The Classic states: Shang Han (Cold Damage) has five categories: Zhong Feng (Wind Stroke), Re Bing (Heat Disease), Wen Bing (Warm Disease), Zhong Shi (Dampness Strike), and Shang Han (Cold Damage). Their suffering differs. These five correspond to the Five Qi and cause disease according to the four seasons. Thus, spring wind, summer heat, autumn dampness, winter cold, and warmth strike the body, while Shang Han is but one of them. (Detailed discussion is in the Collection of Shang Han Syndrome Differentiation). The reason why diseases of the four seasons are collectively called "Shang Han" is that it is an umbrella term. Among them, "Re Bing" refers to illness caused by intense heat qi striking the body during peak summer. "Zhong Shi" refers to illness caused by dampness qi from rain, dew, frost, fog, mist, or water stains striking the body. Children do not suffer from these two diseases because they do not engage in household chores, have no exposure to such dampness, and sleep early and rise late, avoiding exposure. However, children can contract external wind, Shang Han, and Wen Bing. This is because severe cold, fierce winds, and warm qi are ubiquitous, and children have fragile muscles and weak qi and blood, making them susceptible. Therefore, for these three diseases, symptoms and pulse cannot be diagnosed solely by palpation. For slightly older children, pulse diagnosis can distinguish them. The pulse of Shang Feng (Wind Injury) is floating on the exterior (Yang) and weak on the interior (Yin). The pulse of Shang Han is both Yang and Yin pulses full and tight/choppy. The pulse of Wen Bing circulates through various meridians; without knowing which meridian is affected, palpate along the affected meridian. For infants too small for pulse diagnosis, observe their physical signs and manifestations only.
Shang Feng (Wind Injury) is caused by harsh winds from eight directions in all seasons striking the body. Manifestations include drowsiness, yawning, lethargy, and mental cloudiness. Breath is hot from the mouth, or spontaneous sweating, aversion to wind, fever, nasal congestion with humming sound, dry retching. Heat predominates over cold. Face is bright and not sallow. Hands and feet are hot. Irritability. Treatment requires dispersing and releasing the exterior. Some may desire water, some can eat, some cannot. Those who desire water and can eat may be treated with mild purgation; others should not be purged.
Shang Han (Cold Damage) is caused by severe cold qi in winter striking the body. Cold enters the interstices (Cou Li). At this time, robust individuals' qi flows freely and remains healthy, while frail individuals' qi stagnates and becomes diseased. Immediate onset is called Shang Han. Onset in spring is called Wen Bing. (Another view: Before Summer Solstice, non-epidemic warm qi causes disease). Onset in summer is called Re Bing. (Another view: After Summer Solstice, non-heat-strike heat disease). Manifestations include red face, high fever throughout the body, headache, body ache, nasal congestion, heavy voice, clear runny nose, cough, piloerection, rapid breathing, or vomiting clear water. Cold predominates over heat. Face is sallow and uncomfortable. Fingertips and toe tips are slightly cold. No irritability, but aversion to cold. (Shang Feng involves aversion to wind, not cold; Shang Han involves aversion to cold, not wind. Aversion to wind means chills only when wind blows; aversion to cold means chills even without wind. Distinguish by this). Treatment: If pulse is surging and full, body is hot, headache, red face, warm limbs, it is a Yang syndrome, suitable for sweating (diaphoresis). If pulse is faint and thin, body is cold (not severe), headache, body ache, blue-green face, cold limbs, it is a Yin syndrome, suitable for warming.
Wen Bing (Warm Disease) is caused by abnormal pathogenic qi in the four seasons striking the body. For example, spring should be warm but is cold; summer should be hot but is cold; autumn should be cool but is hot; winter should be cold but is warm. It refers to illness caused by prevailing climate at that time, affecting young and old similarly, hence named "Tian Xing Shi Qi" (Heavenly Traveling Seasonal Qi) or "Shi Xing Wen Bing" (Seasonal Warm Disease). Manifestations include headache, heavy body, high fever, body ache, similar to Shang Han. But clinically observe pulse and syndrome to determine which meridian is affected and treat accordingly. The above three diseases are generally treated as Shang Han. Pathogen outside the exterior: resolve and sweat. Pathogen inside the interior: warm or purge. Follow syndrome and method. Qian Yi stated that Shang Han presents with heavy body and yellow face in boys, panting and red face in girls, each with aversion to cold, hot breath, yawning, lethargy, neck stiffness. This is a general statement不分 distinguishing Shang Feng and Shang Han. Moreover, Shang Feng and Shang Han have diverse transmission patterns, relying on specific meridians. Sun Simiao said treating children is no different from adults, only reducing the dosage of decoctions slightly. There is also Shang Feng Han combined with food retention, commonly called "Jia Shi Shang Han" (Cold Damage with Food Retention). Manifestations include high fever, irritability, thirst, nasal congestion, heavy voice, limb fatigue, yellow eyes, abdominal distension and pain, saliva/vomit, or yawning/lethargy. Treatment: Do not attack the retained food first; first release the exterior. Wait for exterior resolution, then purge. Then disease and food will naturally leave. If ignoring the exterior and purging directly, it often leads to Pi Qi (fullness obstruction). Also, eating cold drinks/food due to hunger damages Spleen and Stomach, then contracting wind-cold. Manifestations: chest/diaphragm discomfort, abdominal distension and pain, blue-green face and lips, cold extremities, deep/fine pulse. This is cold damaging Spleen, belonging to Tai Yin meridian. Quacks fail to recognize this, assuming chest discomfort warrants acupuncture or cold herbs, worsening chest discomfort and causing vomiting/diarrhea. Within one or two days, critical danger ensues. This is error due to ignorance, not correct treatment. Shang Han has many drugs and syndromes in the classic, hard to list fully. Now selecting formulas for common pediatric cases and accompanying syndromes, briefly listed. Shang Feng Han with cold hands/feet, fever, red face, headache indicates Spleen deficiency. Harmonize Spleen first, then disperse. Shang Feng Han with spontaneous diarrhea indicates Spleen deficiency. Tonify Spleen first, then disperse. If not cured, regulate middle Jiao first. If purgation indication exists, mild purge. If fright exists, calm fright.
