There is a condition called "Red Silk Sore" (Hong Si Chuang). Although it is not Dan Zhen (a type of acute skin infection characterized by redness like cinnabar), its toxicity is the same. It mostly occurs on the joints of the middle fingers of both hands, and is particularly severe in males on the left side and females on the right side. Its presentation is a single blister, clear, transparent, bright, and lustrous, about the size of a small chicken's head. The base has dozens of tiny puncture-like holes. It is neither itchy nor painful, and generally causes no major harm. However, at the edge of the blister, there is a vein-like thread resembling a red silk strand, faintly visible under the skin. It spreads very rapidly along the arm upwards; if it passes the elbow, the condition becomes critical, and if it reaches the heart, it leads to death. For those with this condition, one should immediately use a needle to pierce and cut off the red silk strand at its leading edge, or apply earwax to seal the wound, or chew white plums to seal the wound, which will stop the spread of the red silk strand. In the Gengwu year of the Shaoxing era, Fang Zi, the magistrate of Wuwei, courtesy name Chuzai, who was a fellow Jinshi with my late father, died three days after his term ended while serving as an acting official returning home. Upon inquiry into the cause of his death, he stated that he had no other illness, only a small blister on his left middle finger joint. This was likely because he did not recognize it as Red Silk Sore. Additionally, Lady Yiren, wife of Inspector Wei, said: A neighbor's daughter suddenly developed a blister on her middle finger joint. The color was extremely clear, and the base was covered with fine puncture-like holes that were clearly countable. There was a strand of red silk beside it. The whole family laughed it off. Suddenly, an old woman came and saw it, exclaiming in alarm, "This is Red Silk Sore! It will take your life!" She urgently applied dozens of moxa cones on the blister and pierced the red silk strand at several points, thereby saving her life.
Chao Yuanfang stated: When children encounter wind-heat toxic factors, these interact with the blood and qi, forming stubborn nodules that arise in the neck region. If constrained by wind-cold, they fail to dissipate, forming Luoli (scrofula). Over time, they ulcerate, suppurate, and become sores. Dr. Tang suggested taking Qingliang Yinzi (Cooling Drink) and Shengma Tang (Cimicifuga Decoction), etc. Lianqiao Wan (Forsythia Pill) and Longdan Tang (Gentian Decoction) from *Qianjin Fang* are also suitable. [Zeng] The condition of Luoli was named "Jiu Lou" (Nine Leaks) by ancient sages. Investigating its etiology suggests it relates to heat, but its morphology varies, named merely according to appearance. Generally, it initially appears between the muscles of the neck. Before suppuration occurs, treating from the root to guide the branch can reduce the condition. Treatments include acupuncture, moxibustion, and topical applications, administered according to the specific cause. However, young children are frail and cannot withstand acupuncture and moxibustion; thus, treatment relies on oral medications and topical pastes. This disease often arises behind the ears and on both sides of the neck. Initially, there is only one nodule, which then multiplies, numbering over ten, winding around the neck like beads strung together. Each swells, suppurates, and ruptures. Mild cases can be cured, while severe ones are difficult to eradicate. If pus leaks from acupoints and remains unhealed for years, it is called "Lou Xiang" (Leaking Neck). Tracing back to the onset, it results from the Three Yang meridians contracting wind-heat, which clashes with blood and qi. Treatment involves Baiejie San (Hundred Resolutions Powder) combined with Danggui San (Angelica Sinensis Powder), decocted with water, ginger, green onion, and Jiaomao Dengxincao (Juncus effusus stem). Subsequently, Xuanshen Yin (Scrophularia Drink), Niubang Tang (Arctium Decoction), Mutong San (Akebia Powder), and Neixiao Wan (Internal Resolution Pill) are used to disperse heat and resolve toxicity. Externally, Hupi San (Oak Bark Powder) is used for washing, and Baiji San (Bletilla Powder) and Erxiang San (Two Fragrances Powder) for application, promoting qi and blood circulation until pus dries and fluids cease, leading to spontaneous recovery. One must avoid dry, toxic wild game meats to prevent recurrence.
