《颅囟经》小儿∶一、眼青揉痒是肝疳。二、孩子齿焦是骨疳。三、肉色白，鼻中干是肺疳。四、皮干肉裂
是筋疳。五、发焦黄是血疳。六、舌上生疮是心疳。七、孩子爱吃泥土是脾疳。
孩子肌骨肥实，皮肤白，无故烦渴，此自小奶猛冲损肺脏，根据后方，内用甘草人参合饮子。若气急，其
骨胀起，鼻连眼下脸青色，呻吟之声者，此必死矣之兆，不得与药。孩子痢如泔淀者难效，痢如鹅鸭血者，脾
已烂损，不宜与药。孩子凡有诸色疾苦，但眼睑下垂牵，必定死矣。孩子疟者皆难效。或发无时，即口噤咬
牙作声，此必死矣。呼为沥瘠疳疟，亦名为锁肠疳。孩子渴吃乳食，夜啼作声，此即是腹肚痛。孩子无故摇头，
此是脑顶风。孩儿吃乳食皆出，此是脾冷。孩子无故肚大，项细，四肢消瘦，筋脉骨节起。自是小来少乳，嚼
食与吃，早成骨热疳劳。先宜与保童丸吃，续与柴胡鳖甲饮子。（保童丸方，见一切疳门中；柴胡饮子方，
见行迟门中。）孩子头面
胸膊肌浓，臂脑细瘦，行走迟者，是小时抱损。孩子鼻流清涕，或鼻下赤痒，此是脑中鼻中疳极，宜用后方。
青黛散吹鼻，兼敷下赤烂处。（方见疳疾，吹鼻门中。）
以前并诊候孩子疾状，孩子气脉未调，脏腑胞薄，腠理开疏，看脉以时，根据方用药。
《华佗九候论》属阴慢疾四般候∶
慢脾风 慢惊风 脾困 虚积
论此四般疾候，其体一般皆瘦弱，无情绪，眼闭慢，脉气微沉细。孩儿病候，审之为妙，切须仔细无令差。
慢脾风候∶吐泻虚损脾胃而成。其候面色青，唇色黄，口角有沫，多睡不醒，或时手脚似搐，四肢冷，脉
气沉弱。歌曰∶
慢脾之候脉微微，昏昏即睡难辨之。或若摇头并口噤，万中无一可能医。
慢惊风候∶惊痰灌心而成。其候唇红目直，手微动如搐，体微热不语，脉气微沉细。歌曰∶
慢惊风候要唇红，仔细推求速有功。囟肿必知无妙药，灵丹与服也成空。
脾困候∶吐泻日久而成。其候孩儿多睡眼不开，饶转动，身体温和，四肢冷，脉气微沉细。歌曰∶
脾困元因转泻虚，连连只睡不开舒。倘若饲之虽吃乳，虫生口内死非殊。
虚积候∶久积频取不尽而成。（一云∶因虚而伤，积聚脏腑而成。）其候肚热，泻白色，多呀水奶食
不化，四肢冷，脉气
微沉。又云∶腹内热，身体温和。歌曰∶
虚积之患事如何，饮水频频不厌多。若是脐凸并眼肿，丹灵若有不医他。
又歌曰∶
或泻或痢又时时，谷道开张不肯肥。见他饮水多频并，愚者犹将疳渴医。
《华佗九候论》属阳急疾四般候∶
急惊风伤寒天 斑疹
论此四般疾候，其体一般。或毒、或涎生，此患须急医。无令慢易，所伤逡巡，死活顷刻。专心用药，审
其精微。
急惊风候∶惊涎流灌肝、心二脏而成。其候吊上眼，手足拳搐，喉内涎响，浑身掣掷，体热，脉洪大。歌曰∶
急惊之候本因涎，积热肝心两所传。眼赤唇青双手搐，下涎风去是精专。
伤寒候∶冷风伤于腠户而成。其候胃气弱，内聚冷风，其形候，面黄颊赤，鼻流清涕，多啼，壮热，其脉
洪大，不可频与表药。歌曰∶
伤寒患最苦，尤须细审详。若言频汗表，七日见乖张。但将凉药解，解晚必生黄。满口疮难救，都缘纯是
阳。
天 候∶非天 人也，因惊热盛而成。其初得之时，频频呵欠，眼中忽然有泪不落，壮热，不时手足微搐，
脉浮洪实大。歌曰∶
寻常天 病，休道小儿娇。积热心留滞，眸翻涎欲潮。后仰多因取，仍兼唇口焦。愚医犹灸烙，必死在三
朝。
斑疹候∶伤寒毒传胃而成。其候有疹，有麻，有痘，其实一体。时多哭叫，手脉来大，浑身甚热，两耳尖
冷，鼻准冷，饮水多吐，宜发出其疮。大为阴，小为阳。歌曰∶
胃热成斑疹，须知此病由。哭多心壅极，舌黑是堪忧。肿满来双水，红涎谷道流。变成如此候，一见命须
休。
《华佗九候论》∶小儿杂病候歌∶
诸般杂病要须知，不问婴孩女与儿。多睡只应肝是本，心留积热夜惊啼。叫呼冷汗因虫痛，寒热于中积在

频频饮水心脏疳，目青胁硬走连肝。肚高唇白脾中起，嗽甚鸣粗肺里干。好食土泥爱饵酸，肾疳之候细明
看。良医若会通神诀，死候方分有数般。
《仙人水鉴》五疳死候歌∶
耳边赤脉起，舌上黑斑生，（斑点也，心疳之候也。）吐甚眼头认，（眼头生黑点，吐甚肝之死候也。）
黑点遍身全。（点子遍身如黑子，肺
之死候。）齿际如同上，（牙齿也。同上生点也，疳极。）轮干死必观。（耳叶也。肾之死候。）
《仙人水鉴》小儿惊候歌∶
地上生时睡，惊风本在肝。夜啼并面白，肺脏与详观。颜色时时变，心形日日观。喘微脾脏乏，咬齿骨家
看。（梦里咬齿，骨家惊风。）
又歌∶
面白肿虚多吹乳，四肢烦热汗频生。双腮五色时时见，恶叫元来尽属惊。
《仙人水鉴》有积病证歌∶
腹胀时时好覆眠，发黄眼赤吐并宣。（小便似油也。）更兼痢下肠鸣甚，去积消症立得痊。
《仙人水鉴》小儿八般伤寒病证歌∶
鼻塞眸黄鼻涕流，非时肌热汗无休，喘鸣吐水并声噎，此是伤寒证有由。
《仙人水鉴》五疳病证歌∶
肌肤汤热甚，羸力不能禁。面黄并颊赤，怕冷爱重衾。口鼻常干涕，根源渐次深。全因惊扑起，此病本从心。
摇头搐眼鼻，白膜上青睛。齿痒多揉嘴，垂眸嗽视亲。颜貌青碧变，遍体癣涂身。毛发多焦立，此病本从肝。
食物难消化，心中好土泥。肚高青脉见，头发薄离离。喘息多饶嗽，无欢只爱啼，痢多腥臭甚，此病本从脾。
嗽来多喘息，口鼻上生疮。粪里常堆谷，身如粟密装。膨膨腹自胀，日日痢非常。呕血并脓甚，全因肺本伤。
泻痢多频并，寻常多爱惊。上焦炎似火，中膈吐如倾。寒热时时有，疮盛转转盈。急疳难治疗，此病肾家生。
王叔和小儿外证一十五候歌∶
眼上赤脉，下观瞳仁。囟门肿起，兼及作坑。鼻干黑燥，肚大青筋。目多直视，都不转睛。指甲黑色，忽
