simmer until reduced to 1 bowl. Take half a wine cup daily, washed down with plain boiled water.
For postpartum breast abscesses that have not yet suppurated, consume Gua Lou Ru Mu San (Trichosanthes, Frankincense, and Myrrh Powder). If pus has already formed despite medication, consume Pai Nong Hui Du San (Pus-Draining and Toxin-Expelling Powder). These two formulas are contraindicated for patients with deficiency constitutions. After pus discharge, consume Shi Quan Da Bu Jin Yin Hua San (Ten Complete Tonifying and Honeysuckle Powder).
Gua Lou Ru Mu San Formula:
Gua Lou (Trichosanthes Fruit) (1 piece, crushed with peel), Dang Gui (Angelica Sinensis) and Jin Yin Hua (Honeysuckle) (each 3 qian), Bai Zhi (Angelica Dahurica) (1 qian), Qing Pi (Citrus Reticulata Green Peel), Ru Xiang (Frankincense), Mo Yao (Myrrh), Gan Cao (Licorice) (each 0.5 qian).
Abscesses occurring during pregnancy can also be treated with this formula with good effect.
Another prescription: Pu Gong Ying (Dandelion) 8 qian, Jin Yin Hua (Honeysuckle) 1 liang.
Decoct in wine and drink until full; the effect is miraculous.
Shi Quan Da Bu Tang (Ten Complete Tonifying Decoction):
For postpartum patients with severe weakness after pus discharge.
Ren Shen (Ginseng), Bai Zhu (Atractylodes Macrocephala), Shu Di Huang (Rehmannia), Huang Qi (Astragalus) (each 2 qian), Fu Ling (Poria) and Chuan Xiong (Ligusticum) (each 0.8 qian), Dang Gui (Angelica Sinensis) and Jin Yin Hua (Honeysuckle) (each 3 qian), Gan Cao (Licorice) (0.5 qian).
Decoct 2 zhong of water until 0.6 zhong remains.
If diarrhea occurs, add 10 lotus seeds and 1 nutmeg; if thirsty, add 1 qian of Mai Dong (Ophiopogon) and 10 Wu Wei Zi (Schisandra).
Postpartum abscesses presenting with chills and fever resembling malaria should be treated as a deficiency syndrome. Lochia (postpartum discharge) that fails to disperse for a long time may coagulate into masses.
Normally, after delivery, lochia flows out freely, resulting in no abdominal pain and a feeling of comfort. If the abdomen is exposed to cold or if cold foods are ingested, lochia stagnates and forms clumps. This gives rise to various deficiency syndromes: abdominal pain, body heat, bone-steaming tidal fever, five-palm heat, poor appetite, emaciation, symptoms resembling malaria, amenorrhea, masses located in the hypochondrium, borborygmus and rumbling upon pain onset, vertigo, intermittent fever, etc.
The treatment method should follow Zhu Danxi's advice: "To drain the pathogen, one must first tonify the deficiency." Use Bu Zhong Yi Qi (Tonify the Middle and Augment the Qi) methods. Zhu Danxi compiled San Xiao Wan (Three Elimination Pills) to dissolve masses without weakening the patient. If masses are not treated with tonification, they will not completely resolve, appetite will decrease sharply, and the patient may even stop eating, leading to consumptive disease and premature death.
Jia Wei Bu Zhong Yi Qi Tang (Modified Tonify the Middle and Augment the Qi Decoction):
Ren Shen (Ginseng), Shan Yao (Dioscorea), Huang Qi (Astragalus) (each 1 qian), Bai Zhu (Atractylodes Macrocephala) (2 qian), Dang Gui (Angelica Sinensis) (3 qian), Chen Pi (Citrus Reticulata Peel), Gan Cao (Licorice) (each 0.4 qian).
Decoct with ginger water.
Treats masses formed by stagnant blood, food retention, and phlegm in women.
