Tonsillitis (Ru E) presents as unilateral, bilateral, or pearl-string types with white purulent powder, often caused by excessive alcohol consumption and sexual activity leading to stagnant heat. On the first day, there is pain; on the second day, swelling; and on the third day, fine star-like white spots appear. Lesions on the left side of the throat belong to the Lung Meridian, while those on the right belong to the Heart Meridian. Unilateral tonsillitis shows white spots on one side only; bilateral tonsillitis has white stars on both sides; pearl-string tonsillitis has white stars above and below. The severity increases from unilateral (lightest) to bilateral (heavier) to pearl-string (most severe). Within four to five days of onset, medicated powders for insufflation can be used, preferably Yudan (Jade Pill), sparingly using Jindan (Gold Pill). If phlegm and saliva are obstructing, decoctions should be taken: for left-sided lesions, use Huanglian Xijiao Tang (Coptis Rhizome and Rhinoceros Horn Decoction); for right-sided lesions, add Chai Hu (Bupleurum Root) and Chishao (Red Peony Root); if constipation occurs, add Yuanmingfen (Mirabilite). If redness and swelling persist for three days without white spots, it indicates Houyong (Throat Abscess), which is easy to cure. Early-stage throat abscesses present with pain and swelling but no visible form, accompanied by chills, fever, and headache, due to excess Lung Fire caused by overconsumption of rich, fried foods; this condition is easily curable. Houquan (Throat Lichen/Tuberculosis) results from deficient fire flaring up, characterized by thread-like red vessels in the throat, requiring clear identification. It causes difficulty in swallowing, slight pain, and if untreated for a long time, leads to hoarseness or loss of voice. Houjun (Throat Fungus/Polyp) arises from melancholy and Qi stagnation, resembling high clouds or floating duckweed, appearing purple near the throat, mostly affecting women. Dousai Yong (Cheek Swelling Abscess) occurs under the cheekbones, caused by heat-toxin from consuming rich foods. Pediatric Me Kou (Childhood Tonsillar Mouth) is also due to heat-toxin, presenting with tongue coating and tonsillar mouth shape. Yellow tongue coating indicates Heart Meridian fire; floating pain on the tongue with fresh yellow coating suggests similar heat. Pearl-string tonsillitis stems from melancholy, while a mouth full of greasy residue indicates deficiency syndromes. Bengsha Kougan (Cracked Sand Mouth Ulcers) are mostly red; She Xia Ya Gan (Sublingual Gum Necrosis) involves swollen, painful gums, foul breath, rotting gum roots, and even tooth loss, which is alarming. Houci (Thorn Throat) features many red spots resembling mosquito bites and can be fatal. Hou Bi (Throat Closure/Acute Airway Obstruction) occurring after Shanghan (Cold Damage) is almost invariably fatal with few effective treatments. For all these oral and tongue conditions, careful syndrome differentiation and medication selection are essential. Mild cases can be treated with Siwei Kougan San (Four-Ingredient Mouth Ulcer Powder) via insufflation; severe cases require Jindan (Gold Pill) or Biyu San (Jasper Powder). Shibao Jinhuang San (Ten-Treasure Golden Yellow Powder) can be taken internally and applied externally for miraculous effects. Xionghuang Jiedu Wan (Realgar Detoxification Pill) is most effective, capable of reviving the dying through profound mysteries. The Tongji Formula for Throat, Tongue, and Gum Roots is universally applicable, adjusted temporarily based on the affected meridian. One must keep the intention to benefit humanity in mind and understand that this is the true secret transmission of the You family.
Jiegeng (Platycodon Root), Qianhu (Peucedani Radix), and Gegen (Pueraria Root), combined with Lianqiao (Forsythia Fruit), Zhizi (Gardeniae Fructus), and Huangqin (Scutellaria Root); Gancao (Licorice Root), Bohe (Menthae Haplocalycis Herba), and Niubangzi (Arctii Fructus); Jinyinhua (Lonicerae Japonicae Flos) detoxifies, assisted by Xuanshen (Scrophulariae Radix). If chills and fever are present, add more Qianhu; for mouth pain, add Shigypsum (Gypsum Fibrosum) to clear Stomach Fire; for thirst, add Maidong (Ophiopogonis Radix) and Zhimu (Anemarrhenae Rhizoma); for fullness, add Zhike (Aurantii Fructus); for Qi stagnation, use Yujin (Curcumae Radix), Beimu (Fritillariae Bulbus), and Chishao (Paeoniae Radix Rubra). These constitute the general decoction formula for throat diseases.