[Xue] Congenital toxin Luoli is caused by stagnant fire and qi in the Liver and Gallbladder meridians. Since the Liver and Gallbladder meridians run along the sides of the body, if afflicted by moving Liver fire, lesions appear in the areas governed by these meridians. One must examine the cause and medicate accordingly. Some cases arise because the wet nurse is angry or suffers from blood deficiency with internal heat. In such cases, the wet nurse should be treated according to the cause. Harsh and drastic medicines should be avoided to prevent damaging the primordial qi. An infant presented with five nodules in the neck at birth. Examination revealed the mother was prone to anger, suffered from alternating chills and fever, occasional breast pain, and slight swelling in the hypochondrium, all indicative of Liver-Gallbladder meridian syndromes. Xiao Chaihu Tang (Minor Bupleurum Decoction) plus Danggui (Angelica sinensis) and Shaoyao (Peony) was prescribed first, resolving the chills and fever instantly. Then, Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) was given to the mother for over two months, resulting in the child's recovery. Another infant suffered from lesions around the ears due to the wet nurse's Liver meridian heat. Treating the mother with Jiawei Xiaoyao San led to the child's spontaneous recovery. A third infant had nodules below the neck and ears for three years. Long-term use of detoxifying agents worsened the lesions and depleted the primordial qi. Diagnosis showed the wet nurse's chronic depression and anger caused Liver-Spleen blood deficiency with heat. Jiawei Guipi Tang (Modified Restore the Spleen Decoction) was used as the main formula, assisted by Jiawei Xiaoyao San. As the mother's heat subsided, she fed the infant a few spoonfuls daily, leading to recovery after two months. Another infant had four nodules in the neck since birth. By age five, nodules around the ears resembled strings of beads. The primordial qi was severely deficient, with alternating chills and fever, and incomplete feeding. This indicated Qi stagnation in the Liver-Gallbladder meridians. Bazhen Tang (Eight Treasure Decoction), Xiaoyao San, and Er San (Two Powders) were given to a healthy adult woman. The infant drank her milk. After half a year, the infant's physique improved, and nodules gradually diminished. Further treatment with Dihuang Wan (Rehmannia Pill) and Xiaoyao San led to complete recovery. Another infant had three Luoli nodules in the neck at birth, spreading to the front of the ears. I stated this resulted from congenital Liver-Gallbladder issues. The mother's Liver-Gallbladder pulse was surging and rapid. I advised treating the mother, as mother and child share one body. The mother refused, taking Bixiao San (Highly Effective Powder) instead, resulting in vomiting and diarrhea, and death within a night.
Heat-toxin Luoli is a syndrome of wind-heat in the Hand-Foot Shaoyang and Foot-Jueyin meridians, or caused by Liver malnutrition and food accumulation. Lesions appear in the neck, axilla, or around the ears, sometimes like beads. Differentiate exterior-interior and excess-deficiency. If red and swollen, it is Liver meridian heat-toxin; treat with Renshen Baidu San (Ginseng Detoxification Powder). If painful and feeling blocked/heat, it is internal Liver fire; treat with Jiawei Xiao Chaihu Tang. If painless with yellow urine, it is Liver blood deficiency; treat with Liuwei Dihuang Wan (Six Ingredient Pill with Rehmannia). If hidden in the flesh with unchanged color, it is internal Liver malnutrition; treat with Jiawei Luohua Wan (Nine Ingredient Aloe Vera Pill). If pus forms but does not rupture, or ruptures but does not heal, it is Spleen qi deficiency; treat with Yiqi Yangrong Tang (Benefit Qi and Nourish Blood Decoction). For swelling and pain with chills, fever, and thirst, indicating excess pathogenic qi and deficient form qi, clear Liver fire and generate Liver blood. For hard swelling without rupture or non-healing ulcers, indicating deficiency of both pathogenic and form qi, tonify Kidney water and strengthen Spleen earth. If caused by the wet nurse's anger transmitting Liver fire, treat according to the cause. An infant with dripping pus and unresolved nodules, suffering from fever and aversion to cold, had Liver meridian qi-blood deficiency with heat. Treated mainly with Buyin Bazhen Tang (Nourish Yin Eight Treasure