[Haizang] When heat is accumulated in the meridians of infants and children, causing sores to appear on the head, face, and body, Si Jun Zi Tang (Four Gentlemen Decoction) can be used with added Gualou Gen (Trichosanthis Radix) and Jiegeng (Platycodonis Radix), each half a qian, decocted and taken orally.
[Tang Shi] For infants and children suffering from malignant sores, if the weather is mild, frequent bathing and changing of clothes should be provided. This method is called "Wai Xuan" (External Ventilation/Dispersion), and internal medication is not advisable at this time. If medication is unavoidable for the child, it constitutes a purgative method. If proper care is maintained, what disease could invade? Nursing should be regulated, avoiding overfeeding, so that the child's body becomes as robust as a medicinal tree; this is the principle of health preservation.
[Tang Shi] Formula for treating malignant sores: Frequent bathing ensures safety, and external dispersion prevents illness. In spring, use willow twigs and Jingjie (Schizonepetae Herba); in summer, use jujube leaves and huai zhi (Sophorae Tomentosae Ramulus); in autumn and winter, use Kushen (Sophorae Flavescentis Radix). All are decocted in water for bathing.
Jia Wei Qiang Huo San (Modified Notopterygium Powder): Treats infantile urticaria caused by the invasion of wind, cold, summer-heat, and dampness into the skin, accompanied by aversion to cold, fever, and generalized itching.
Ingredients: Qianghuo (Notopterygii Rhizoma et Radix), Qianhu (Peucedani Radix) (each 1 liang); Renshen (Ginseng Radix), Jiegeng (Platocodonis Radix), Fuling (Poria), Gancao (Licorice Root, honey-fried), Chuanxiong (Ligusticum Chuanxiong Rhizome), Zhike (Aurantii Fructus, bran-fried), Tianma (Gastrodiae Rhizoma) (each 0.5 liang); Chantui (Cicadae Periostracum, whiskers removed), Bohe (Menthae Haplocalycis Herba) (each 2 qian).
Preparation and Administration: Coarsely crush the herbs. Take 3 qian per dose, add one cup of water and three slices of fresh ginger, decoct until reduced to 0.6 cups, and take at any time.
Dang Gui Yin Zi (Angelica Decoction): Treats blood stasis in the heart vessels and internal accumulation of wind-heat in infants and children, manifesting as generalized sores and scabs on the skin, swelling, itching, or oozing pus.
Ingredients: Danggui (Angelica Sinensis Radix, radix removed), Chi Shaoyao (Paeoniae Radix Rubra), Chuanxiong (Ligusticum Chuanxiong Rhizome), Sheng Dihuang (Rehmanniae Radix Recens), Jili (Tribuli Fructus, stir-fried, thorns removed), Jingjie Sui (Schizonepetae Spica), Fangfeng (Saposhnikoviae Radix, radix removed) (each 1 liang); Heshouwu (Polygoni Multiflori Radix), Huangqi (Astragali Radix, radix removed), Gancao (Licorice Root, honey-fried) (each 0.5 liang).
Preparation and Administration: Coarsely crush the herbs. Take 4 qian per dose, add one cup of water and three slices of ginger, decoct until reduced to 0.6 cups, and take at any time.
