壮热者，一向不止，由血气壅实，五脏生热，蒸熨于内则眠卧不安，精神恍忽，
熏发于外则表里俱热，烦躁喘粗，甚则发惊痫也。轻剂，火府丹、地黄煎。重剂、
双解饮、七宝散、大黄朴硝汤。
地黄煎 治小儿壮热烦心，眠卧不安。
生地黄（汁一升） 白沙蜜（三合） 酥（三合） 生门冬（汁三合）
上，重汤煮至成膏，每服数匙。
火府丹 治小儿壮热。
生地黄 木通 甘草 黄芩
上，水一钟，煎服。
金莲饮子 治小儿蕴积壮热，赤眼口疮，心烦躁闷，咽干多渴，潮热不止。
防风 甘草（炙） 连翘 柴胡（去芦） 山栀子（各半两）
上为末。每服二钱，用水六分，煎至三分，食后服。
黄龙汤 治发热不退，或寒热往来。
柴胡（五钱） 黄芩（炒） 甘草（炙。各二钱） 赤芍药（三钱）
上，每服一钱，姜枣水煎。
牛黄膏 治壮热，咽喉涎响，或不省人事，或左右手偏搐，或唇口眼鼻颤动，此热
涎内蓄，风邪外感也，宜急服之。
蝎尾（四十九枚） 巴豆肉（去油膜，一钱半） 梅花脑（半匙） 辰砂（研，二钱） 郁金（三钱，
皂角水煮） 牛黄（少许） 麝香（一匙）
上为末。每服一匙，蜜水调下。量儿虚实用之。
栀子仁汤 治阳毒壮热，百节疼痛，下后热不退者。
栀子仁（酒炒） 赤芍药 大青 知母（各一两） 升麻 黄芩（酒炒） 石膏（各二两）
柴胡（一两半） 甘草（五钱） 杏仁（二两，浸，去皮，麸炒微黄）
上，每服三钱，生姜三片，水煎服。
六物黄芩汤 治壮热腹大短气，往来寒热，饮食不化。
黄芩（酒炒） 大青 甘草（炙） 麦门冬（去心） 石膏（各半两） 桂（一钱）
上，每服一二钱，水煎服。
五物人参饮 治壮热咳嗽，心腹胀满。
人参 甘草（各半两） 麦门冬（去心） 生地黄（各一两半） 茅根（半握）
上，每服二三钱，水煎服。
壮热，是指发热持续不退。这是由于气血壅盛实邪，五脏产生内热，热气在体内蒸腾熏灼，导致睡眠和卧息不安稳，精神恍惚不定；热气熏蒸发散于体表内外，则表现为表里俱热，烦躁不安，呼吸急促粗重，严重时甚至引发惊风痫证。治疗轻症者，可用火府丹、地黄煎；治疗重症者，可用双解饮、七宝散、大黄朴硝汤。
地黄煎：治疗小儿壮热烦闷，睡眠卧息不安。
生地黄（取汁一升） 白沙蜜（三合） 酥油（三合） 生门冬（取汁三合）
以上药物，隔水加热熬煮至成为膏状，每次服用数匙。
火府丹：治疗小儿壮热。

以上药物，用水一钟，煎煮后服用。
金莲饮子：治疗小儿积热蕴结导致的壮热，伴有红眼、口疮，心中烦躁闷乱，咽干多渴，潮热不止。

以上药物研为细末。每次服用二钱，用水六分，煎煮至三分，饭后服用。
黄龙汤：治疗发热不退，或寒热往来交替。
柴胡（五钱） 黄芩（炒制） 甘草（炙烤。各二钱） 赤芍药（三钱）
以上药物，每次服用一钱，用生姜、大枣的水煎煮。
牛黄膏：治疗壮热，咽喉间有痰涎声响，或神志不清不省人事，或左右手偏侧抽搐，或嘴唇、口角、眼睛、鼻子颤动，这是热痰涎蓄积在内，外感风邪所致，应当立即服用此药。
蝎尾（四十九枚） 巴豆肉（去除油和膜，一钱半） 梅花脑（半匙） 辰砂（研磨，二钱） 郁金（三钱，用皂角水煮过） 牛黄（少许） 麝香（一匙）
以上药物研为细末。每次服用一匙，用蜂蜜水调服。根据患儿的虚实情况斟酌使用。
栀子仁汤：治疗阳毒壮热，全身关节疼痛，经过泻下治疗后热势仍不退者。
栀子仁（酒炒） 赤芍药 大青 知母（各一两） 升麻 黄芩（酒炒） 石膏（各二两）
柴胡（一两半） 甘草（五钱） 杏仁（二两，浸泡，去皮，麸皮炒至微黄）
以上药物，每次服用三钱，加生姜三片，用水煎煮服用。
六物黄芩汤：治疗壮热伴腹部胀大短气，寒热往来，饮食不消化。
黄芩（酒炒） 大青 甘草（炙烤） 麦门冬（去心） 石膏（各半两） 桂枝（一钱）
以上药物，每次服用一二钱，用水煎煮服用。
五物人参饮：治疗壮热咳嗽，心胸腹胀满。
人参 甘草（各半两） 麦门冬（去心） 生地黄（各一两半） 茅根（半握量）
以上药物，每次服用二三钱，用水煎煮服用。
Zhuangre (Intense Fever) refers to fever that persists continuously without abating. This condition arises from the stagnation and excess of Qi and Blood, generating internal heat within the Five Zang Organs. When this heat steams and scorches internally, it causes restlessness in sleep and lying down, along with a trance-like state of spirit and mind. When this heat fumigates and manifests externally on both the interior and exterior, it presents as intense heat throughout the body, irritability, restlessness, and coarse, rapid breathing. In severe cases, it may even trigger Jingfeng (Convulsions) or Xianzheng (Epilepsy). For mild cases, Huofu Dan (Fire Mansion Pill) or Dihuang Jian (Rehmannia Decoction) may be used. For severe cases, Shuangjie Yin (Double-Resolution Drink), Qibao San (Seven Treasures Powder), or Dahuang Puxiao Tang (Rhubarb and Mirabilite Decoction) are indicated.
