The Classics state: "Body pain with slightly yellow complexion, yellow dental plaque, and yellow nails indicate Jaundice." It also states: "Yellow urine, preference for lying down, indicates Jaundice. Feeling hungry after eating indicates Gastric Ju." Regarding the syndrome of Jaundice, ancient physicians mostly attributed it to Damp-Heat and divided it into five types of Jaundice, which is insufficient to cover all aspects. I believe the essence of Jaundice lies in only two categories: Yang Jaundice and Yin Jaundice. Cold, Heat, Deficiency, and Excess are all encompassed within these two, leaving no other meanings.
First, Yang Jaundice Syndrome arises when Dampness transforms into Heat, and Heat generates Yellow. This is what is called the Damp-Heat Syndrome. Symptoms must include fever, irritability, and thirst, or restlessness and inability to calm down, or rapid digestion with easy hunger, or hot and painful urination, or constipation. The pulse is excess and forceful. This syndrome can be caused by external contraction of Wind-Damp or internal injury from food and drink. Provided the child's Yuan Qi (Original Qi) is still strong, the Spleen and Stomach are not damaged, and Damp-Heat is indeed abundant, direct treatment should clear Fire Evil and promote urination. Yichen Wuling San (Artemisiae Scopariae Five-Ingredient Powder) is most stable; Weiling Tang (Stomach and Five-Ingredient Decoction) added with Yichen (Herba Artemisiae Scopariae) is also good. If there is severe constipation and heat, use Jicheng Hangxie Dan.
Second, Yin Jaundice Syndrome is entirely not Damp-Heat, but results from the failure of Qi and Blood. Since Qi fails to generate Blood, Blood fails; since Blood fails to nourish the complexion, the color fails. Any case of yellowing without any Yang symptoms or Yang pulses is Yin Jaundice. This may occur after a major illness or due to long-term Spleen-Stomach deficiency, causing the color of Spleen Earth to manifest externally. Symptoms include preferring quietness and disliking movement, preferring darkness and fearing light, mental fatigue, weak speech, aversion to cold, reduced appetite, weakness in the four limbs, loose stools, or urine resembling gruel. These are all signs of Yang Deficiency, contrary to those with Damp-Heat jaundice. Unless Yuan Qi is rapidly rescued and the Spleen and Stomach are greatly tonified, recovery is impossible. This syndrome is very common, yet ignorant practitioners fail to observe it, claiming that Jaundice is always Damp-Heat and using Yichen (Herba Artemisiae Scopariae) and Zhizi (Fructus Gardeniae) to clear fire and promote urination, leading inevitably to death upon taking the medicine. Suitable formulas include Sijunzi Tang (Four Gentlemen Decoction), Wu Jun Jian (Five Gentlemen Decoction), and Wenwei Yin (Warming the Stomach Drink).
Whenever children have inherently weak Spleen and Stomach and often have accumulations, their complexion is often yellowish-white. Do not simply reduce accumulation; instead, administer much Jicheng Feier Wan (Integrate and Enrich Child Pill). This strengthens the Spleen and Stomach, allowing food to digest naturally, nourishing the Zang-Fu organs, and circulating Ying and Wei (Nutritive and Defensive) Qi. Naturally, the five colors will become bright, and where would Jaundice come from?
Regarding pediatric jaundice, ignorant people generally call it Damp-Heat, employing only methods to drain dampness, promote urination, clear heat, and resolve jaundice, with no other methods known. They do not realize that few cases are caused by Damp-Heat, while many are caused by Spleen Deficiency. When Spleen Earth is strong, it can defend against Damp-Heat, preventing jaundice. Only when Spleen Deficiency fails to transport does Damp-Heat take advantage. Treatment involves temporarily removing Damp-Heat; Yichen Wuling San is good. Once the jaundice recedes slightly, immediately strengthen the Spleen. Do not repeatedly use draining drugs under the guise of "treating the disease with specific medicines."
If the face and eyes are both yellow with a puffy appearance, lips are white, tongue is pale, mouth is not thirsty, body is not hot, there is no nighttime irritability/heat, and urination is not obstructed, do not diagnose as Damp-Heat and promote urination. Instead, rapidly rescue the Spleen and Stomach. Sijunzi Tang (Four Gentlemen Decoction) or Liujunzi Tang (Six Gentlemen Decoction) are suitable.
For those who have taken draining/diuretic medicines, if jaundice persists for a long time, resulting in bland taste, palpitations, weakness in the four limbs, aversion to cold with fever, and turbid urine, this indicates extreme deficiency. Use Sijunzi Decoction to deliver Bawei Dihuang Wan (Eight-Ingredient Rehmannia Pill). Do not continue draining, lest it lead to Spleen failure and Kidney exhaustion, resulting in death.
