Infants and children who consume milk and food without moderation, or overeat raw, cold, hard, and solid substances, may cause the spleen and stomach to fail in digestion and transformation. This leads to food stagnation in the Zhongwan (CV12) region. When symptoms such as high fever, cold feet, abdominal distension, lethargy, and loss of appetite appear, one should employ methods to purge the stagnation. The tuina techniques should include dividing yin and yang, moving the eight trigrams, moving the five meridians, pinching the transverse crease at the base of the fingers (Xiao Hengwen), rubbing HT Men (likely referring to Banmen/ST10 or a specific local point, retained as per text), pushing the Large Intestine meridian, pushing San Guan (Three Passes), retreating Liu Fu (Six Bowels), pushing from Tianmen (Yintang to Hairline) towards Tiger's Mouth (He Gu) up to the elbow, heavily tonifying Spleen Earth, and rubbing Zhongwan (CV12). If accompanied by fever, add scooping Mingyue (Lao Gong area) and rubbing the navel and Guiwei (GV8). For abdominal pain, pinch Yi Wo Feng (LI5 area) and rub Zhongwan. For abdominal distension, add pressing the strings and rubbing motion (An Xian Zuo Cuo Mo). For undigested food, rub Wai Lao Gong.
There are three types of abdominal pain in infants: cold pain, heat pain, or food stagnation pain. Pain above the umbilicus is heat pain; pain at the umbilicus is food stagnation pain; pain below the umbilicus is cold pain. Since infants cannot speak, one must observe their complexion. Heat pain presents with a red face, abdominal distension, and intermittent pain, being most common during the summer months. The treatment method involves dividing yin and yang (with heavier yin techniques and lighter yang techniques), moving the eight trigrams, moving the five meridians, pushing San Guan (lightly), retreating Liu Fu (heavily), rubbing Yi Wo Feng, pushing from Da Ling (PC7) upwards to Wai Lao Gong, then tonifying Spleen Earth, and pushing from Tiger's Mouth to the elbow. If caused by food injury, the complexion remains normal, but the chest and heart area appear elevated, and the abdomen is tender to touch due to intestinal obstruction. This is often caused by damage from consuming raw, cold, and hard foods, leading to qi stagnation. The treatment involves dividing yin and yang, moving the eight trigrams, moving the five meridians, laterally pushing Tiger's Mouth, tonifying Spleen Earth, rubbing Yi Wo Feng, rubbing Zhongwan, rubbing HT Men, pushing from Tianmen to Tiger's Mouth up to the elbow, rubbing the navel and Guiwei, pushing from Da Ling to Wai Lao Gong, and finally performing "moving earth into water" (Yun Tu Ru Shui). If it is cold pain, the face appears cyan, the belly makes sounds, the lips are white, and the pain intensity does not fluctuate. The treatment involves dividing yin and yang (with heavier yang techniques and lighter yin techniques), moving the eight trigrams, moving the five meridians, pinching Yi Wo Feng, pressing the strings and rubbing motion, pushing San Guan, pushing Du Jiao (abdominal corner point), rubbing the navel, pushing Spleen Earth, pushing from Tianmen to Tiger's Mouth, rubbing the elbow, pushing from Da Ling to Wai Lao Gong with draining technique, and then tonifying Spleen Earth. If cold qi attacks the heart causing pain, symptoms include cold hands and feet, cold sweat all over the body, and in severe cases, cyanotic and black fingernails and toenails, with a deep, fine, and weak pulse. The treatment involves dividing yin and yang, moving the eight trigrams, pushing San Guan, tonifying Kidney Water, rubbing Er Feng Men (Two Gate Doors), and Huang Feng Ru Dong (Yellow Bee Enters the Cave).
