《圣济经》慈幼篇·稽原疾证章曰∶婴孩气专志一，终日号而嗌不嗄，和之至也。然五脏未定，虽微喜怒嗜
欲之伤，风雨寒暑，饮食居处，易以生患。故外邪袭虚，入为诸风。肥甘之过，积为疳黄。襁褓不慎，则肤腠
受邪而寒热。出处不时，则精神不守而客忤；蕴热而斑毒；积冷而夜啼。皆阴阳之寇甚于刚壮也。况根于中者，
与生俱生。如母惊伤胎，生而癫疾。肾气不成，生而解颅。风热伤胎，生而口噤。风冷伤胎，生而躯啼。纳污
之为血癖也。胎弱之为诸痫也。率由孕育之初，殆非一朝一夕之故。是人善保赤子，治法尤详吐、下、灸、刺、
熨、浴、粉、摩，泛应而机随。若病在胸中，秽汁既吞，必吐而愈。病在肠中，乳哺不进，必下而愈。重 、
重龈，治以微针。暴痫身直，治以灸KT 。熨风池以泄微邪，浴皮肤以散寒热。摩囟以通鼻塞，粉汗以密腠理。
至若重舌之膜断之以爪，邪厉之气禳以祝由。盖稚弱感疾，易于滋蔓。惟恻怛之心者，要在防微杜渐，故无所
不用其至也。彼拘于无治，或欲如田舍儿任其自然，未免为失病之机。过于救治，或欲不问春夏，荡以快剂，
未免有汤液之伤。是皆一偏之蔽，非知治之大体也。
《颅囟经》∶初生小儿鹅口、撮噤，并是出胎客风着颅脐致有，可以小灸三壮及烙愈。
初生小儿至夜啼者，是有瘀血腹痛，夜乘阴而痛则啼。初生小儿，一月内乳痢如胶，是母寒气伤胃所致也。
初生小儿，一月内乳痢如血，是母胸有滞热所作也。初生小儿，一月内两眼赤者，是在胎之时，母吃炙爆热面
壅滞，气入胎中熏儿脑所致也。
小儿温热皆同从气实而搏胃气，然若下之，气平即愈，气虚则生惊而变痫。小儿惊痫。一从虚邪客热相
搏而生其候。当用补养安和即愈，加以性冷及太过即死。小儿哕逆、吐，皆胃虚气逆客于脏气而作，当和胃养
气，止如下冷即极。小儿霍乱吐逆，皆胃气与阴阳气上下交争而作，当用分和补药调养即愈，余皆死。小儿客
忤无辜，皆因客入所触及暴露星月，小儿嫩弱，所以此候多恶。
唐·孙真人《玉关诀》云∶夫小儿之病先辩形证。歌曰∶
摇头揉目，肝热生风。眵泪憎明，三焦积热。
鼻生清涕，肺受其寒。颊赤面黄，风伤腑热。
霍乱吐逆，胃积气伤。泻痢不常，气攻肠滑。
面青呵欠，惊气传肝。盗汗频频，脏伏虚热。
伤寒惊搐，风盛发狂。胃热生斑，气伤冷厥。
长吁啮齿，风盛气生。上窜摇头，涎高胃结。
肺壅气伤，咳嗽咯血。涎盛发 ，积伤风热。
小便淋赤，热聚膀胱。疝气因啼，胎中积结。
奶脾痞癖，因物所伤。喉闭生疮，肺之受热。
爱吃泥土，脾脏生疳。呕吐痰涎，蛔虫上出。
脱肛泻血，冷热积伤。消渴口疮，心家受热。
面黄浮肿，积气所攻。鹤膝解颅，因风腑热。
行迟语涩，胎积气伤。项硬肝风，气伤木舌。
医经要略，病源辩别。审而用之，细详使乐。
杜光庭《指迷赋》∶
阴升阳加，四时无差。万物禀天地一气，男女从精血而邪。三旬而阴气纯浓，子称襁褓；两月而阳气方生，
号曰婴儿。百日之内名曰奶腥；至半 而为 乳。童周期而阴阳各半；孩儿千日而真气方奢。五六岁小儿之誉
定，十岁外童子之名华。若论变蒸之候，法主数家。魂自变而目明，魄自变而气加。百日神化而喜笑，胎中有
滞而显邪。蒸变兮气血自劣，不变兮真本无遮。乳痢兮五日乃有，撮口兮十日以遐。胎惊兮未及蒸变，内
兮百日如拿。胎疸因母气之传热，黄病由脾胃之热瘕。热极则咬人啮齿，风盛则面色如花。内热则浑身似
火，腹痛则恶哭 。于是乎察其形色，辨于邪正。面黄赤而大怕体凉，吐与泄而最嫌热盛。惊痫兮身体宜温，
伤寒兮脉洪邪迸。腹胀唯忧足冷膝沉，咳嗽本喜浮滑为病。消渴须身热脉洪，夜啼为逆；霍乱而脉大体温，心
疼忧命。相顺则施功用药，相反则与言危病。面黄色赤唇青而吐逆堪论，目赤唇红内热而发潮当脉。唇角微赤
而脸黑防惊，指黄足冷而惊热为证。摇头揉目兮肝热多眵，上窜喘粗兮风极涎诤。咳嗽与鼻塞相连，斑疮与伤
寒同类。明之则愚化为贤，悟之则超凡出圣。是故小儿之候，难以参详。寒多则皮肤燥涩，热盛则面赤颊黄。
热气盛则惊痫须发，千足冷则气厥宜防。热连夜而阴少，寒兼昼而阳伤。积热则壮热不歇，伏寒则乍温乍凉。
伤寒热兮鼻冷耳热而喘粗，麻痘热兮两耳角冷而是常。唇黄微青而多睡，乳食欲吐而脾殃。睛急目青而色黔，
惊候涎生而胃亡。但能消息而根据此，何虑远近而不扬。是知急惊因内热而所蕴，慢脾因中气之虚荒。肝伏热而
雀目，秽兼风而脸疮。青盲肝冷，翳膜胆强。盗汗因内虚而起亦虚劳而骨伤，兼惊伏于内痞及冷热而两详。鼻
衄连绵，心盛鼻疡。拘挛而筋脉皆冷，口噤而风击胃强。客忤而物忤无异，卒中而中恶同章。欲识丁奚腹大，
气聚成积。病在腑而异治，积居阴而难详。始因乳哺不消，三焦难整；渐次结聚成块，一身消历。胃寒而珍馔
不餐，脾热而逢粗也吃。疳瘦由中热而胃伤，腹胀是脾衰而气塞。吐本胃虚，泻由寒癖。生恋甜肥而乃生疳虫，
疮起脾元而毒之所适。心脾热而口内生疮，二脏极而重舌相击。明之则万举万全，用之则功名不溺。是故邪生
则病至，积聚则疳居。皮肤涩而头发作穗，面色赤而渴饮难除。肺气热而鼻疮不止，大肠冷而粪如下泔。心疳
则五心烦热，肾疳则面肿骨疏。且脏气主失固，使真气之不疏。三焦不调而形饶丹肿，荣卫不治而频出痈疽。
都缘皮肉之沮滞，又详血气之无余。积成因乳食太早，疳瘦由餐甜不除。于是脏气不宣，乃生诸患。泻有八种，