作鸦声。虚舌退场门，啮齿咬人。鱼口气急，啼不作声。蛔虫既出，必是死形。用药急速，十无一生。
茅先生∶小儿杂病候∶
唇红面赤是伤寒候，脸青唇黑是惊风候，鼻青面白是疟子候，面黄如土是有气来攻候。
茅先生辨小儿五绝歌∶
项袅肾家绝，目直绝于肝。喉牵锯脾绝，舌卷向心言。鼻黑脓绝肺，五绝不须看。
茅先生小儿受诸病死候歌∶
小儿有病实难恻，下药喉中涎粘塞。两眼半开并半闭，灸烙皮肤无血色。汗出如珠头后 ，目无光泽鼻干
黑。冲心气筑连双臂，手足如水脚面直。吐泻无常渴嗽多，混浊脉绝似沉寂。眼坑陷时热转多，腹中干泄眼
浮极。胸肿心高连掣手，挥手摇头命难得。是物拈来不奈何，或将口啮难将息。吃药无效直穿肠，啼哭无泪
邪声极。喉中牵锯吹花沫，出气长嘘休奶食。泻痢清脓日转多，千万归冥休费力。
茅先生小儿又受诸病死候歌∶
小儿死候要消详，背母摇头搐一厢。鼻上流汗肚若袋，（杨大邺云∶兼吐榻。）手拿胸膈必须亡。白睛贯入瞳仁内，
四肢不认痛无常。五干五硬并五软，下气频频亦不祥。囟门肿气难为疗，鱼口邪声立见亡。指甲内有红丝见，
手心红赤似生疮。久患唇红不须治，惊来眼慢不相当。眼青脸赤耳叶黑，手掌无纹齿白亡。鼻头浑赤口角垂，
昏昏只睡口开张。发直邪声干啼叫，归冥休用更思量。
茅先生小儿外有二十四种死候∶
太冲无脉，直视看人，鱼口自动，忽作邪声。（《惠眼观证》注云∶若时复一声者死，若气急作声不死。）满
口粘涎，时时恶叫，
身生黑片，五心凸肿，舌出虚搐，（《惠眼观证》注云∶舌出不动者不死。）或缩生疮，伏热不退，喉中空响，泻出黑血，
面黑狂躁，吐泻不止，两眼半开，惊叫咬人，指甲青黑，走马落齿，囟肿或拓，泻止又泻，丹毒遍身，啼高声
咽，鱼际不匀。
上件前项形候是病犯五七候，十无一生。
《惠眼观证》云∶凡得此候，所去不远，更看脉息如何。阳病得之，其脉反细；阴病得之，其脉反洪，不出一日死。
汉东王先生《家宝》诸杂病证诀∶
慢惊风眼喜张，慢脾风眼喜闭。鱼目定睛夜死，面青唇黑昼亡。啼而不哭是烦，哭而不啼是躁。张口出舌
是风，瞑目似睡不思乳食是虚。摇头项硬三焦壅，摇头项软涎在胃，清涕常出肺受寒。舒手长虚，闻言即悲却是
实。腹痛盈盈是气壅，翻身卧是脾疾。
汉东王先生《家宝》小儿形证歌∶（茅先生死候歌与此大同而少异。此又有注，故双存之。）
小儿形证卒难测，满口顽涎喉中塞。（此是风痰闭其九窍，顽涎方退场门中也。）吐泻无时加咳嗽，
（此为脾胃俱虚而至不能化其谷食，
无时泻也。咳嗽即是胃生风。）身上皮肤无血色。（即是血脉不应也。）汗出如油头 峻，（其汗出者，
即是阴阳相离，荣卫相别。津液为汗，故如油糠，
其头 峻即是囟门崩，是心绝，即死。）目无光彩鼻中黑。（其鼻中黑即是肺绝。鼻是肺之外应，
目无光彩即是肾绝。肾主眼之瞳仁，肾绝即目无光彩也。）浮
胸心凸牵撮口，（凸言垤高也，肺胀脉绝即胸凸。）手足如冰脚面直。（胃绝则手足拘直也）。搐
搦睛斜连唇口，（其筋绝即搐，着其眼睛也。）将
手抱头难可救。（其筋搐上，故抱头。此大恶候矣。）眼傍青色多焦渴，（即是肝绝也。肝主东方
甲乙木，其色青，虚即渴矣。）饮水百杯犹未歇。
（其胃管直，即水不能 也。）脸肿眼浮脉不来，（心绝则脸肿。心主血，故血脉不能 ，故便脸肿
矣。）是物拈身将口呷。（脾胃绝状如鱼口一般，往往如
呷水相似也。）啼哭无泪及邪声，（则是肝绝。肝主于流泪，若绝则无其泪。肺主声。肺欲绝，声只
出而无返，谓之邪声。）喉中牵锯口吹沫。（其喉中响
是九窍被痰闭绝，气出入不能，故如牵锯之声。其沫即是卫气出而不回。）此疾诚难可疗之，免被
时人道医杀。（闻是上件诸多恶候，不可下药，若下
药，愚人不知脏腑病绝，只道医人医杀矣。）
汉东王先生《家宝》小儿诸杂病状∶
小儿惊哭，声沉不响者，是病重，用药难瘥。若声浮者，轻调惊便瘥。（何谓惊哭？正睡忽惊起，啼哭叫
不止，谓之惊哭。）
头皮干枯，筋脉紧急，唇外赤而内白，有疮痍如无津至者，兼惊而难痊。此是热过极矣。（何以难瘥？其皮
肤薄则筋脉急
出，其唇外赤而内白者，则是脾虚欲绝不能 也，久则多死。其生疮痍，则肺虚燥，不能荣于身，故不瘥，
急医防失。）赤眼是肝积热。（其眼是肝之外应。
积者，聚也。其有积，亦眼赤，及其时气归肝，故是积聚亦眼赤也。）怕明，心肝有惊。（其心属南方火，
惊邪所伤心，肝肾，其太阳则相克矣。）牙疳，
奶食之毒。（其牙疳者，即是牙中出血而口臭是也。只因吃食馋了便 奶与，故令口臭。须与凉药及消食
毒矣。）吃食不消是脾积冷（其脾主化，食而久冷，
即食不消。宜暖脾也。）耳聋是肾之积。（其耳属肾。被积气所攻。即不闻人声。宜将转药入羊肾中煨，
服之。）胎癣是肺积风。（其胎癣即是长下体上，皮
起成癣。只因父母或有其肺不和，或者长下不避风冷也，而浴得之，是积滞为之也。）潮热是因惊而得。
（其潮热者，只因惊在胸膈；不入脏腑，故发热不时，宜
与惊药。）喉内如锯，是膈上客风入肺及因惊入大肠而得。（肺者，诸脏之上盖也，亦主气。大肠是肺之腑，
受外风，大肠受惊，故如此，即是夹
惊伤寒耳。）或悲或歌是邪入脾。（其邪气入脾，自然无时作歌而哭，故知邪气入脾脏也。）口疮，肚胀疳气
逆。（其口疮即是气不顺攻心。心主应口舌，上逆则生
疮。气逆则肚胀，宜下疳药乃气药服之。）吐虫，心与大肠热。（其心与大肠俱热则不能安，其虫故不在其大
肠，奔上而蛔口出，久则令入黄瘦耳。）疟疾是脾之积（其