Fetal toxin sores and scabs result from inheriting fetal heat, maternal dietary toxins, or emotional fire. Initially resembling dry tinea, they later ooze pus or form crusts in patches. Eruptions between the eyebrows or around the ears belong to the Hand Shaoyang Meridian. Eruptions on the limbs belong to the Spleen and Stomach Meridians. Eruptions on the hypochondrium belong to the Liver Meridian. Eruptions on the forehead belong to the Heart Meridian. Eruptions on the back of the head belong to the Bladder Meridian. Eruptions on the chin and cheeks belong to the Kidney Meridian. Treatment should follow the governing organs of each meridian, the balance of the five zang organs, and factors such as the wet nurse's diet of rich flavors or emotional anger. Do not arbitrarily use Huadu Wan (Detoxification Pill) or Xijiao Wan (Rhinoceros Horn Pill); damaging the primordial qi may lead to difficult-to-treat complications. One case involved a child with generalized sores cured by Niuhuang Jiedu Wan (Calculus Bovis Detoxification Pill), except for crusting, itching, and weeping on the head, indicating Kidney meridian deficiency heat, cured by Dihuang Wan (Rehmannia Pill) and Jiedu San (Detoxification Powder). Another case involved itching at the hairline, attributed to the mother's liver fire, cured by Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) plus Loulu (Euphorbiae Humifusae Herba), along with Niuhuang Jiedu Wan and Jiedu San. (Jiedu San is also known as Tuodu San). A third case involved spreading itching from the left ear hairline, attributed to liver-gallbladder heat toxicity, cured by Chaihu Qinggan San (Bupleurum Liver-Clearing Powder) taken by both mother and child. Relapse due to rich diet led to weeping pus and redness on the right cheek, indicating stomach heat; treated first with Qingwei San (Stomach-Clearing Powder), then Chaihu Qinggan San for liver fire, plus Lixiao San (Efficient Powder) and Niuhuang Jiedu Wan, curing both. Another case involved eyebrows spreading to limbs with weeping pus and alternating chills and fever, attributed to liver-spleen accumulated heat, cured by Qingwei San, Xiaochaihu Tang (Minor Bupleurum Decoction), and Lixiao San. Subsequent eyebrow recurrence with eye twitching and turbid urine was cured by Simei Fei'er Wan (Four-Ingredient Stomach-Nourishing Pill) and Jiwei Lushen Wan (Nine-Ingredient Aloe Pill). A child whose mother ignored emotional and dietary restrictions was cured by the mother taking Qingwei San and Xiaoyao San, and the child taking Niuhuang Jiedu Wan. Later, sweet foods caused eyebrow sores with itching, pain, and eye twitching, cured by Simei Fei'er Wan as the main formula, assisted by Jiawei Xiaoyao San and Qingwei San. Another case with severe hypochondriac involvement was cured by the child taking Simei Fei'er Wan and Lixiao San, and the mother taking Chaihu Zhizi San (Bupleurum and Gardenia Powder).
Niuhuang Jiedu Wan (Calculus Bovis Detoxification Pill): Treats fetal toxin boils and all types of sores.
Ingredients: Niuhuang (Calculus Bovis, 3 qian), Gancao (Licorice Root), Jinyinhua (Lonicerae Japonicae Flos, 1 liang), Cao Zihewche (Placenta Humanum, 5 qian).
Preparation and Administration: Grind into powder. Form pills with refined honey, dosing according to the child's age.
Lixiao San (Efficient Powder): Treats temporal sores, ear sores, and all types of sores and scabs.
Ingredients: Dingfen (Lead Carbonate Powder), Songxiang (Pini Resin), Huangbai (Phellodendri Cortex), Huanglian (Coptidis Rhizoma), Kufan (Alumen) (each 1 liang).
Preparation and Administration: Grind each into fine powder separately. Mix with clear sesame oil or candle oil for topical application.
Fuyao Jiedu San (Topical Detoxification Powder): Treats all toxic sores, wind-rash, itching, and pain.
Ingredients: Dahuang (Rhei Radix et Rhizoma), Huangbai (Phellodendri Cortex), Shanzhi (Gardeniae Fructus), Hanshuishi (Calcitum) (equal parts).
Preparation and Administration: Grind into powder. Mix with water for topical application. If broken with weeping pus, mix with Danggui Gao (Angelica Ointment) or clear candle oil for better effect.
Chaihu Zhizi San (treats fever), Simei Fei'er Wan (treats gan syndrome), Jiwei Lushen Wan (treats gan syndrome), Jinhuang San (treats bullous impetigo).