惊生急慢。胃逆肠寒而吐泻并兴，脾肺俱虚而泻痢无间。表里冷而水沫俱奔，脾胃寒而泻物成瓣。脾热胃寒而
泻水兼黄，脾虚心热而如汤之状。小肠传热而便血，大肠冷极而脓绽。似泻不泻是脾冷心热所蒸，饮水不休盖
气盛金火相诤。证频睹则逆顺方明，病数见而智性方惯。是知膀胱受热尿血而必患五淋，膀胱与小肠积冷偏坠
而气厥渐深。伤风因当风解脱，鼻塞由母气囟侵。疟疾是暑气之不出，脱肛乃肠冷之泻频。衣安月下抬无辜以
为殃，瞳子未成见希物而触忤。风搏血气上攻而结成瘰 ，热壅脏极内冲而丹肿难禁。伤饱则骨软脚弱，失乳
则项细少音。解颅盖骨气之不足，鹤膝由肌肉之不任。既晓病源，须识浮沉。惊热兮下之当愈，泻痢兮调胃
宜深。风盛兮解之即退，伤冷兮和胃温阴。论脉兮尺寸未定，别病兮虎口纹临。次指而分于三等，青红而在于
两寻。穷之则病无不辩，用之则法无不深。要在女寻右手，男看左指。气在下纹，风居中里。过风关名曰命关，
（此亦以气、风、命为次。）定其色青之与紫。先论管气之枝初节为始，若见红紫微微脾气常滞。外有枝青兮风已入脾，
或见紫枝兮邪气难止。枝头分开兮黑色相连，气虚为病兮风气将起。但观于初节之间详吉凶而定其病矣，是知
中主于风亦须详视。青纹紫枝兮慢惊当耳，紫纹青枝兮似虫不出。虚惊气散，涎盛难治。若见青枝冲命，风盛
而搐掣须当；或见枝赤纹红，定知是风来侵气。纹紫枝青，宜防慢惊。及已忽然纹青枝紫，过命关而惊候须来。
枝分两头，如线细而涎潮当死，红纹微散而可医，黪黑忽见而将毁。红如米而肺热，喘泄胃而未已。两米青紫
而入于命关，病候虽在而去如流水。红紫迸甲而命殂，内缠指面而无死。若能在意而精通，假使珠珍而无比。
茅先生小儿受病根源∶
小儿五脏受诸病，听说根源仔细明。眼赤肝家壅毒热，怕明肝与心受惊，肝脾积聚成雀目，嗌气脾家积虚
膨，积热在脾多爱睡，牙疳奶食毒相生。吐虫大肠有余热，脾疳吃土有风生。毒食脾家俱积热，或者疟积又还
荣。大伤积食脾热泻，耳聋肾积热毒并。重舌口疮心极热，奶癣脾积热气行。脾中有积腹高凸，心虚见水忽然
惊。肺中壅热鼻多塞，木舌心肠积共停。囟门肿起肾之死，大肠积热有疮生。脏中有热常合口，有积心胀眼斑
成。五心潮热疳劳盛，疳之余热障虚鸣。脾脏毒盛奶不化，咬奶风兼入骨惊。龟背肺中有积热，龟胸客风伤背
平。 耳元是肾中得，毒因伤肺候分明。遍身虚肿积不尽，走马疳因肾得名。天 脐风因此得，喉中痰上恶风
惊。小便不利缘何事，积气不散办其形。要识惊癫痫痴者，肝之候与及脾惊。或喜或悲人莫测，邪气入脾速
疗轻。肚胀口疮疳会气，逆胸涎隔热交横。夜多盗汗如汤泼，心热不和人不明。多笑是惊余热在，不然心热乱
心惊。泻之日久惊食得，冷热相并特为生。略述因由容易见，免交性命入泉 。
《五关贯真珠囊》小儿受病候∶
多烦则面青。涎盛则目转。实极则吐涎。虚极则霍乱。伤乳则面黄。心热则舌重生疮。撮口则属气啮齿兼
风。声嘶而哭伤于肺。虚惊面青合地卧伤于脾。摸手则脏腑俱虚。
《五关贯真珠囊》小儿三结候∶
上结者多因热及伤寒，麻豆疹子安后方结，病多腮肿，喉塞，咽物不下。中结者多因吐泻不止，被冷气结
于脾脏，令上不能下食，渐次气闷，便变成慢惊风。下结者多因医人错认病源，乱下转药，后不曾调气，令大
肠受风虚，便结不通，肚中虚鸣，才下转药，泻住，又秘结。
《五关贯真珠囊》论∶凡一腊已后，百日已前，怕有两般病起。
一者天 。天能盖人，地能载人。天为父，地为母。盖以天 惊风自天而得之。二者内 。内 者，盖因
咽于乳母败血，乳入腹中，得冷见发，攻触心胸。其状初发便不食乳，口开不利，手足挛搐，忍其疼痛，谓之
内也。
《惠眼观证》论杂病形证源∶惊风了后，又癫又痴，由惊气入心未退。患龟背，主因伤寒，客风伤于背。闭
口不开，主大肠积热气盛。或惊、或喜、或悲，惊气入脾。怕明爱暗，主心与肝家热。患痢焦渴，主大肠干燥。
痢病浑下膏血，主大肠气虚。吐血衄血，主伤寒后气血壅滞，不循故道。阴囊重大为肾气伤冷，流入于中，下
患疝气。头面患疮，阳气聚在头面，乘热而生。遍身壮热，口鼻干燥，头发作穗者，主热气冲四肢。鼻下赤烂
生疮，时咳嗽，主肺脏疳冲于脑。遍身生疮头，眼赤，小便涩滞，主心肺热极。生下多患 耳，主脑内热风兼
毒瓦斯入肾。肚干气痛，主生冷冲胃，或硬物伤脾。小肠急痛，小便黄涩淋沥。一日、两日时时有汗，或一日、
两日非时壮热，乃生骨蒸。
《庄氏家传》小儿病源∶夫人禀天地之精变化为形。父之精为魂，母之血为魄，积父母之精血以成其胎。怀
胎一月如白露，二月似桃花，三月男女分，四月形象足，五月能动手足，至九月而三转身，十月而形满足，子
母分解于中。有延月者少病，月不足者病多。生后六十日瞳人就而能识母，二百一十日筋骨成而能坐，三百日
掌骨成而能匍匐。骨木开，脏腑宽，毛发长，齿牙生。或吐，或泻，或热，或惊，作时发歇。岂辩孩子变蒸
形状，却呼鬼祟，或即灸烧，致心脏发躁，变作惊风。或作惊痫搐搦，凡人呼作风。或变奶痫，泻痢清水，奶
食不消，上热下冷，喘息不调，四肢厥冷，虚汗遍身。若不细观证候，病变多端，不遇妙医，枉死多矣。孩子
未满七岁，有一十五种无辜之疾，后有五疳、八痢、二十四候，须明形证。不辨根源，药应无效。所谓无辜者∶