Decoction), intermittently with Qinggan Yirong Tang (Clear Liver and Benefit Blood Decoction), leading to recovery. Later, recurrent nodules with dark, scanty urine and afternoon fever/thirst were treated mainly with Shenzhu Chailing Tang, assisted by Liuwei Dihuang Wan material plus Chaihu (Bupleurum) and Shanzhi (Gardenia), and Simei Feier Wan (Four Ingredient Fattening Child Pill), leading to healing. A fifteen-year-old infant with fever, thirst, afternoon facial redness, and weak rapid pulse had yin deficiency with heat. Treated with Buyin Bazhen Tang for fifty doses, plus Renshen (Ginseng) for twenty doses, leading to rupture. However, thin pus and lack of muscle growth indicated Spleen qi deficiency. Treated mainly with Renshen, Danggui (Angelica), Baizhu (Atractylodes), assisted by Shaoyao (Peony), Shudihuang (Prepared Rehmannia), Maidong (Ophiopogon), and Wuweizi (Schisandra), restoring qi and blood. Then, one dose of Bixiao San was taken, expelling toxins and curing the disease. An infant treated with harsh purgatives developed chills, fever, and abdominal distension, indicating Liver-Spleen malnutrition. Treated initially with Wuyi Yigong San (Five Agents Extraordinary Merit Powder) plus Chaihu and Shengma (Cimicifuga), assisted by Jiawei Luohua Wan, gradually receding. Then, Simei Feier Wan and Wuyi Yigong San eliminated the condition. A nine-year-old infant with constant greenish complexion, hard non-suppurating nodules, and unchanged skin color was being treated with Liver-draining and phlegm-resolving methods. I advised regulating and tonifying Liver and Spleen. He refused, resulting in a surge of deficiency symptoms. Upon re-consultation, he still wanted Liver-draining. I explained: Green face indicates Liver deficiency revealing its true color; white face indicates Lung deficiency; rushing phlegm indicates Spleen deficiency failing to control; blinking eyes indicate Liver blood deficiency generating wind. The Classics state: "The Stomach is the root of the Five Zang organs." Save the Stomach qi first. Used Wuyi Yigong San plus Shengma and Chaihu. As primordial qi recovered, morning administration of Buzhong Yiqi Tang (Center-Benefiting Qi-Ascending Decoction) and evening administration of Wuyi Yigong San, assisted by Jiawei Luohua Wan, turned the complexion yellow and nodules shrank. Intermittent Dihuang Wan and Simei Feier Wan led to full recovery.

[Xue] Fright-induced nodules belong to wind-fire activity in Liver-Gallbladder meridians. Wood excess generates wind, heat transforms similarly. Symptoms include twitching and agitation, caused by wind-heat and blood dryness. Wind movement increases Liver fire; excessive fire consumes Liver blood; blood fails to nourish tendons, causing spasms and bead-like nodules. Neck sides are governed by Liver-Gallbladder meridians. Treatment should nourish Kidney water, clear Liver fire, nourish yin blood, and strengthen Spleen earth. Strong Kidney water clears Liver fire spontaneously; clear Liver fire generates yin blood spontaneously; generated yin blood calms ministerial fire spontaneously; calm fire prevents damage to primordial qi and fire attacking Earth. An infant aged one year had neck nodules and hyperextended arms, demanding detoxification. I suspected Liver blood dryness. Inquiry confirmed prior fright syndrome treated with Cinnabar. Prescribed Liuwei Dihuang Wan to nourish Liver blood; recovered after a few doses. An infant with lateral neck nodles, profuse phlegm, and convulsions took metallic aromatic-dry agents, causing limb spasms. This re-injured Liver blood, constituting acute fright. Treated with Jiawei Xiao Chaihu Tang plus Gouteng (Uncaria), Shanzhi, Chuanxiong, Danggui, and Liuwei Wan material plus Wuweizi, Maidong; recovered. An infant had neck nodules, fever, reduced appetite, and mild limb twitching during sleep after fright. This is Liver wood insulting Spleen earth. Treated with Wuyi Yigong San plus Chaihu, Shengma, Gouteng; recovered. After marriage, nodules appeared in legs/arms/wrists. Mistakenly taking qi-moving and blood-breaking herbs caused limb spasms and emaciation resembling tuberculosis. Upon my return to Beijing, used Dihuang Wan to generate Liver-Kidney blood, assisted by Buzhong Yiqi Tang to tonify Spleen-Lung qi; recovered. An infant had ear-neck nodules, profuse phlegm, teeth grinding, and shaking upon fright. Strictly taking aromatic-dry herbs led to slow fright and death.