疟疾者，是脾受其宿食在，又被新食来冲，其或常睡而不磨消，则与脾气相冲。脾气为正气，胜则热，为邪气胜
则寒。邪正相冲，故发寒热。又云∶有时行者，则是夏
受其热，气在脾遇凉，被秋风吹着则发，亦是邪正交争，阴阳相交，故发寒热，四季一般也。）开口睡是五
脏毒盛。（其五脏受于毒瓦斯则气节，其口开而不合也。）
多哭是惊风入肺。（肺主于声，故多哭也。）
爱吃泥土是脾生虫。（其脾属土，所生其虫，因甜物及糯米食而生，故使本所好吃泥土也。）夜多盗汗
是虚热疳气不顺。（其气主于肺，肺
《颅囟经》关于小儿疾病的论述：一、眼睛发青且揉搓瘙痒，是肝疳的表现。二、孩子牙齿焦黑，是骨疳的表现。三、肌肉颜色苍白，鼻孔干燥，是肺疳的表现。四、皮肤干枯、肌肉皲裂，是筋疳的表现。五、头发焦黄，是血疳的表现。六、舌头上生疮，是心疳的表现。七、孩子爱吃泥土，是脾疳的表现。
如果孩子肌肉骨骼肥壮结实，皮肤白皙，却无故感到烦躁口渴，这是因为自幼吃奶过猛损伤了肺脏，应根据后方，内服甘草人参合饮子。如果出现气急、骨头胀起、鼻子及眼下脸部呈现青色、发出呻吟声的情况，这是必定死亡的征兆，不可给药治疗。如果孩子痢疾排泄物像米泔水沉淀物那样难以见效，或者痢疾排泄物像鹅鸭血一样，说明脾脏已经腐烂损坏，不宜给药治疗。孩子凡是有各种颜色的疾病痛苦，只要眼睑下垂牵引，必定死亡。孩子患疟疾都难以治愈。如果发作没有定时，就出现牙关紧闭、咬牙发声的情况，这也是必定死亡的征兆。称为沥瘠疳疟，也叫做锁肠疳。
孩子想喝乳食，夜间啼哭发声，这就是肚子痛。孩子无故摇头，这是脑顶风。孩儿吃进去的乳食全部吐出，这是脾冷。孩子无故肚子大、脖子细、四肢消瘦、筋脉骨节突起。这是因为从小母乳不足，过早咀嚼喂食，早成骨热疳劳。先宜服用保童丸，随后服用柴胡鳖甲饮子。（保童丸方，见一切疳门中；柴胡饮子方，见行迟门中。）
孩子头面胸膊肌肉浓密，手臂脑袋细瘦，行走迟缓的，是因为小时候抱持不当受损所致。孩子鼻流清涕，或鼻下赤痒，这是脑中鼻中疳极严重，宜用后方。用青黛散吹鼻，兼敷下部赤烂处。（方见疳疾，吹鼻门中。）
以上都是诊察候望孩子病状的方法。孩子气血脉络未调，脏腑胞膜薄弱，腠理开疏，看脉象要按时令，根据方剂用药。

小儿有病实难恻，下药喉中涎粘塞。两眼半开并半闭，灸烙皮肤无血色。汗出如珠头后[缺字]，目无光泽鼻干黑。冲心气筑连双臂，手足如水脚面直。吐泻无常渴嗽多，混浊脉绝似沉寂。眼坑陷时热转多，腹中干泄眼浮极。胸肿心高连掣手，挥手摇头命难得。是物拈来不奈何，或将口啮难将息。吃药无效直穿肠，啼哭无泪邪声极。喉中牵锯吹花沫，出气长嘘休奶食。泻痢清脓日转多，千万归冥休费力。
茅先生小儿又受诸病死候歌：
小儿死候要消详，背母摇头搐一厢。鼻上流汗肚若袋，（杨大邺云∶兼吐榻。）手拿胸膈必须亡。白睛贯入瞳仁内，四肢不认痛无常。五干五硬并五软，下气频频亦不祥。囟门肿气难为疗，鱼口邪声立见亡。指甲内有红丝见，手心红赤似生疮。久患唇红不须治，惊来眼慢不相当。眼青脸赤耳叶黑，手掌无纹齿白亡。鼻头浑赤口角垂，昏昏只睡口开张。发直邪声干啼叫，归冥休用更思量。
茅先生小儿外有二十四种死候：
太冲无脉，直视看人，鱼口自动，忽作邪声。（《惠眼观证》注云∶若时复一声者死，若气急作声不死。）满口粘涎，时时恶叫，身生黑片，五心凸肿，舌出虚搐，（《惠眼观证》注云∶舌出不动者不死。）或缩生疮，伏热不退，喉中空响，泻出黑血，面黑狂躁，吐泻不止，两眼半开，惊叫咬人，指甲青黑，走马落齿，囟肿或拓，泻止又泻，丹毒遍身，啼高声咽，鱼际不匀。
上件前项形候是病犯五七候，十无一生。
《惠眼观证》云∶凡得此候，所去不远，更看脉息如何。阳病得之，其脉反细；阴病得之，其脉反洪，不出一日死。
汉东王先生《家宝》诸杂病证诀：
慢惊风眼喜张，慢脾风眼喜闭。鱼目定睛夜死，面青唇黑昼亡。啼而不哭是烦，哭而不啼是躁。张口出舌是风，瞑目似睡不思乳食是虚。摇头项硬三焦壅，摇头项软涎在胃，清涕常出肺受寒。舒手长虚，闻言即悲却是实。腹痛盈盈是气壅，翻身卧是脾疾。
汉东王先生《家宝》小儿形证歌：（茅先生死候歌与此大同而少异。此又有注，故双存之。）
小儿形证卒难测，满口顽涎喉中塞。（此是风痰闭其九窍，顽涎方退场门中也。）吐泻无时加咳嗽，（此为脾胃俱虚而至不能化其谷食，无时泻也。咳嗽即是胃生风。）身上皮肤无血色。（即是血脉不应也。）汗出如油头[缺字]峻，（其汗出者，即是阴阳相离，荣卫相别。津液为汗，故如油糠，其头[缺字]峻即是囟门崩，是心绝，即死。）目无光彩鼻中黑。（其鼻中黑即是肺绝。鼻是肺之外应，目无光彩即是肾绝。肾主眼之瞳仁，肾绝即目无光彩也。）浮胸心凸牵撮口，（凸言垤高也，肺胀脉绝即胸凸。）手足如冰脚面直。（胃绝则手足拘直也）。搐搦睛斜连唇口，（其筋绝即搐，着其眼睛也。）将手抱头难可救。（其筋搐上，故抱头。此大恶候矣。）眼傍青色多焦渴，（即是肝绝也。肝主东方甲乙木，其色青，虚即渴矣。）饮水百杯犹未歇。（其胃管直，即水不能[缺字]也。）脸肿眼浮脉不来，（心绝则脸肿。心主血，故血脉不能[缺字]，故便脸肿矣。）是物拈身将口呷。（脾胃绝状如鱼口一般，往往如呷水相似也。）啼哭无泪及邪声，（则是肝绝。肝主于流泪，若绝则无其泪。肺主声。肺欲绝，声只出而无返，谓之邪声。）喉中牵锯口吹沫。（其喉中响是九窍被痰闭绝，气出入不能，故如牵锯之声。其沫即是卫气出而不回。）此疾诚难可疗之，免被时人道医杀。（闻是上件诸多恶候，不可下药，若下药，愚人不知脏腑病绝，只道医人医杀矣。）