脐风、撮口、口噤、重舌、木舌、雀舌、乳颊、奶痫、奶霍、龟胸、厥胁、猢狲噤、惊风、天 、赤瘤共十五
种。所谓五疳者∶心脏惊疳，肝脏风疳，肺脏气疳，脾脏食疳，肾脏急疳。
心脏歌∶
浑身皆壮热，肢体不能任。怕冷增重覆，腮红面似金。口干常鼻燥，渐次病根深。只因惊扑得，此病本从
心。
肝脏歌∶
摇头揉口鼻，白膜翳睛瞒。背痛常生涩，双眸闭不观。青黄容儿丑，疮癣体消斑。发立毛焦悴，元因得自肝。
肺脏歌∶
咳嗽多啼哭，疮生口鼻傍。饶声惟啮齿，滑痢瘦 。肢体无筋力，形容不似常。吐脓并吐血，此是肺家
伤。
脾脏歌∶
乳食难消化，惟便在土泥。腹高青脉现，发薄顶毛稀。喘息多饶嗽，无欢只爱啼。痢多酸臭甚，此病本从
脾。
肾脏歌∶
泻痢多频并，寻常只是惊。粪中多米出，皮上粟纹生。身体烘如火，才凉却似水。急疳何以疗，本自肾家
生。
《圣济经》慈幼篇·稽原疾证章说道：婴儿气息专一，意志纯真，整天啼哭而嗓子不哑，这是阴阳调和达到极致的表现。然而，婴儿的五脏功能尚未定型，即使是微小的喜怒情绪或嗜欲损伤，加上风雨寒暑的变化、饮食起居的不当，都容易引发疾病。因此，外邪侵袭虚弱的身体，侵入体内就会变成各种风病；过食肥甘厚味，积滞体内就会形成疳黄病；在襁褓期间护理不慎，肌肤腠理受邪就会出现寒热症状；起居作息没有规律，精神不能安守就会遭遇客忤（外来邪气侵扰）；体内蕴结热毒就会发斑疹毒疮；体内积冷就会导致夜间啼哭。这些都是因为婴幼儿阴阳之气薄弱，比成年人更容易受到侵害。况且，有些病因根植于体内，是与生俱来的。例如母亲受惊伤及胎儿，孩子出生后就会患癫痫；肾气发育不全，出生后就出现解颅（囟门迟闭）；风热伤及胎儿，出生后就口噤（牙关紧闭）；风冷伤及胎儿，出生后就躯干抽搐啼哭；母体摄入污秽之物导致胎儿血癖；胎元虚弱导致各种痫证。这些大多源于孕育之初，绝非一朝一夕的原因。所以，善于保护婴儿的人，治疗方法尤为详尽，包括吐法、下法、灸法、刺法、熨法、浴法、敷粉、按摩等，广泛适用且灵活变通。如果病邪在胸中，秽浊汁液已经吞入，必须通过催吐才能痊愈；如果病邪在肠中，乳食无法进食，必须通过泻下才能痊愈。对于重舌（舌下肿起如再长一小舌）、重龈（牙龈肿胀），用微针治疗；突发癫痫身体强直，用艾灸治疗。熨烫风池穴以疏散轻微邪气，洗浴皮肤以散除寒热。按摩囟门以通鼻塞，敷粉止汗以固密腠理。至于重舌的膜，可以用指甲切断；邪厉之气，可以用祝由术驱除。大概是因为幼儿体质柔弱，患病后容易蔓延滋长。只有怀着怜悯关爱之心，关键在于防微杜渐，所以要在各方面做到极致。那些拘泥于“无治”观点，或者想像乡下孩子那样放任自然的人，未免会错失治疗时机。过于急切地救治，或者不分春夏秋冬，都用猛药快剂，未免会损伤汤药带来的伤害。这些都是偏执的弊端，不是懂得治疗大体原则的人所为。
《颅囟经》说：初生小儿患鹅口疮、撮口、口噤，都是出胎时客风侵袭头颅和脐部导致的，可以用小艾炷灸三壮以及烙法治愈。
初生小儿到了晚上啼哭，是因为有瘀血腹痛，夜晚属阴，阴气盛时疼痛加剧所以啼哭。
初生小儿，一个月内乳汁排泄像胶一样，是母亲寒气伤胃所致。
初生小儿，一个月内乳汁排泄像血一样，是母亲胸部有滞热造成的。
初生小儿，一个月内双眼发红，是在胎内时，母亲吃了炙烤爆炒的热性面食导致气机壅滞，热气进入胎中熏蒸胎儿脑部所致。
小儿温热病大多都是从气实搏结胃气引起的，然而如果使用下法，气机平和就痊愈了，如果气虚就会产生惊风并转变为痫证。小儿惊痫，一种是从虚邪与客热相搏而生其症状。应当使用补养安和的方法就能痊愈，如果加用性质寒凉的药物或者用药太过就会死亡。小儿呃逆、呕吐，都是因为胃虚气逆客居于脏气而发作，应当调和胃气、养护正气，如果只是单纯止住下利就会极其危险。小儿霍乱吐逆，都是因为胃气与阴阳之气上下交争而发作，应当使用调和分补的药物调养就能痊愈，其余情况都会死亡。小儿客忤、无辜病，都是因为外来邪气侵入所触及的事物或暴露于星月之下，小儿嫩弱，所以这种病症多表现为凶恶之象。
唐代孙思邈《玉关诀》说：小儿的疾病首先要辨别形证。歌诀说：
摇头揉眼，是肝热生风。眼屎多流泪怕光，是三焦积热。
鼻流清涕，是肺受寒邪。脸颊红面色黄，是风伤腑热。
霍乱吐逆，是胃积气伤。泻痢无常，是气攻肠滑。
面色青呵欠，是惊气传肝。盗汗频频，是脏伏虚热。
伤寒惊搐，是风盛发狂。胃热生斑，是气伤冷厥。
长吁咬齿，是风盛气生。上窜摇头，是涎高胃结。
肺壅气伤，咳嗽咯血。涎多发热，是积伤风热。
小便淋沥赤痛，是热聚膀胱。疝气因啼哭，是胎中积结。
奶脾痞癖，是因物所伤。喉闭生疮，是肺受热邪。
爱吃泥土，是脾脏生疳。呕吐痰涎，是蛔虫上出。
脱肛泻血，是冷热积伤。消渴口疮，是心家受热。

行走迟缓言语涩，是胎积气伤。项硬肝风，是气伤木舌。