Huang Lian (Coptis) 1.5 liang. Take 1 liang of Huang Lian, soak it in juice decocted from 4 qian of Wu Zhu Yu (Evodia) for one liang, then remove the dregs. Stir-fry the soaked Huang Lian until dry (5 qian). Yi Zhi Ren (Alpinia Oxyphylla) (stir-fried, heart removed), Luo Bo Zi (Radish Seed) 1.5 liang (stir-fried), Tai Wu (Aucklandia), Tao Ren (Peach Kernel), Zhi Zi (Gardenia) (each stir-fried, 5 qian). Mai Ya (Barley Sprout), San Leng (Kyllinga), E Zhu (Curcuma) all stir-fried with vinegar.
Stir-fry the above ingredients until each weighs 5 qian. Xiang Fu (Cyperus) 1 liang (soaked in child's urine, then stir-fried), Shan Zha (Hawthorn) 1 liang. Grind all into powder. Make pills using steamed wheat cake. Take 50-60 pills after meals, delivered with Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction). Alternatively, deliver with a decoction of 3 qian Bai Zhu (Atractylodes Macrocephala) and 0.5 qian Chen Pi (Citrus Reticulata Peel) in 1 zhong of water, reduced to 0.5 zhong.
Constipation after childbirth is due to blood deficiency and intestinal dryness. Weak postpartum women who frequently consume Sheng Hua Tang (Birth-Expelling Decoction) will have abundant blood and smooth Qi flow, naturally resolving constipation. One must absolutely avoid using Xi Xiao (Mirabilite) or Huang Lian (Rhubarb) as purgatives, as this severely depletes Yin and Blood, worsening constipation. If this leads to abdominal distension or uncontrollable diarrhea, one should treat with modified Sheng Hua Tang (Birth-Expelling Decoction).
Zhu Xue Run Chang Wan (Blood-Assisting and Intestine-Moistening Pills):
Treats postpartum constipation or abdominal distension caused by erroneous use of purgatives.
Chuan Xiong (Ligusticum) (1 qian), Dang Gui (Angelica Sinensis) (4 qian), Tao Ren (Peach Kernel) (10 grains), Gan Cao (Licorice) (0.5 qian), Ma Ren (Hemp Seed) (1.5 qian, stir-fried), Chen Pi (Citrus Reticulata Peel) (0.4 qian).
For pain due to blood clumps, add Rou Gui (Cinnamon Bark) and Yuan Hu Suo (Corydalis) each 0.5 qian. Decoct in 2 zhong of water until 0.7 zhong remains. Take slightly warm before meals.
For Qi deficiency with excessive sweating, add 1-2 qian of Ren Shen (Ginseng) and 1 qian of Huang Qi (Astragalus). For excessive sweating with thirst, add 1-2 qian of Ren Shen (Ginseng), 2.5 qian of Mai Dong (Ophiopogon), and 8 Wu Wei Zi (Schisandra).
If feces are dry and hard for more than ten days, there will likely be dry stool in the anus. Use honey date suppositories. Simmer honey until it becomes a brown paste, shape into dates, insert into the anus, and the dry stool will liquefy and pass. Alternatively, inserting a candle may also help liquefy it.
Another method: Hold sesame oil in the mouth, insert a bamboo tube into the anus, and blow 4-5 mouthfuls of oil in. The feces in the abdomen will pass immediately. Alternatively, use one pig gallbladder and insert a bamboo tube as described above.
Postpartum delirium and hallucinations are caused by severe deficiency of Qi and Blood, loss of essence, and confusion of the spirit, leading to false perceptions and speech. Mild cases involve muttering in dreams; severe cases involve sleeplessness and excessive talking. Phlegm-turbidity invades the Middle Jiao due to deficiency, causing dysfunction of the five sense organs, resulting in false vision, hearing, speech, and movement. Do not mistake this for ghostly possession; using talismans and water erroneously may lead to fatal consequences. Zhu Danxi said: "Deficiency syndromes sometimes resemble demonic possession." I have treated this syndrome many times, and it responds well after taking several doses of medicine.
Jia Wei Sheng Hua An Shen Tang (Modified Birth-Expelling and Spirit-Calming Decoction):
Treats delirium and hallucinations within three days postpartum when blood clumps remain. Take 3-4 doses, then modify based on response.