Shang Feng Han with abdominal distension and wheezing indicates Spleen deficiency. Tonify Spleen first, reduce distension, stop wheezing. Wait for wheezing to stop, then disperse.
Shang Feng Han after purgation, major symptoms subsided but residual heat remains, indicates excessive purgation, Stomach deficiency allowing heat to invade. Stomach deficiency heat, little desire for water, should generate Stomach fluids. Once fluids generated, heat and thirst cease. (Above is Qian Yi's discussion, formulas recorded in this collection).
Shang Feng Han with heat outside is Exterior disease. If not entered interior but purged too early, interior deficiency leads to rebellious qi, heat knots in chest, hard and painful, this is Jie Xiong (Chest Binding). If hard/full but not painful, this is Pi Qi (Fullness Obstruction). If caused by drinking water, it is Shui Jie (Water Knot). If after 3-4 days heat enters interior, abdominal fullness/pain, internal excess with dry feces, must purge. If heat enters Large Intestine, constipation difficult/unobstructed. If heat enters Small Intestine, dysuria difficult/unobstructed. Heat entering both intestines, both urination and defecation obstructed. Heat entering Spleen/Stomach (Earth element, manifests in muscles), heat steaming externally causes jaundice. Heat steaming Heart/Chest causes irritability. Fluid depletion causes thirst. Heat rushing up throat/mouth/tongue causes sores/swelling/pain. Blood leakage: heat causes blood to flow wildly. From nose: Bi Hou (Epistaxis). From mouth: Tu Xue (Hematemesis). From stool: Bian Xue (Melena/Hematochezia). From urine: Yi Xue (Hematuria). If high fever without sweat, restless panting, nasal humming, epistaxis of 1 sheng several he allows heat to subside, disease resolves. If bleeding reaches 2-3 sheng, critical. If heat toxin accumulates internally, unable to purge timely, toxin attacks blood stasis, severe irritability/madness. If blocked, coma, confusion, death. If Stomach cold clashes, vomiting/cough/reversal. If Lung cold clashes, cough/rapid breathing. If Qi deficient, transmitting to Yin, cold extremities, blue face, cold body, diarrhea with undigested food, all treat according to syndrome.
Qian Yi discussed Shang Feng, Shang Han, rash, food retention, high fever, drowsiness, all similar. If unable to distinguish hastily, use Ge Gen Tang (Kudzu Decoction), Xing Xing San (Awakening Powder), Xiao Chai Hu Tang (Minor Bupleurum Decoction). These few medicines treat universally, very effective.
Sheng Hui Fang states: Infant Shang Han, do not use drying herbs to induce sweat. Dryness adds to Zang-Fu, extreme heat damages Heart, leading to inversion and difficult treatment. Do not use hot-natured herbs, as they assist Yang. Extreme Yang causes Yin struggle, limbs sweat like oil, hands/feet cold or hot, often leading to madness, epilepsy, convulsions, difficult treatment. Do not use cold-natured herbs, as cold makes Yang suffer chill, cold-heat clash, constant crying/sleeplessness, heat rushes to brain, skull sutures open, difficult treatment. I was stranded in Qiantang, teaching at my relative Su Bozheng's house. Su Bozheng was famous for treating Shang Han. I often observed his medication sequence.
If infant Shang Han with high fever, headache, body ache, large pulse, nasal congestion, sneezing/coughing, yawning, aversion to wind/cold, exterior disease suitable for sweating, use Yin Bai San (Silver White Powder). If more heat, use Gan Lu San (Sweet Dew Powder). If food/drink retention, Spleen/Stomach disharmony, alternate An Wei Wan (Stomach-Anchoring Pill). If chest/diaphragm discomfort, alternate Xiang Ke San (Fragrant Shell Powder). If irritability/thirst, or alternating chills/fever, alternate Ling Sha San (Poria-Arena Powder). If fright, use Tian Ma Fang Feng Wan (Gastrodia-Saposhnikovia Pill), Ren Shen Hua Feng Gao (Ginseng Wind-Transforming Paste). If excessive phlegm/cough, use Xiao Zhen Zhu Wan (Little Pearl Pill). Such treatment succeeds in 80-90% cases. Remaining 10-20% are long-standing, deeply transmitted, or complicated cases, seeking doctor late, still cured by following methods. Medications used are mostly not ancient formulas, nature not drying/hot, all warm/neutral.