阴升阳加，四季无差错。万物禀受天地一气，男女从精血而产生。三十天阴气纯浓，称为襁褓；两个月阳气方生，称为婴儿。一百天之内称为奶腥；到半岁称为乳。儿童周期一岁阴阳各半；孩子千天真气才盛大。五六岁小儿的声誉确定，十岁外童子的名字华丽。若论变蒸的候象，法则主要依据数家理论。魂随变蒸而目明，魄随变蒸而气加。百日神化而喜笑，胎中有滞而显邪。蒸变则气血自劣，不变则真本无遮。乳痢五天才有，撮口十天以后。胎惊来不及变蒸，内热百日如拿。胎疸因母气传热，黄病由脾胃热瘕。热极则咬人啮齿，风盛则面色如花。内热则浑身似火，腹痛则恶哭。于是观察其形色，辨别邪正。面黄赤而大怕体凉，吐与泄而最嫌热盛。惊痫身体宜温，伤寒脉洪邪迸。腹胀唯忧足冷膝沉，咳嗽本喜浮滑为病。消渴须身热脉洪，夜啼为逆；霍乱脉大体温，心疼忧命。相顺则施功用药，相反则与言危病。面黄色赤唇青而吐逆堪论，目赤唇红内热而发潮当脉。唇角微赤而脸黑防惊，指黄足冷而惊热为证。摇头揉眼肝热多眵，上窜喘粗风极涎诤。咳嗽与鼻塞相连，斑疮与伤寒同类。明之则愚化为贤，悟之则超凡出圣。是故小儿的候象，难以参详。寒多则皮肤燥涩，热盛则面赤颊黄。热气盛则惊痫须发，千足冷则气厥宜防。热连夜而阴少，寒兼昼而阳伤。积热则壮热不歇，伏寒则乍温乍凉。伤寒热则鼻冷耳热而喘粗，麻痘热则两耳角冷而是常。唇黄微青而多睡，乳食欲吐而脾殃。睛急目青而色黔，惊候涎生而胃亡。但能消息而根据此，何虑远近而不扬。是知急惊因内热而所蕴，慢脾因中气之虚荒。肝伏热而雀目，秽兼风而脸疮。青盲肝冷，翳膜胆强。盗汗因内虚而起亦虚劳而骨伤，兼惊伏于内痞及冷热而两详。鼻衄连绵，心盛鼻疡。拘挛而筋脉皆冷，口噤而风击胃强。客忤而物忤无异，卒中而中恶同章。欲识丁奚腹大，气聚成积。病在腑而异治，积居阴而难详。始因乳哺不消，三焦难整；渐次结聚成块，一身消历。胃寒而珍馔不餐，脾热而逢粗也吃。疳瘦由中热而胃伤，腹胀是脾衰而气塞。吐本胃虚，泻由寒癖。生恋甜肥而乃生疳虫，疮起脾元而毒之所适。心脾热而口内生疮，二脏极而重舌相击。明之则万举万全，用之则功名不溺。是故邪生则病至，积聚则疳居。皮肤涩而头发作穗，面色赤而渴饮难除。肺气热而鼻疮不止，大肠冷而粪如下泔。心疳则五心烦热，肾疳则面肿骨疏。且脏气主失固，使真气之不疏。三焦不调而形饶丹肿，荣卫不治而频出痈疽。都缘皮肉之沮滞，又详血气之无余。积成因乳食太早，疳瘦由餐甜不除。于是脏气不宣，乃生诸患。泻有八种，惊生急慢。胃逆肠寒而吐泻并兴，脾肺俱虚而泻痢无间。表里冷而水沫俱奔，脾胃寒而泻物成瓣。脾热胃寒而泻水兼黄，脾虚心热而如汤之状。小肠传热而便血，大肠冷极而脓绽。似泻不泻是脾冷心热所蒸，饮水不休盖气盛金火相诤。证频睹则逆顺方明，病数见而智性方惯。是知膀胱受热尿血而必患五淋，膀胱与小肠积冷偏坠而气厥渐深。伤风因当风解脱，鼻塞由母气囟侵。疟疾是暑气之不出，脱肛乃肠冷之泻频。衣安月下抬无辜以为殃，瞳子未成见希物而触忤。风搏血气上攻而结成瘰 ，热壅脏极内冲而丹肿难禁。伤饱则骨软脚弱，失乳则项细少音。解颅盖骨气之不足，鹤膝由肌肉之不任。既晓病源，须识浮沉。惊热兮下之当愈，泻痢兮调胃宜深。风盛兮解之即退，伤冷兮和胃温阴。论脉兮尺寸未定，别病兮虎口纹临。次指而分于三等，青红而在于两寻。穷之则病无不辩，用之则法无不深。要在女寻右手，男看左指。气在下纹，风居中里。过风关名曰命关，（此亦以气、风、命为次。）定其色青之与紫。先论管气之枝初节为始，若见红紫微微脾气常滞。外有枝青兮风已入脾，或见紫枝兮邪气难止。枝头分开兮黑色相连，气虚为病兮风气将起。但观于初节之间详吉凶而定其病矣，是知中主于风亦须详视。青纹紫枝兮慢惊当耳，紫纹青枝兮似虫不出。虚惊气散，涎盛难治。若见青枝冲命，风盛而搐掣须当；或见枝赤纹红，定知是风来侵气。纹紫枝青，宜防慢惊。及已忽然纹青枝紫，过命关而惊候须来。枝分两头，如线细而涎潮当死，红纹微散而可医，黪黑忽见而将毁。红如米而肺热，喘泄胃而未已。两米青紫而入于命关，病候虽在而去如流水。红紫迸甲而命殂，内缠指面而无死。若能在意而精通，假使珠珍而无比。
茅先生小儿受病根源：
小儿五脏受诸病，听说根源仔细明。眼赤肝家壅毒热，怕明肝与心受惊，肝脾积聚成雀目，嗌气脾家积虚膨，积热在脾多爱睡，牙疳奶食毒相生。吐虫大肠有余热，脾疳吃土有风生。毒食脾家俱积热，或者疟积又还荣。大伤积食脾热泻，耳聋肾积热毒并。重舌口疮心极热，奶癣脾积热气行。脾中有积腹高凸，心虚见水忽然惊。肺中壅热鼻多塞，木舌心肠积共停。囟门肿起肾之死，大肠积热有疮生。脏中有热常合口，有积心胀眼斑成。五心潮热疳劳盛，疳之余热障虚鸣。脾脏毒盛奶不化，咬奶风兼入骨惊。龟背肺中有积热，龟胸客风伤背平。 耳元是肾中得，毒因伤肺候分明。遍身虚肿积不尽，走马疳因肾得名。天 脐风因此得，喉中痰上恶风惊。小便不利缘何事，积气不散办其形。要识惊癫痫痴者，肝之候与及脾惊。或喜或悲人莫测，邪气入脾速疗轻。肚胀口疮疳会气，逆胸涎隔热交横。夜多盗汗如汤泼，心热不和人不明。多笑是惊余热在，不然心热乱心惊。泻之日久惊食得，冷热相并特为生。略述因由容易见，免交性命入泉 。
《五关贯真珠囊》小儿受病候：
多烦则面青。涎盛则目转。实极则吐涎。虚极则霍乱。伤乳则面黄。心热则舌重生疮。撮口则属气啮齿兼风。声嘶而哭伤于肺。虚惊面青合地卧伤于脾。摸手则脏腑俱虚。
《五关贯真珠囊》小儿三结候：
上结者多因热及伤寒，麻豆疹子安后方结，病多腮肿，喉塞，咽物不下。中结者多因吐泻不止，被冷气结于脾脏，令上不能下食，渐次气闷，便变成慢惊风。下结者多因医人错认病源，乱下转药，后不曾调气，令大肠受风虚，便结不通，肚中虚鸣，才下转药，泻住，又秘结。