Heat-toxin sores and scabs result from early nursing, preference for sweet/greasy foods causing organ heat, maternal rich diet, or emotional fire. Differentiate by organ involvement and excess/deficiency patterns to regulate qi and blood. Liver excess heat: Chaihu Qinggan San; deficiency heat: Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill). Heart excess heat: Daochi San (Guide Out Redness Powder); deficiency heat: Buxin Tang (Heart-Nourishing Decoction). Spleen excess heat: Xiehuang San (Drain Yellow Powder); deficiency heat: Buzhong Tang (Middle-Supporting Decoction). Lung excess heat: Xiebai San (Drain White Powder); deficiency heat: Wuwei Yigong San (Five-Ingredient Extraordinary Assistant Powder). Kidney heat: Liuwei Dihuang Wan. Cold extremities indicate deficiency-cold; hot extremities indicate excess-heat. Deep, rapid, forceful pulse with thirst for cold drinks and constipation indicates interior pathogen, treat with internal purgation. Floating, rapid, forceful pulse with thirst for cold drinks indicates exterior pathogen, treat with dispersal. Floating, large, weak pulse with thirst for warm drinks and poor appetite indicates primordial qi deficiency fever; regulate spleen and stomach, avoid cold medicines to prevent further damage. Infant treatment focuses on the wet nurse. One case of hypochondriac sores with malaria-like chills/fever and frequent urination, attributed to liver fire, was cured by Chaihu Qinggan San followed by Jiawei Xiaoyao San. Relapse due to maternal liver fire was cured by Jiawei Xiaoyao San plus Bazhen Tang (Eight-Treasure Decoction) with Mudanpi (Moutan Cortex) and Zhizi (Gardeniae Fructus), taken by both. One case of abdominal sores with fever, bloody stool, sallow complexion, vomiting/diarrhea, cold extremities, and wiry-rapid right guan pulse indicated spleen deficiency with liver fire. Treated first with Wuwei Yigong San plus Shengma (Cimicifugae Rhizoma), Chaihu (Bupleuri Radix), Zhizi to benefit liver qi and clear liver fire, then Dihuang Wan to nourish kidney water and generate liver blood. One case of inner thigh sores with redness, fever, and variable facial color indicated kidney yin deficiency with hyperactive wood-fire. Treated first with Chaihu Zhizi San to clear liver-heart, then Dihuang Wan to tonify liver-kidney. One case of elbow sores with thirst for cold drinks and weak rapid right cun/guan pulses indicated stomach heat transmitting to lungs. Treated first with Xiehuang San and Xiebai San, then Wuwei Yigong San and Simei Fei'er Wan. One case of back/shoulder sores spreading due to rich diet indicated spleen blood injury from dietary heat, cured by Qingwei San and Xiehuang San, then Wuwei Yigong San to restore qi.
Various gan sores and scabs result from spleen-stomach deficiency, fluid depletion, and deficient fire, or maternal exposure to six excesses/seven emotions and improper lifestyle. Differentiate causes and meridians. Green face with chills/fever or white film over eyes indicates liver meridian syndrome. Red face with fever, thirst, palpitations indicates heart meridian syndrome. Sallow face with emaciation, thirst, diarrhea indicates spleen meridian syndrome. Pale face with cough or nasal sores indicates lung metal syndrome. Dark face with emaciation or preference for lying on damp ground indicates kidney meridian syndrome. Infant treatment focuses on the wet nurse. Failure to assess organ balance and excess/deficiency before using toxic-clearing methods often leads to fatal outcomes.

[Tian Shi] Treatment for sores/tinea: For eroding sores, apply Ku Hu San (Bitter Gourd Powder). For dry tinea, apply natural juice of Yangti Gen (Aconiti Radix preparation) mixed with Nifen (Calomelas). For wet tinea, apply Qingjin San (Green Gold Powder).
Ku Hu San: Treats progressive eroding sores in infants.
Ingredients: Ku Hu (Benincasa Fructus, 2 liang), She Tui (Bombyx Batryticatus, burnt to ash), Feng Fang (Vespa Nidus, lightly fried), Liang Shang Chen (Dust from roof beam, 1 he).