小儿惊哭，声沉不响者，是病重，用药难瘥。若声浮者，轻调惊便瘥。（何谓惊哭？正睡忽惊起，啼哭叫不止，谓之惊哭。）
头皮干枯，筋脉紧急，唇外赤而内白，有疮痍如无津至者，兼惊而难痊。此是热过极矣。（何以难瘥？其皮肤薄则筋脉急出，其唇外赤而内白者，则是脾虚欲绝不能[缺字]也，久则多死。其生疮痍，则肺虚燥，不能荣于身，故不瘥，急医防失。）赤眼是肝积热。（其眼是肝之外应。积者，聚也。其有积，亦眼赤，及其时气归肝，故是积聚亦眼赤也。）怕明，心肝有惊。（其心属南方火，惊邪所伤心，肝肾，其太阳则相克矣。）牙疳，奶食之毒。（其牙疳者，即是牙中出血而口臭是也。只因吃食馋了便[缺字]奶与，故令口臭。须与凉药及消食毒矣。）吃食不消是脾积冷（其脾主化，食而久冷，即食不消。宜暖脾也。）耳聋是肾之积。（其耳属肾。被积气所攻。即不闻人声。宜将转药入羊肾中煨，服之。）胎癣是肺积风。（其胎癣即是长下体上，皮起成癣。只因父母或有其肺不和，或者长下不避风冷也，而浴得之，是积滞为之也。）潮热是因惊而得。（其潮热者，只因惊在胸膈；不入脏腑，故发热不时，宜与惊药。）喉内如锯，是膈上客风入肺及因惊入大肠而得。（肺者，诸脏之上盖也，亦主气。大肠是肺之腑，受外风，大肠受惊，故如此，即是夹惊伤寒耳。）或悲或歌是邪入脾。（其邪气入脾，自然无时作歌而哭，故知邪气入脾脏也。）口疮，肚胀疳气逆。（其口疮即是气不顺攻心。心主应口舌，上逆则生疮。气逆则肚胀，宜下疳药乃气药服之。）吐虫，心与大肠热。（其心与大肠俱热则不能安，其虫故不在其大肠，奔上而蛔口出，久则令入黄瘦耳。）疟疾是脾之积（其疟疾者，是脾受其宿食在，又被新食来冲，其或常睡而不磨消，则与脾气相冲。脾气为正气，胜则热，为邪气胜则寒。邪正相冲，故发寒热。又云∶有时行者，则是夏受其热，气在脾遇凉，被秋风吹着则发，亦是邪正交争，阴阳相交，故发寒热，四季一般也。）开口睡是五脏毒盛。（其五脏受于毒瓦斯则气节，其口开而不合也。）

According to the *Luxin Jing* (Cranial Fontanelle Classic) regarding pediatric diseases: 1. Eyes appearing blue and rubbing/itching indicate Liver Gan (Liver malnutrition syndrome). 2. Scorched teeth in children indicate Bone Gan. 3. Pale flesh color and dry nostrils indicate Lung Gan. 4. Dry skin and cracked muscles indicate Sinew Gan. 5. Scorched yellow hair indicates Blood Gan. 6. Sores on the tongue indicate Heart Gan. 7. Children craving to eat dirt indicate Spleen Gan.
If a child has solid muscle and bone, white skin, yet experiences unprovoked irritability and thirst, this is due to damage to the Lung Zang caused by forceful breastfeeding since infancy. According to the following formula, internally administer Gancao Renshen He Yinzi (Licorice and Ginseng Combined Decoction). If there is rapid breathing, protruding bones, bluish discoloration of the nose and lower eyelids, and groaning sounds, this is a sign of certain death; do not administer medicine. If a child's dysentery resembles rice water sediment, it is difficult to be effective; if the dysentery resembles goose or duck blood, the Spleen has already rotted and been damaged, and medicine should not be administered. For any disease causing distress in children, if the eyelids droop and pull, death is certain. Pediatric malaria is generally difficult to cure. If attacks occur without fixed timing, accompanied by locked jaws, grinding teeth, and vocalization, this is also a sign of certain death. This is called Li Ji Gan Malaria (Dripping Emaciation Gan Malaria), also known as Suo Chang Gan (Locking Intestine Gan).