《五关贯真珠囊》论：凡一腊已后，百日已前，怕有两般病起。
一者天 。天能盖人，地能载人。天为父，地为母。盖以天 惊风自天而得之。二者内 。内 者，盖因咽于乳母败血，乳入腹中，得冷见发，攻触心胸。其状初发便不食乳，口开不利，手足挛搐，忍其疼痛，谓之 内也。
《惠眼观证》论杂病形证源：惊风了后，又癫又痴，由惊气入心未退。患龟背，主因伤寒，客风伤于背。闭口不开，主大肠积热气盛。或惊、或喜、或悲，惊气入脾。怕明爱暗，主心与肝家热。患痢焦渴，主大肠干燥。痢病浑下膏血，主大肠气虚。吐血衄血，主伤寒后气血壅滞，不循故道。阴囊重大为肾气伤冷，流入于中，下患疝气。头面患疮，阳气聚在头面，乘热而生。遍身壮热，口鼻干燥，头发作穗者，主热气冲四肢。鼻下赤烂生疮，时咳嗽，主肺脏疳冲于脑。遍身生疮头，眼赤，小便涩滞，主心肺热极。生下多患 耳，主脑内热风兼毒瓦斯入肾。肚干气痛，主生冷冲胃，或硬物伤脾。小肠急痛，小便黄涩淋沥。一日、两日时时有汗，或一日、两日非时壮热，乃生骨蒸。
《庄氏家传》小儿病源：夫人禀天地之精变化为形。父之精为魂，母之血为魄，积父母之精血以成其胎。怀胎一月如白露，二月似桃花，三月男女分，四月形象足，五月能动手足，至九月而三转身，十月而形满足，子母分解于中。有延月者少病，月不足者病多。生后六十日瞳人就而能识母，二百一十日筋骨成而能坐，三百日掌骨成而能匍匐。骨木开，脏腑宽，毛发长，齿牙生。或吐，或泻，或热，或惊，作时发歇。岂辩孩子变蒸形状，却呼鬼祟，或即灸烧，致心脏发躁，变作惊风。或作惊痫搐搦，凡人呼作风。或变奶痫，泻痢清水，奶食不消，上热下冷，喘息不调，四肢厥冷，虚汗遍身。若不细观证候，病变多端，不遇妙医，枉死多矣。孩子未满七岁，有一十五种无辜之疾，后有五疳、八痢、二十四候，须明形证。不辨根源，药应无效。所谓无辜者：脐风、撮口、口噤、重舌、木舌、雀舌、乳颊、奶痫、奶霍、龟胸、厥胁、猢狲噤、惊风、天 、赤瘤共十五种。所谓五疳者：心脏惊疳，肝脏风疳，肺脏气疳，脾脏食疳，肾脏急疳。
心脏歌：
浑身皆壮热，肢体不能任。怕冷增重覆，腮红面似金。口干常鼻燥，渐次病根深。只因惊扑得，此病本从心。
肝脏歌：
摇头揉口鼻，白膜翳睛瞒。背痛常生涩，双眸闭不观。青黄容儿丑，疮癣体消斑。发立毛焦悴，元因得自肝。
肺脏歌：
咳嗽多啼哭，疮生口鼻傍。饶声惟啮齿，滑痢瘦 。肢体无筋力，形容不似常。吐脓并吐血，此是肺家伤。
Chapter "Investigating the Origin of Diseases" in the "Ciyu" (Nurturing Children) section of *Shengji Jing* states: Infants have concentrated Qi and pure will, crying all day without hoarseness, which is the ultimate state of harmony. However, their five Zang organs are not yet settled; even slight injuries from emotions or desires, as well as wind, rain, cold, heat, improper diet, and living conditions, can easily cause illness. Therefore, when external pathogens invade deficiency, they enter and become various Wind diseases. Excessive consumption of rich and sweet foods leads to accumulation and causes Gan jaundice. Negligence during infancy allows pathogens to affect the skin and pores, causing chills and fever. Irregular daily activities cause the spirit to be unguarded, leading to Ke Wu (intruding evil); internal heat accumulation causes macules and toxins; internal cold accumulation causes night crying. These occur because Yin and Yang in infants are more vulnerable than in robust adults. Moreover, some causes are rooted internally and present from birth. For example, if the mother is startled