Chuan Xiong (Ligusticum) (2 qian), Dang Gui (Angelica Sinensis) (4 qian), Fu Ling (Poria) (1 qian), Gan Cao (Licorice) (0.4 qian), Gan Jiang (Dried Ginger) (0.4 qian), Suan Zao Ren (Ziziphus) (1 qian), Tao Ren (Peach Kernel) (10 grains), Da Zao (Jujube) (2 pieces).
Decoct 2 zhong of water until 0.6 zhong remains. Take after meals.
Yi Rong An Shen Tang (Nourish Prosperity and Calm Spirit Decoction):
Treats hallucinations and delirium with extreme deficiency, where blood clumps do not cause pain, within three days before or after childbirth. If the medicine brings stability but no major improvement, wait for the medicinal power to accumulate, and all symptoms will suddenly resolve. Taking 20 doses often yields complete recovery.
Chuan Xiong (Ligusticum) (1.5 qian), Dang Gui (Angelica Sinensis) (3 qian), Fu Ling (Poria), Ren Shen (Ginseng), Bai Zi Ren (Biota Seed) (each 1 qian), Suan Zao Ren (Ziziphus) (1 qian), Gan Cao (Licorice) (0.5 qian), Long Yan Rou (Longan Aril) (8 pieces), Chen Pi (Citrus Reticulata Peel, pith removed) (0.3 qian), Zhu Ru (Bamboo Shavings) (2 pills).
For excessive sweating, add 1 qian Huang Qi (Astragalus) and 1 qian Ma Huang Gen (Ephedra Root); for diarrhea, add 1.5 qian Bai Zhu (Atractylodes Macrocephala); for phlegm, add a small wine cup of Zhu Li (Bamboo Sap) and a teaspoon of Jiang Zhi (Ginger Juice); for constipation, add 1.5 qian Ma Ren (Hemp Seed). Absolutely do not use Da Huang (Rhubarb).
If postpartum milk production is insufficient and one cannot afford to hire a wet nurse, forcing breastfeeding causes exhaustion for both mother and infant, along with reduced food intake. One should urgently wean the infant and quickly take the following prescription.
Chuan Xiong (Ligusticum), Dang Gui (Angelica Sinensis), Huang Qi (Astragalus), Mai Dong (Ophiopogon), Fu Ling (Poria) (each 1 qian), Zhi Gan Cao (Prepared Licorice) (0.5 qian), Wu Wei Zi (Schisandra) (15 grains), Ren Shen (Ginseng) (2 qian), Da Shu Di (Rehmannia), Bai Zhu (Atractylodes Macrocephala) (each 2 qian), Chen Pi (Citrus Reticulata Peel) (0.4 qian), Zao (Jujube) (2 pieces).
Decoct 2 zhong of water until 0.6 zhong remains.
If fever accompanies bone-steaming tidal fever, concurrently take Zi He Che Wan (Placenta Pills). For fright with sweating, add 1 qian of Suan Zao Ren (Ziziphus).
For postpartum lactating mothers with breast abscesses that have ruptured and discharged pus, presenting with alternating chills and fever like malaria (once daily, 2-3 times daily, or every two days), do not treat as malaria. Do not solely use toxic-clearing and abscess-resolving drugs, nor antimalarial agents. Instead, tonify Qi and Blood, and lightly supplement with Jin Yin Hua (Honeysuckle) and other detoxifying herbs to dissipate residual pathogens.
Huang Qi (Astragalus), Jin Yin Hua (Honeysuckle), Fu Ling (Poria) (each 1 qian), Ren Shen (Ginseng), Bai Zhu (Atractylodes Macrocephala), Sheng Di Huang (Rehmannia) (each 2 qian), Gan Cao (Licorice), Lian Qiao (Forsythia) (each 4 qian), Dang Gui (Angelica Sinensis) (2 qian), Qing Pi (Citrus Reticulata Green Peel) (0.3 qian), Bai Zhi (Angelica Dahurica) (0.5 qian), Wu Mei (Mume Fruit) (1 piece), Zao (Jujube) (1 piece).
Decoct in 2 zhong of water and take.
