Whenever a child develops fever due to food stagnation in the Middle Jiao (Middle Burner, referring to the Spleen and Stomach) that fails to digest, there will inevitably be an aversion to food. Some may experience belching with a sour taste, some may dislike the smell of food, some may feel the urge to vomit but cannot, some may vomit incompletely, some may feel nauseous, some may suffer from shortness of breath and epigastric fullness and oppression, some may have pain in the stomach region, and some may have fullness and distension below the heart that is painful upon pressure. These are all signs of food retention. If caused by cold pathogenic factors, the pulse at Renying (corresponding to the Left Guan position) on the left hand and the pulse at Cunkou (corresponding to the Right Guan position) on the right hand will both appear large. External symptoms include headache, aversion to cold, and physical stiffness; internal symptoms include epigastric fullness, vomiting, retching, or pain. In such cases, Huoxiang Zhengqi San (Agastache Powder for Rectifying the Qi) or Renshen Yangwei Tang (Ginseng Decoction for Nourishing the Stomach), or herbs such as Muxiang (Aucklandiae Radix) and Sharen (Amomi Fructus), should be used. If meat digestion is impaired, powder of Tangzi (i.e., Crataegi Fructus, Hawthorn) must be added; if flour-based food digestion is impaired, Shenqu (Medicated Leaven) and Maiya (Hordei Fructus Germinatus, Barley Sprout) should be added; if raw/cold foods and meat digestion are impaired, Caoguo (Tsaoko Fructus), Sharen, Zhishi (Aurantii Fructus Immaturus), and Qingpi (Citri Reticulatae Pericarpium Viride) should be added as the main treatment. If food remains above the stomach entrance and has not yet entered the stomach, emesis (vomiting) can be induced; if emesis is not suitable, digestive promotion methods should be used; once the food enters the stomach and transforms into waste, and external symptoms have resolved, purgation (downward draining) can be employed to expel the retained food, using Sanhuang Zhishi Wan (Three-Yellow Zhi Shi Pill). If heat predominates, Da Chaihu Tang (Major Bupleurum Decoction) is used; if there is no external contraction and it is merely food injury, purgation may be applied. When treating Shanghan (Cold Damage) complicated by food retention, one must not first attack the food; rather, one must first disperse the cold pathogen, and only then proceed with digestive promotion.