during pregnancy, the child may be born with Dian Ji (epilepsy). If Kidney Qi is incomplete, the child may be born with Jie Lu (delayed closure of fontanelles). If wind-heat injures the fetus, the child may be born with Kou Jin (lockjaw). If wind-cold injures the fetus, the child may be born with body spasms and crying. Ingestion of filth leads to Xue Pi (blood stasis); fetal weakness leads to various Xian syndromes. These mostly stem from the beginning of conception, not a matter of one day or night. Thus, those who skillfully protect infants use detailed treatment methods including emesis, purgation, moxibustion, acupuncture, hot compress, bathing, powdering, and massage, adapting flexibly to situations. If the pathogen is in the chest and foul fluid has been swallowed, emesis is required for cure. If the pathogen is in the intestines and milk cannot be taken, purgation is required for cure. For Chong She (double tongue) and Chong Yin (swollen gums), treat with fine needles. For sudden epilepsy with straight body, treat with moxibustion. Apply hot compress to Fengchi (GB20) to dissipate minor pathogens; bathe the skin to disperse chills and fever. Massage the fontanelle to relieve nasal congestion; apply powder to stop sweating and secure the pores. For the membrane of double tongue, cut it with fingernails; for fierce evil qi, use Zhuyou (incantation therapy) to exorcise. Generally, because children are weak, diseases spread easily. Only with a compassionate heart, focusing on preventing problems before they arise, can one exhaust all measures. Those who adhere to "no treatment" or let nature take its course like country children miss the timing of disease. Those who over-treat, using strong medicines regardless of seasons, risk damaging the body with decoctions. Both are biased errors, not understanding the general principles of treatment.
*Culun Jing* states: Newborns suffering from E Kou (goose mouth/thrush), Cuo Jin (mouth grasping), and Kou Jin are caused by Ke Feng (intruding wind) affecting the head and umbilicus after birth. They can be cured by applying three small moxa cones and cauterization.
