The term "failure to suckle" refers to the condition where an infant, immediately after birth, is unable to suckle.
When an infant first emerges from the womb and has not yet cried, one should promptly wipe their mouth with a hand to ensure that lochia (postpartum discharge of blood and mucus) is completely cleared and does not get swallowed. If managed properly without lochia entering the stomach, no other illnesses will arise. However, if the mouth is not wiped thoroughly, allowing filth to enter the abdomen, it may cause abdominal distension and shortness of breath, leading to an inability to suckle. Alternatively, if the mother exposed herself to excessive cold during childbirth, causing cold to invade the fetus, resulting in infant abdominal pain, these conditions are suitable for treatment with Fuling Wan (Poria Pill) and Muxiang Binglang San (Costus and Areca Nut Powder).
Fuling Wan (Poria Pill)
Ingredients: Chi Fuling (Red Poria), Chuan Huanglian (Coptis Rhizome, whiskers removed), Zhiqiao (Bitter Orange peel, stir-fried).
Equal parts of the above ingredients. Grind into fine powder, mix with refined honey to form pills the size of sophora seeds. Administer one pill per dose, mixed with breast milk. This formula treats vomiting and failure to suckle caused by the ingestion of filth.
Muxiang Binglang San (Costus and Areca Nut Powder)
Ingredients: Muxiang (Costus Root), Sheng Ganjiang (Raw Dried Ginger), Fuling (Poria), Gancao (Licorice Root), Suan Mugua (Chaenomeles Fruit), Dingxiang (Cloves) in equal parts.
Grind the above ingredients into a coarse powder. Take a small pinch, decoct in water, dip cotton wool into the liquid, and drip it into the infant's mouth.
Formula from *Waitai Miyao* (Essential Prescriptions from the Imperial Library)
Ingredients: Breast milk 2 he, scallion white 1 cun.
Boil together for one or two boils. Remove the scallion white and administer the milk to restore suckling.
Renshen Wan (Ginseng Pill) from *Shenghui Fang*
Ingredients: Renshen (Ginseng Root), Longdan (Gentian Root), Huanglian (Coptis Rhizome), Mayaxiao (Saltpeter), Zhigancao (Honey-fried Licorice Root, roasted until slightly red), Zhishi (Immature Bitter Orange, bran-fried until slightly yellow) each 2 liang.
Crush and sieve into fine powder, mix with refined honey to form pills the size of sophora seeds. Crush two pills in breast milk and instill into the mouth, administer four to five times daily until recovery.
Zhusha Wan (Cinnabar Pill)
Indications: Infants under three years old suffering from gastric closure and refusal to suckle.
Ingredients: Zhusha (Cinnabar), Niuhuang (Calculus Bovis), Shexiang (Moschus), Dingxiang (Clove), Zhigancao (Honey-fried Licorice Root), Renshen (Ginseng Root) each 1 fen; Xijiao (Rhino Horn), Huangqin (Scutellaria Root), Shigao (Gypsum Fibrosum, finely ground and washed with water), Wulingzhi (Trogopterus Dung) each 0.5 liang.
Grind all ingredients uniformly, mix with honey to form pills the size of mung beans. Administer three pills with warm water, four to five times daily.
Moxibustion Method
For infants who do not suckle and cry excessively within fourteen days after birth, this is often due to external wind pathogens invading the umbilicus and flowing into the Heart and Spleen. Moxibustion should be applied to Chengjiang (CV24), located in the depression below the lower lip border. Next, moxibustion should be applied to Jiache (ST6), located behind the mandible below the ear. Each point receives seven moxa cones, with cone sizes resembling sparrow droppings. This condition cannot be cured without moxibustion. If the infant exhibits stridor in the throat and difficulty swallowing milk, apply three moxa cones to Xuanji (an extra point located 1 cun below Tiantu, CV22, in the depression).
