For pediatric acute convulsion (Jing Jing), moxa the Qian Ding (GV20, Frontal Vertex) acupoint once, three moxibustion cones. The location is one cun anterior to Baihui (GV20). If ineffective, moxa the area between the two eyebrows (Yintang, EX-HN3) and the Ren Zhong (GV26) point below the nose, using moxa cones the size of a wheat grain. For pediatric chronic convulsion (Man Jing), moxa the Chi Ze (LU5) acupoint, three cones per side, located in the transverse crease of the elbow near the arterial pulse, using moxa cones the size of a wheat grain. If convulsions occur during sleep, moxa the distal ends of the first and second toes of the foot, approximately the width of a leek leaf from the nail, one cone per site. If opisthotonos (body stiffening backward like a bow) and body rigidity occur, moxa three fen above the nasal bridge entering the hairline, three cones; then moxa the interspace below the Dazhui (GV14) vertebra, three cones. If startled during sleep with eyes unable to close, moxa three fen within the transverse crease at the posterior aspect of the flexed elbow, one cone.
[Qian Yi] Severe teeth grinding indicates onset of fright. Fixed gaze, cyanotic complexion, and body arching backward indicate development of fright. Yawning with a cyanotic complexion indicates wind-phlegm convulsion. Yawning with a yellowish complexion indicates spleen deficiency with fright. Red eyes combined with cyanosis indicate spasms.
In convulsive epilepsy, when boys have spasms, looking left without sound is normal, while looking right with sound is abnormal. When girls have spasms, looking right without sound is normal, while looking left with sound is abnormal. This is due to the principle of mutual overcoming among the Five Elements. There are also specific manifestations during attacks. (Mr. Jiegu stated: Boys belong to Wood, so looking left towards the Wood position produces no sound, while looking right towards the Metal position causes Metal and Wood to clash, producing sound. Girls belong to Metal, so looking right towards the Metal position produces no sound, while looking left towards the Wood position also causes clashing and sound.) The son of Official Li, aged three, had spasms from Mao hour to Si hour, looking right and crying loudly. Qian Yi observed and said this is a reverse pattern. Boys are Yang; normally looking left without sound is smooth/normal, while looking right with sound is reverse. This is because the left corresponds to Liver Wood and the right to Lung Metal. In reverse patterns, these two organs battle, Metal and Wood clash, resulting in sound. Treatment should reduce the excess and tonify the deficiency. Suppose a girl has spasms and looks left; this indicates Lung Metal overacting on Liver Wood, which cannot withstand it, hence crying. One should first reduce the Lung, then treat the Heart, and subsequently treat the Liver. If the disease occurs in Autumn (similarly at dusk), when Lung Metal is prosperous, one should heavily reduce the Lung. If the disease occurs in Spring (similarly in the morning), when Liver Wood is prosperous yet still cannot overcome Lung Metal, it indicates Lung is strong and Liver Fire is weak; one should tonify the Liver and Kidney and heavily reduce the Lung. If a boy has spasms and looks right, this indicates Liver Wood overcoming Lung Metal, causing crying; one should reduce the Liver and Heart. If the disease occurs in Spring and Summer (similarly at noon), when Liver and Heart are prosperous, one should heavily reduce the Liver. If the disease occurs in Autumn and Winter (similarly in the afternoon), when Lung Metal is prosperous yet still cannot overcome Liver Wood, it indicates Liver is strong and Lung is extremely deficient; one should tonify the Lung and heavily reduce the Liver. The reason for retroversion of the eyes is that the Liver governs the eyes. All spasms result from Wind-Heat battling internally. Wind belongs to the Liver, thus manifesting externally in the eyes. Since this case shows a boy exhibiting a girl's pattern, it is slightly easier to treat than a girl's case. First, reduce the Lung using Xie Fei Tang (Lung-Reducing Decoction). If after two days there is no restlessness, the disease is receding. Then use Di Huang Wan (Rehmannia Pill) to tonify the Kidney. After three doses, use Xie Qing Wan (Green-Reducing Pill) twice each to reduce the Heart and Liver. Recovery in five days. Furthermore, why not reduce the Lung if it is deficient? The answer is: Suppose a boy looks right, indicating Wood overcoming Metal, with Liver excess overcoming Lung. If only the Liver is reduced, and especially if the disease occurs in Spring and Summer when Metal Qi is extremely deficient, one must tonify the Lung and absolutely avoid reducing it. [Tang Shi] Generally, spasms where boys look left and girls look right are smooth/normal patterns, easy to treat; boys looking right and girls looking left are reverse patterns, difficult to treat. *Mai Jue Qi Meng* states: A rapid and urgent pulse in children indicates deficiency-fright. *Zhi Zhi* states: Floating, rapid, full, and tight pulses indicate acute convulsion. Deep, slow, scattered, and slack pulses indicate chronic convulsion. Cyanotic and purplish tiger-mouth vein patterns indicate wind-phlegm convulsion. Red indicates mild wind-heat. Crimson indicates severe wind-heat. Purple indicates fright-heat. Green indicates fright-stagnation. Half green and half purple indicates both fright-stagnation and wind-heat, primarily acute convulsion. Pale green mixed with purple, with veins extending and retracting, indicates chronic convulsion. Intermixed faint purple, green, or black threads, appearing but not fully emerging, indicate chronic Spleen-wind. Curving inward is a smooth/normal sign. Extending outward is a reverse/adverse sign.