Newborns crying at night have abdominal pain due to blood stasis; crying increases at night due to Yin dominance.
Newborns within one month having milky diarrhea like glue are affected by maternal cold injuring the Stomach.
Newborns within one month having milky diarrhea like blood are affected by maternal chest stagnation-heat.
Newborns within one month with red eyes were exposed to maternal consumption of roasted/fried hot foods causing Qi stagnation, which steamed the fetal brain.
Pediatric warm diseases generally arise from excess Qi clashing with Stomach Qi. If purged, Qi levels and health returns; if Qi is deficient, it generates Jing (startle) and transforms into Xian (epilepsy). Pediatric Jing-Xian arises from the clash of deficient evil and intruding heat. Treatment should use nourishing and harmonizing methods; adding cold-natured drugs or overdosing leads to death. Pediatric hiccups and vomiting arise from Stomach deficiency and rebellious Qi invading Zang Qi; treat by harmonizing Stomach and nourishing Qi. Simply stopping diarrhea is extreme/dangerous. Pediatric Huoluan (cholera-like vomiting/diarrhea) arises from conflict between Stomach Qi and Yin-Yang Qi; treat with harmonizing and tonifying drugs. Other cases result in death. Pediatric Ke Wu and Wugu diseases arise from contact with intruding evils or exposure to stars/moonlight; children are tender and weak, so these present as malignant.
Sun Simiao's *Yuguan Jue* says: First distinguish the form and syndrome of pediatric diseases. The song says:
Shaking head and rubbing eyes indicates Liver Heat generating Wind.
Eye discharge, tearing, and photophobia indicate Sanjiao heat accumulation.
Clear nasal discharge indicates Lung Cold.
Red cheeks and yellow face indicate Wind injuring Fu organs with Heat.
Huoluan vomiting indicates Stomach Qi injury from accumulation.
Irregular diarrhea indicates Qi attacking slippery intestines.
Blue face and yawning indicate Startle Qi transmitting to Liver.
Frequent night sweats indicate hidden Zang deficiency-heat.
Shanghan with startle convulsions indicates excessive Wind causing madness.
Stomach Heat generating macules indicates Qi injury with cold厥 (reversal).
Sighing and grinding teeth indicate excessive Wind generating Qi.
Looking up, shaking head, and high saliva indicate Stomach knotting.
Lung obstruction and Qi injury cause coughing and hemoptysis.
Excess saliva and fever indicate accumulation injuring Wind-Heat.
Scanty red urination indicates Heat gathering in Bladder.
Hernia from crying indicates fetal accumulation.
Milk spleen and masses are caused by injury from substances.
Throat closure and sores indicate Lung receiving Heat.
Eating dirt indicates Spleen generating Gan.
Vomiting phlegm-saliva indicates Ascaris rising.
Rectal prolapse and bloody stool indicate mixed cold-heat accumulation.
Xiaoke (wasting-thirst) and mouth sores indicate Heart receiving Heat.
Yellow face and edema indicate Qi attack from accumulation.
Crane knee and jie lu indicate Wind-Fu Heat.
Delayed walking and slurred speech indicate fetal Qi injury.
Neck stiffness and Liver Wind indicate Qi injury with Mu She (wooden tongue).
Medical classics require distinguishing disease sources. Careful application brings benefit.
Du Guangting's *Zhimi Fu* states:
Yin rises, Yang adds, seasons differ not. All things receive Heaven-Earth Qi; males and females originate from essence and blood. At thirty days, Yin is pure and thick, called Qiangbao; at two months, Yang begins to grow, called Ying'er. Within one hundred days, called Naixing; at half a year, called Ru. Children reach one year, Yin and Yang are half; at one thousand days, True Qi is abundant. Five-six years old reputation is set; ten years old name is glorious. Regarding Bianzheng (transformation/evaporation) signs, laws follow numerical schools. Hun becomes clear-eyed through transformation; Po gains Qi through transformation. At one hundred days, spirit transforms and smiles; fetal stagnation shows evil. Transformation makes Qi-Blood inferior; non-transformation leaves true root unobstructed. Milk diarrhea appears after five days; cuokou appears after ten days. Fetal startle occurs before transformation; internal heat at one hundred days resembles holding. Fetal jaundice comes from maternal heat transmission; Huang disease comes from Spleen-Stomach heat masses. Extreme heat bites people and grinds teeth; excessive Wind makes face like flowers. Internal heat makes body like fire; abdominal pain causes crying. Thus observe color and shape, distinguish evil and righteousness. Yellow-red face fearing coolness, vomiting and diarrhea dislike excessive heat. Jing-Xian body should be warm; Shanghan pulse is surging with evil breakthrough. Abdominal distension fears cold feet and sinking knees; cough prefers floating-slippery pulse. Xiaoke requires body heat and surging pulse; night crying is rebellious. Huoluan with large pulse and warm body, heart pain threatens life. Compatible signs allow medication; incompatible signs warn of critical illness. Yellow-red face, blue lips, vomiting indicate discussion needed; red eyes, red lips, internal heat, tidal fever require pulse diagnosis. Slightly red lip corners and black face prevent startle; yellow fingers and cold feet indicate startle-heat signs. Shaking head and rubbing eyes indicate Liver Heat with eye discharge; looking up and rough breathing indicate extreme Wind with salivary struggle. Cough connects with nasal congestion; macules are similar to Shanghan. Understanding turns fools to sages; realizing transcends ordinary to sagehood. Thus pediatric signs are hard to detail. More cold means dry skin; excessive heat means red face and yellow cheeks. Excessive heat causes Jing-Xian hair growth; cold feet cause Qi reversal prevention. Night heat reduces Yin; day cold damages Yang. Accumulated heat