Dihuang Jian (Rehmannia Decoction): Treats Zhuangre (Intense Fever) and vexation in children, accompanied by restlessness in sleep.
Sheng Dihuang (Fresh Radix Rehmanniae, juice one sheng) Baisha Mi (White Sand Honey, three he) Su (Butter/Ghee, three he) Sheng Mendong (Fresh Radix Asparagi, juice three he)
The above ingredients are cooked over a double boiler until they form a paste. Take several spoons per dose.
Huofu Dan (Fire Mansion Pill): Treats Zhuangre (Intense Fever) in children.
Sheng Dihuang (Fresh Radix Rehmanniae) Mutong (Herba Akebiae) Gancao (Radix Glycyrrhizae) Huangqin (Radix Scutellariae)
The above ingredients are decocted in one zhong of water and taken orally.
Jinlian Yinzi (Golden Lotus Drink): Treats Zhuangre (Intense Fever) caused by accumulated heat in children, presenting with red eyes, oral ulcers, mental vexation and oppression, dry throat with excessive thirst, and unceasing tidal fever.
Fangfeng (Radix Saposhnikoviae) Gancao (Radix Glycyrrhizae, honey-fried) Lianqiao (Fructus Forsythiae) Chaihu (Radix Bupleuri, remove rhizome) Shanzhizi (Fructus Gardeniae, each half liang)
The above ingredients are ground into powder. Take two qian per dose, decocted in six fen of water until reduced to three fen, taken after meals.
Huanglong Tang (Yellow Dragon Decoction): Treats persistent fever or alternating chills and fever.
Chaihu (Radix Bupleuri, five qian) Huangqin (Radix Scutellariae, stir-fried) Gancao (Radix Glycyrrhizae, honey-fried, each two qian) Chi Shaoyao (Radix Paeoniae Rubra, three qian)
The above ingredients: take one qian per dose, decocted in water with fresh ginger and jujube.
Niuhuang Gao (Calculus Bovis Paste): Treats Zhuangre (Intense Fever), rattling phlegm in the throat, loss of consciousness, hemiconvulsions of either hand, or trembling of the lips, mouth, eyes, and nose. This is due to the accumulation of hot phlegm internally and external contraction of Wind pathogen; it should be administered urgently.
Xiewei (Cauda Scoropionis, forty-nine pieces) Badou Rou (Semen Crotonis, de-oiled and de-membraned, one qian and half) Meihua Nao (Borneol, half spoon) Chensha (Cinnabaris, powdered, two qian) Yujin (Rhizoma Curcumae, three qian, boiled in soap pod water) Niuhuang (Calculus Bovis, a small amount) Shexiang (Moschus, one spoon)
The above ingredients are ground into powder. Take one spoon per dose, mixed with honey water. Adjust dosage according to the child's deficiency or excess condition.
Zhizi Ren Tang (Gardenia Seed Decoction): Treats Yangdu (Yang Toxin) causing Zhuangre (Intense Fever) and pain in all joints, where fever persists despite purgative treatment.
Zhizi Ren (Semen Gardeniae, wine-stir-fried) Chi Shaoyao (Radix Paeoniae Rubra) Daqing (Folium Isatidis) Zhimu (Rhizoma Anemarrhenae, each one liang) Shengma (Rhizoma Cimicifugae) Huangqin (Radix Scutellariae, wine-stir-fried) Shigao (Gypsum Fibrosum, each two liang)
Chaihu (Radix Bupleuri, one liang and half) Gancao (Radix Glycyrrhizae, five qian) Xingren (Semen Pruni Armeniacae, two qian, soaked, peeled, bran-stir-fried until slightly yellow)
The above ingredients: take three qian per dose, with three slices of fresh ginger, decocted in water and taken orally.
Liuyu Huangqin Tang (Six-Ingredient Scutellaria Decoction): Treats Zhuangre (Intense Fever) with abdominal distension and shortness of breath, alternating chills and fever, and indigestion.
Huangqin (Radix Scutellariae, wine-stir-fried) Daqing (Folium Isatidis) Gancao (Radix Glycyrrhizae, honey-fried) Maidong (Radix Ophiopogonis, remove heart) Shigao (Gypsum Fibrosum, each half liang) Guizhi (Ramulus Cinnamomi, one qian)
The above ingredients: take one or two qian per dose, decocted in water and taken orally.
Wuwu Renshen Yin (Five-Ingredient Ginseng Drink): Treats Zhuangre (Intense Fever) with cough, and chest/abdominal fullness.
Renshen (Radix Ginseng) Gancao (Radix Glycyrrhizae, each half liang) Maidong (Radix Ophiopogonis, remove heart) Sheng Dihuang (Fresh Radix Rehmanniae, each one liang and half) Maogen (Radix Imperatae, half handful)
The above ingredients: take two or three qian per dose, decocted in water and taken orally.