If a child desires breast milk and food, cries at night with sound, this indicates abdominal pain. Unprovoked head shaking in children indicates Brain Top Wind. If a child vomits all ingested milk and food, this indicates Cold Spleen. Unprovoked abdominal distension with a thin neck, emaciated limbs, and protruding sinews, vessels, and joints indicates that insufficient breastfeeding from childhood and early introduction of solid foods have led to early onset of Bone Heat Gan Lao (Bone Heat Consumption Syndrome). Initially, administer Bao Tong Wan (Protect Child Pill), followed by Chaihu Biejia Yinzi (Bupleurum and Turtle Shell Decoction). (The formula for Bao Tong Wan is found in the Section on All Gan Syndromes; the formula for Chaihu Yinzi is found in the Section on Delayed Walking.)
If a child has thick flesh on the head, face, chest, and back, but thin arms and head, and walks slowly, this is due to improper holding during infancy. Clear nasal discharge or redness and itching under the nose indicate severe Nasal and Brain Gan. Use the following formula. Blow Qingdai San (Indigo Naturalis Powder) into the nose and apply to the red, ulcerated area below. (The formula is found in the Section on Gan Diseases, Nose Blowing Section.)
The above are methods for diagnosing and observing pediatric conditions. Children's Qi and pulse are unregulated, their Zang-Fu organs and membranes are thin, and their Couli (pores/skin texture) are open and sparse. Pulse diagnosis must consider seasonal timing, and medication should be prescribed according to formulas.
*Huatuo Jiu Hou Lun* (Huatuo's Nine Observations Treatise) describes four types of Yin Slow Diseases:
Man Pi Feng (Slow Spleen Wind), Man Jing Feng (Slow Convulsion Wind), Pi Kun (Spleen Distress), Xu Ji (Deficiency Accumulation).
Discussing these four syndromes, the constitution is generally emaciated and weak, lacking spirit or emotion, eyes close slowly, and the pulse is faint, sinking, and fine. It is crucial to carefully examine pediatric symptoms to avoid error.