causes persistent fever; hidden cold causes fluctuating temperature. Shanghan heat means cold nose, hot ears, rough breathing; measles/smallpox heat means cold ear angles. Yellow lips, slightly blue, sleepy; milk appetite vomits, Spleen suffers. Urgent eyes, blue color, dark complexion; startle signs, saliva generation, Stomach fails. Following this ensures success. Knowing Ji Jing (acute startle) stems from internal heat accumulation; Man Pi (slow spleen) stems from middle Qi deficiency. Hidden Liver heat causes Que Mu (night blindness); filth plus Wind causes facial sores. Qing Mang (blindness) is Liver cold; Yi Mo (membrane) is Gallbladder strength. Night sweats arise from internal deficiency and consumptive bone injury; startle hidden in internal masses and cold-heat details. Continuous epistaxis indicates Heart excess and nasal sores. Spasms mean cold tendons; lockjaw means Wind striking strong Stomach. Ke Wu is same as object collision; stroke is same as evil poisoning. To recognize Ding Xi (abdominal distension), Qi gathers into accumulation. Disease in Fu treats differently; accumulation in Yin is hard to detail. Initially due to undigested milk, Sanjiao difficult to organize; gradually knots form, body wastes. Stomach cold refuses delicacies; Spleen heat eats coarse food. Gan thinness comes from internal heat injuring Stomach; abdominal distension is Spleen failure blocking Qi. Vomiting is Stomach deficiency; diarrhea is cold mass. Loving sweets/fat generates Gan worms; sores rise from Spleen origin where poison goes. Heart-Spleen heat causes mouth sores; two Zang extremes cause double tongue hitting. Understanding ensures success; using ensures merit. Thus evil generates disease; accumulation generates Gan. Dry skin and tufted hair; red face and unquenchable thirst. Lung Heat causes continuous nasal sores; Large Intestine cold causes watery stool. Heart Gan causes five-palm heat; Kidney Gan causes swollen face and sparse bones. Zang Qi losing control prevents Qi distribution. Sanjiao imbalance causes swelling; Ying-Wei disorder causes frequent abscesses. Skin-muscle stagnation and blood-Qi deficiency. Accumulation from early feeding; Gan thinness from sweets. Zang Qi not spreading causes illnesses. Eight types of diarrhea; startle generates acute/slow. Rebellious Stomach and cold intestines cause vomiting/diarrhea; deficient Spleen-Lung cause uninterrupted diarrhea. Exterior-interior cold causes water foam; Spleen-Stomach cold causes瓣 (petal-like) stool. Spleen heat/Stomach cold causes yellow water; Spleen deficiency/Heart heat causes soup-like stool. Small Intestine heat transmits causing bloody stool; Large Intestine extreme cold causes pus. Resembling diarrhea but not is Spleen cold/Heart heat steaming; drinking endlessly is Qi excess Metal-Fire struggling. Frequent signs clarify reverse/forward; frequent diseases familiarize wisdom. Bladder heat causes hematuria and five lin; Bladder-Small Intestine cold causes hernia and deep Qi reversal. Wind injury from exposure; nasal congestion from maternal Qi/fontanelle invasion. Malaria is summer heat not exiting; rectal prolapse is frequent cold intestinal diarrhea. Sleeping under moon invites Wugu disaster; immature pupils seeing rare objects trigger Ke Wu. Wind clashing Blood-Qi attacks upward forming scrofula; Heat obstructing Zang extremes causes uncontrollable erysipelas. Overeating causes soft bones/weak legs; loss of milk causes thin neck/less sound. Jie Lu is insufficient bone Qi; Crane Knee is muscle inability. Knowing disease source requires knowing floating/sinking. Startle-heat purges to cure; diarrhea/purges regulate Stomach deeply. Excessive Wind resolves to retreat; cold injury harmonizes Stomach/warms Yin. Pulse sizes undecided; differentiate disease by Tiger Mouth lines. Secondary finger divides into three grades; green/red in two searches. Exhausting distinguishes all diseases; using deepens all methods. Girls check right hand; boys check left finger. Qi in lower line; Wind in middle interior. Passing Wind gate is Life gate (order: Qi, Wind, Life). Determine green/purple colors. Discuss initial branch node first; faint red-purple indicates Spleen Qi stagnation. External green branch indicates Wind entering Spleen; purple branch indicates difficult-to-stop evil. Branch tips separating with black connection indicates Qi deficiency and Wind onset. Observing initial node determines auspicious/inauspicious and disease. Middle governs Wind, must observe carefully. Green line/purple branch indicates slow startle; purple line/green branch indicates worm not emerging. Empty startle disperses Qi; excess saliva is hard to treat. Green branch rushing Life indicates excessive Wind convulsions; red branch/red line indicates Wind invading Qi. Purple line/green branch warns of slow startle. Suddenly green line/purple branch passing Life gate indicates startle coming. Branch splitting two ends, thread-thin with saliva tide indicates death; faintly scattered red line is treatable; sudden grey-black indicates destruction. Red like rice indicates Lung Heat; wheezing/diarrhea Stomach not ending. Two rices green-purple entering Life gate, disease present but leaving like flowing water. Red-purple bursting nail indicates death; internal wrapping finger surface indicates no death. If attentive and proficient, pearls are incomparable.
Mr. Mao on Pediatric Disease Origins:
Five Zangs suffer diseases, listen to origins carefully. Red eyes indicate Liver toxin-heat; fear of light indicates Liver-Heart startle. Liver-Spleen accumulation causes night blindness; throat Qi indicates Spleen deficiency bloating. Heat in Spleen causes sleepiness; tooth gan comes from milk-food toxicity. Vomiting worms indicates Large Intestine excess heat; Spleen gan eating dirt generates Wind. Toxic food Spleen heat causes malaria accumulation.
