[Acute and Chronic Convulsions] Three Sections in Total
*Yangsheng Biyong* discusses: Infantile convulsions and epilepsy were referred to as "Yin-Epilepsy" (Epilepsy of Yin nature) or "Yang-Epilepsy" (Epilepsy of Yang nature) in ancient medical classics, whereas modern practitioners call them "Acute Convulsion" (Ji Jing) and "Chronic Convulsion" (Man Jing). Current prescriptions are formulated strictly following the methods of ancient sages. Yang-Epilepsy belongs to the Fu organs; warming herbs should be removed from epilepsy formulas. Yin-Epilepsy belongs to the Zang organs; warming herbs should be included in epilepsy formulas. Cold and warming herbs are adjusted within epilepsy formulas to ensure accuracy. Additionally, infantile worm infestation symptoms resemble epilepsy; students must distinguish them carefully.
Qian Yi's supplementary discussion: The terms "Acute Convulsion" and "Chronic Convulsion" do not appear in ancient texts, which only mention Yin-Epilepsy and Yang-Epilepsy. These terms were coined by later generations, similar to how dysentery is classified into red and white types. Yang energy is active and rapid, hence Yang diseases are called Acute Convulsion; Yin energy is quiet and slow, hence Yin diseases are called Chronic Convulsion. This reflects distinctions in Yin/Yang, Deficiency/Excess, and Cold/Heat, which must not be confused during treatment. Both acute and chronic convulsions arise from Heat, which generates Wind. They may also be triggered by fright, resulting in upward deviation of the eyes, tidal flow of phlegm-saliva, spasms, and heat in the body and breath. Once the episode subsides or upon waking, the child regains consciousness clearly, characterizing the Acute Convulsion syndrome. When the spasm intensity begins to decrease, administer one or two doses of Heart-calming and Heat-clearing medicines (such as *Shexiang Wan* (Musk Pill), *Zhenxin Wan* (Heart-Calming Pill), *Baolong Wan* (Embrace Dragon Pill), *Chensha Wan* (Cinnabar Pill), *Zhibao Dan* (Precious Treasure Elixir), or *Zixue* (Purple Snow) from *Xiao'er Yaozheng Zhijue*). After the convulsive state stabilizes for a moment, use medicine to purge Phlegm-Heat (such as *Lijing Wan* (Phlegm-Purging Pill), *Ruanjindan* (Soft Gold Pill), *Taozhi Wan* (Peach Twig Pill), or herbs like *Dahuang* (Rhubarb Root) and *Puxiao* (Mirabilite)). Purging Phlegm-Heat leads to a calm mind and recovery.
Qian Yi states: When an infant suffers from Acute Convulsion and is spasming, do not cause panic or disturbance; this is not frightening. Although Chronic Convulsion appears quiet, it is a critical condition. During Acute Convulsion spasms, support the child but do not forcibly restrain them. Since Wind pathogen is strong, forceful restraint might drive it into the sinews, causing limb contracture.
Qian Yi's supplementary discussion on treating acute and chronic convulsions notes that people often use these methods indiscriminately. A single herb may have warming or cooling properties; they must not be used generically but selected after careful differentiation. For Chronic Convulsion treatments, *Longnao* (Borneol) should be excluded. If it must be combined, it should be used with warming herbs as assistants or in small amounts.
*Wanquan Fang* discusses formulas for various infantile winds, including *Tiandiao* (Heavenly Suspension) and *Kewu* (Guest Intrusion): Infants may present with Acute Convulsion, Chronic Convulsion, *Tiandiao*, or *Kewu*. These conditions share similarities but have minor differences. Sudden high fever with limb jerking, phlegm obstruction, and locked jaw is called Acute Convulsion. High fever with mental restlessness, vomiting of phlegm-saliva, frequent night terrors, alternating episodes of agitation and calm lasting days, is called Chronic Convulsion. Both result from internal accumulated Heat and external Wind invasion, differing only in speed of onset. *Tiandiao* originates from convulsion symptoms, named so because limbs spasm and eyes roll back like a fish on a hook. Generally, Fright generates Heat, and Heat generates Wind. Referring to the disease as a whole is "Convulsion-Wind"; referring to the specific symptom posture is "Tiandiao". Treatments are identical. *Kewu* implies being startled or offended by outsiders. Infants lack discernment; sudden arrival of strangers causes fright and offense, termed *Kewu*. Ancient texts state that odors from clothes worn outside or from cattle/horses can cause "offense". Symptoms include vomiting and purging of green, yellow, red, or white fluids, abdominal pain with twisting, facial color changes, appearance resembling epilepsy but without upward eye deviation, and a wiry, rapid pulse. Treatments may include *Fenwan* (Powder Pill) or magical arts.
*Yu Jue* poem on Infantile Convulsion Syndrome:
Green face and yawning indicate Fright; crying and restless sleep follow.
Rapid breathing and frequent fever rise; milky intestines weak, diarrhea green arises.
Treatment order: First remove Fright-Qi, then subdue Convulsion-Wind, next purge Phlegm-Saliva, finally regulate Stomach-Qi to achieve peace.
*Yu Jue* on treating Convulsion-Wind: This condition arises from Fright accumulating Phlegm-Saliva, obstructing the chest and diaphragm. Use *Shengyin Wan* (Raw Silver Pill) or *Wuxi Gao* (Black Rhinoceros Horn Ointment) to move Wind and descend Phlegm. (Formulas see this section.)
*Xianren Shuijian* Sleep-Fright Pill for Infants:
*Tian Nan Xing* (One large tuber, soaked in wine, pounded to powder) *Ruxiang* (Frankincense) *Shuiyin* (Mercury, made into cinnabar grains) *Hupo* (Amber, powdered. Each 1 qian) *Niuhuang* (Calculus Bovis) *Bailongnao* (White Borneol. Each 0.5 qian) *Qingdai* (Indigo Naturalis. 3 qian)
Grind finely, mix with *Shinao* (Camphor oil). For infants with acute/chronic convulsions, form pills the size of red beans. Dissolve one pill in Mint Decoction for the infant to drink, ensuring sleep and immediate relief. Adults with wind-cold may chew one pill with Mint Tea for relief.
Ancient Formula for Infantile Acute/Chronic Convulsions:
One jar of Cinnabar, one jar of Snow; One large beetle, one scorpion.
Grind four ingredients to dust, mix with fresh human blood.
Take an empty sparrows' rice jar, measure one jar of refined fine Cinnabar and one jar of Calomel.