**Man Pi Feng (Slow Spleen Wind) Syndrome:** Caused by vomiting and diarrhea leading to Spleen-Stomach deficiency. Symptoms include bluish complexion, yellow lips, foam at the mouth corners, excessive sleepiness, occasional limb twitching, cold limbs, and a sinking-weak pulse. The song states:
*The pulse of Slow Spleen is faint, drowsy sleep makes it hard to distinguish. Or if there is head shaking and jaw locking, one in ten thousand can be cured.*
**Man Jing Feng (Slow Convulsion Wind) Syndrome:** Caused by phlegm-fluid invading the Heart due to fright. Symptoms include red lips, straight eyes, slight hand movements resembling convulsions, mild body heat with muteness, and a faint-sinking-fine pulse. The song states:
*For Slow Convulsion Wind, look for red lips; careful deduction yields quick results. If the fontanelle is swollen, know there is no miraculous drug; even taking spiritual pills is in vain.*
**Pi Kun (Spleen Distress) Syndrome:** Caused by prolonged vomiting and diarrhea. Symptoms include excessive sleep with eyes closed, restlessness despite movement, warm body, cold limbs, and a faint-sinking-fine pulse. The song states:
*Spleen Distress originates from chronic diarrhea deficiency, continuously sleeping without comfort. Even if fed milk, worms will grow in the mouth, leading to inevitable death.*
**Xu Ji (Deficiency Accumulation) Syndrome:** Caused by long-standing accumulation that is frequently purged but not fully cleared. (Alternatively: Injury due to deficiency, leading to accumulation in the Zang-Fu.) Symptoms include abdominal heat, white diarrhea, frequent drinking of water/milk with indigestion, cold limbs, and a faint-sinking pulse. Another description notes internal abdominal heat and a warm body. The song states:
*How does the affliction of Deficiency Accumulation manifest? Drinking water frequently without satiety. If the umbilicus protrudes and eyes swell, even Dan Ling may not cure him.*
Another song states:
*Either diarrhea or dysentery occurs constantly, the anus remains open and the child fails to gain weight. Seeing them drink water frequently, fools still treat it as Gan-Thirst.*
*Huatuo Jiu Hou Lun* describes four types of Yang Urgent Diseases:
Ji Jing Feng (Urgent Convulsion Wind), Shang Han (Cold Damage), Tian Diao (Heavenly Hook), Ban Zhen (Macules and Papules/Rash).
Discussing these four syndromes, the constitutions are generally similar. Whether due to toxic pathogens or phlegm-fluid, this condition requires urgent medical attention. Do not neglect or delay; injury spreads rapidly, and life or death hangs in the balance. Apply medication with focus, examining the subtleties.
**Ji Jing Feng (Urgent Convulsion Wind) Syndrome:** Caused by phlegm-fluid flooding the Liver and Heart due to fright. Symptoms include upward-gazing eyes (Diao Yan), fist-clenched convulsions of hands and feet, rattling phlegm in the throat, whole-body jerking, body heat, and a surging-large pulse. The song states:
*The syndrome of Urgent Convulsion originates from phlegm, accumulated heat transmits to both Liver and Heart. Red eyes, blue lips, and bilateral hand convulsions; removing phlegm and dispelling wind is the essence.*
**Shang Han (Cold Damage) Syndrome:** Caused by cold wind injuring the Couli (skin pores). Symptoms include weak Stomach Qi, internal accumulation of cold wind, yellow face with red cheeks, clear nasal discharge, frequent crying, high fever, and a surging-large pulse. Do not frequently administer exterior-releasing medicines. The song states:
*Cold Damage is most bitter, requiring careful examination. If one speaks of frequent sweating to release the exterior, by the seventh day, contradictions appear. Only use cooling medicines to resolve; if resolution is late, jaundice will arise. Mouth sores are hard to save, all because it is purely Yang.*
**Tian Diao (Heavenly Hook) Syndrome:** Not caused by spirits, but by intense heat from fright. Initially, there is frequent yawning, sudden tears in the eyes that do not fall, high fever, occasional slight limb convulsions, and a floating-surge-solid-large pulse. The song states:
*Ordinary Heavenly Hook disease, do not say the child is delicate. Accumulated heat lingers, eyes roll and phlegm surges. Backward arching often results from treatment, accompanied by scorched lips and mouth. Foolish doctors still use moxibustion/cautery, death will come within three days.*
**Ban Zhen (Rash) Syndrome:** Caused by Cold Damage toxins transmitting to the Stomach. Symptoms include macules, measles, and smallpox, which are essentially one entity. Frequent crying/shouting, large hand pulse, intense whole-body heat, cold ear tips, cold nasal tip, frequent vomiting after drinking water. It is appropriate to induce the eruption of sores. Large lesions indicate Yin, small lesions indicate Yang. The song states:
*Stomach heat creates macules and rashes, know the origin of this disease. Excessive crying indicates extreme Heart congestion, black tongue is worrisome. Swelling and double edema, red phlegm flows from the anus. Once it transforms into such a syndrome, seeing it means life must end.*
*Huatuo Jiu Hou Lun*: Song of Pediatric Miscellaneous Disease Syndromes:
Know all miscellaneous diseases well, regardless of infant gender. Excessive sleep stems from the Liver; Heart retention of accumulated heat causes night frights and crying. Crying with cold sweat is due to worm pain; chills and fever accumulate in the Spleen. Red cheeks and mouth sores indicate Lung-Heart congestion; deficient wind causes convulsions and drooping limbs. Cold slipperiness injures the Spleen, causing diarrhea/dysentery with pus or blood at times. Excess accumulation leads to large abdomen and watery stools; asthma and yellow cheeks require no doctor. Eye swelling, spitting phlegm, and urgent head shaking; do not let the pulse become slow and faint. Patients seek gods and ghosts, refusing good medicine and seeking masters bitterly.
*Xian Ren Shui Jian* (Immortal Water Mirror): Song of Five Gan Disease Syndromes:
Frequent drinking indicates Heart Gan; blue eyes and hard ribs connect to Liver. High abdomen and white lips arise from Spleen; severe coughing and coarse breathing indicate dry Lung. Craving dirt and sour foods; carefully observe Kidney Gan signs. If a good physician knows the divine secret, he can distinguish several fatal syndromes.
*Xian Ren Shui Jian*: Song of Five Gan Fatal Syndromes:
Red veins rise behind the ears, black spots appear on the tongue (spots indicate Heart Gan signs). Severe vomiting reveals eye corners (black spots at eye corners with severe vomiting indicate fatal Liver signs). Black spots cover the whole body (spots like moles over the body indicate fatal Lung signs). Similar spots on teeth (teeth showing spots indicate extreme Gan). Dry earlobes indicate death (earlobes indicate fatal Kidney signs).
*Xian Ren Shui Jian*: Song of Pediatric Fright Syndromes:
Sleeping upon birth, Convulsion Wind originates in the Liver. Night crying and pale face, observe the Lung Zang in detail. Colors change constantly, observe the Heart shape daily. Mild asthma indicates Spleen deficiency, grinding teeth indicates Bone family fright. (Grinding teeth in dreams indicates Bone family Convulsion Wind.)
Another song:
Pale face, swelling, deficiency, frequent blowing of milk; limbs feel hot with frequent sweating. Five colors seen on both cheeks constantly; evil screaming originally belongs entirely to fright.
*Xian Ren Shui Jian*: Song of Accumulation Disease Syndromes:
Abdominal distension, often preferring to sleep on the back; yellowing, red eyes, vomiting and urination. (Urine resembles oil.) Plus severe intestinal rumbling with diarrhea; removing accumulation and eliminating stagnation brings immediate recovery.
*Xian Ren Shui Jian*: Song of Eight Types of Cold Damage in Children:
Nasal congestion, yellow eyes, flowing snot; irregular body heat with ceaseless sweating; asthma, wheezing, spitting water, and choking sounds; this is Cold Damage with a cause.
*Xian Ren Shui Jian*: Song of Five Gan Disease Syndromes:
Skin feels like boiling water, weakness cannot withstand it. Yellow face and red cheeks, fear of cold, loving heavy quilts. Dry mouth and nose with snot, roots gradually deepen. Entirely caused by fright/falling, this disease originates from the Heart.
Head shaking, eye/nose twitching, white membrane over blue pupils. Itchy teeth, rubbing mouth, drooping eyes, coughing while looking at relatives. Greenish-blue facial changes, body covered in ringworm-like patches. Scorched standing hair, this disease originates from the Liver.
Food hard to digest, craving dirt in heart. High abdomen with visible blue veins, thin scattered hair. Asthma with frequent coughing, no joy only loves crying, foul-smelling dysentery, this disease originates from the Spleen.
Cough comes with asthma, sores on mouth/nose. Grain piles in stool, body covered densely like millet. Bloated self-distending abdomen, daily unusual dysentery. Vomiting blood and pus severely, entirely due to original Lung injury.
Frequent diarrhea/dysentery, usually loves being frightened. Upper Jiao inflammation like fire, middle diaphragm vomiting like pouring. Chills and fever occur constantly, sores increase progressively. Urgent Gan is hard to treat, this disease originates from the Kidney.
Wang Shuhe's Song of Fifteen Exterior Signs in Children:
Red veins on eyes, observe pupils below. Fontanelle swells, forming pits. Dry black/brown nose, large abdomen with blue veins. Eyes stare straight, never turning. Black nails, suddenly making crow sounds. Flaccid tongue retreats to anus, biting teeth and people. Fish-mouth gasping, crying without sound. Ascaris expelled, surely a dead form. Medication applied urgently, ten out of ten die.
Mao Xiansheng: Pediatric Miscellaneous Disease Syndromes:
Red lips and face indicate Cold Damage; blue face and black lips indicate Convulsion Wind; blue nose and white face indicate Malaria; yellow-as-earth face indicates Qi attack.
Mao Xiansheng's Differentiation of Five Extinctions in Children:
Neck limp indicates Kidney extinction; eyes straight indicate Liver extinction; throat pulling like a saw indicates Spleen extinction; tongue curled toward heart indicates Heart extinction; black nose with pus indicates Lung extinction; do not need to look further for the Five Extinctions.
Mao Xiansheng's Song of Fatal Signs in Children Receiving Various Diseases:
Pediatric illness is truly pitiable, medicine down the throat blocked by sticky phlegm. Eyes half-open and half-closed, moxibustion/cautery leaves skin without blood color. Sweat like pearls on the back of the head [missing character], eyes lack luster, nose dry and black. Qi rushing to heart beats connecting both arms, hands/feet like water, sole of foot straight. Unpredictable vomiting/diarrhea, frequent thirst/coughing, turbid pulse extinct like silence. When eye pits sink, heat increases; abdominal dry diarrhea, eye float extremely. Chest swelling, heart protrusion pulling mouth corner, waving hands, shaking head, life is hard to keep. Cannot help when picking up objects, or biting mouth, hard to soothe. Medicine ineffective, intestines perforate directly, crying without tears, evil voice extreme. Throat pulling like saw, blowing flower foam, long sighing breath, stop milk/food. Clear pus diarrhea increases daily, millions return to underworld, stop wasting effort.
Mao Xiansheng's Another Song of Fatal Signs in Children:
Fatal signs in children must be detailed: shaking head away from mother, convulsing to one side. Sweat on nose, belly like bag (Yang Daye says: plus vomiting/tucking). Hands grasping chest/diaphragm, must perish. White sclera penetrating pupil, limbs do not recognize pain, pain is irregular. Five Dries, Five Hards, and Five Softs, frequent downward Qi is also inauspicious. Fontanelle swelling with Qi is hard to treat, fish-mouth evil voice leads to immediate death. Red threads visible inside nails, palm red like sores. Long-term red lips need no treatment, fright with slow eyes is inappropriate. Blue eyes, red face, black earlobes, palm without lines, white teeth indicate death. Red nose tip, drooping mouth corners, drowsy sleep with open mouth. Straight hair, evil voice, dry crying, returning to underworld needs no further thought.
Mao Xiansheng's Twenty-Four External Fatal Signs in Children:
Tai Chong pulse absent, staring straight at people, fish-mouth moving automatically, suddenly making evil sounds. (*Huiyan Guanzheng* note: If occurring intermittently once, death; if rapid breathing with sound, not death.) Full mouth sticky phlegm, frequent evil screaming, black patches on body, five centers convex/swollen, tongue protruding with false convulsion, (*Huiyan Guanzheng* note: Tongue protruding immobile is not death.) Or retracting with sores, latent heat not retreating, hollow sound in throat, black blood in diarrhea, black face with mania, continuous vomiting/diarrhea, eyes half-open, fright shouting biting people, nails blue-black, running horse falling teeth, fontanelle swollen or depressed, diarrhea stops then resumes, erysipema covering body, crying with high voice and hoarseness, fish-interstice uneven.
The aforementioned forms and syndromes, if five or seven signs are violated, ten out of ten die.
*Huiyan Guanzheng* (Benefit Eye Observation of Evidence) states: Whoever obtains these signs is near death; further observe the pulse. If Yang disease obtains this, the pulse reversely becomes fine; if Yin disease obtains this, the pulse reversely becomes surge; death occurs within one day.
Han Dong Wang Xiansheng *Jia Bao* (Family Treasure) Verses on Miscellaneous Disease Syndromes:
Slow Convulsion Wind eyes prefer opening; Slow Spleen Wind eyes prefer closing. Fish-eye fixed gaze dies at night; blue face black lips perish by day. Crying without wailing is vexation; wailing without crying is agitation. Opening mouth protruding tongue is Wind; closing eyes seeming to sleep, not desiring milk/food is Deficiency. Head shaking with stiff neck indicates Three-Jiao congestion; head shaking with soft neck indicates phlegm in Stomach; constant clear snot indicates Lung cold. Stretching hands long indicates Deficiency; hearing words and becoming sad indicates Excess. Abdominal pain full indicates Qi congestion; turning over to lie indicates Spleen disease.
Han Dong Wang Xiansheng *Jia Bao* Song of Pediatric Form/Syndromes: (Mao Xiansheng's Fatal Signs Song is largely identical but slightly different. This has annotations, so both are preserved.)

Fluids become sweat, thus like oil bran. Head [missing character] steep indicates fontanelle collapse, Heart extinction, i.e., death.) Eyes lack luster, nose black inside. (Nose black inside indicates Lung extinction. Nose is Lung external manifestation; eyes lacking luster indicates Kidney extinction. Kidney governs eye pupils; Kidney extinction means eyes lack luster.) Accumulation means gathering. Having accumulation, eyes also red; when seasonal Qi returns to Liver, thus accumulation also causes red eyes.) Fear of brightness, Heart-Liver have fright. (Heart belongs to South Fire, fright pathogen harms Heart, Liver-Kidney, Tai Yang then counter-controls.) Tooth Gan, poison of milk/food. (Tooth Gan is bleeding from teeth with bad odor. Just because eating greedily then [missing character] giving milk, thus causing bad odor. Must give cooling medicine and eliminate food poison.) Eating food not digesting is Spleen accumulated cold. (Spleen governs transformation; food long cold, thus food not digests. Warm Spleen is suitable.) Deafness is Kidney accumulation. (Ear belongs to Kidney. Attacked by accumulated Qi. Thus cannot hear human voice. Suitable to put transforming medicine into sheep kidney to roast, take orally.) Fetal ringworm is Lung accumulated wind. (Fetal ringworm grows on lower body, skin rises into ringworm. Just because parents have Lung disharmony, or raising child avoids wind/cold, bathing gets it, is accumulation doing it.) Tidal fever is obtained due to fright. (Tidal fever just because fright in chest/diaphragm; not entering Zang-Fu, thus fever irregular, suitable to give fright medicine.) Throat inside like saw, is guest wind from Diaphragm entering Lung and due to fright entering Large Intestine. (Lung is cover of all Zang, also governs Qi. Large Intestine is Lung Fu, receives external wind, Large Intestine receives fright, thus so, this is夾惊 Cold Damage.) Either sadness or singing is pathogen entering Spleen. (Pathogen enters Spleen, naturally sings and cries without time, thus know pathogen enters Spleen Zang.) Mouth sores, abdominal distension Gan Qi reversal. (Mouth sores is Qi not smooth attacking Heart. Heart responds to mouth/tongue, upward reversal causes sores. Qi reversal causes abdominal distension, suitable to give downward Gan medicine then Qi medicine take.) Spitting worms, Heart and Large Intestine heat. (Heart and Large Intestine both heat thus cannot settle, worms thus not in Large Intestine, rush up and roundworm exit mouth, long term causes yellow emaciation.) Malaria is Spleen accumulation. (Malaria is Spleen receiving old food remaining, again new food rushes in, either often sleep not grinding/digesting, thus conflicts with Spleen Qi. Spleen Qi is Zheng Qi, victory then heat, Xie Qi victory then cold. Xie-Zheng conflict, thus chills/fever arise. Also say: Some travelers, then Summer receive heat, Qi in Spleen meets cool, blown by Autumn wind then arises, also Xie-Zheng struggle, Yin-Yang intersection, thus chills/fever arise, four seasons generally same.)
