Treating cases where the smallpox pustules are full, but blood stasis causes unbearable pain.
Grind Bai Shao Yao (White Peony Root, *Paeonia lactiflora*, stir-fried with wine, 2 qian) into a fine powder, and take it mixed with wine.
In various diseases, itching indicates deficiency, while pain indicates excess; itching is suitable for tonification, and pain is suitable for purgation. This pain is caused by blood excess and stagnation, therefore Shaoyao (White Peony Root) is used to calm the blood, and wine is used to promote circulation.
Alternatively, apply ash from decaying grass externally, or apply ash from silkworm cocoons mixed with a small amount of Ku Fan (Alum) externally, or apply ash from white snail shells found on walls externally, or apply Ge Fen (Diatomite Powder) externally.
Sheng Ma (Cimicifugae Rhizoma), Ge Gen (Puerariae Lobatae Radix), Shao Yao (Paeoniae Radix Alba), Gan Cao (Glycyrrhizae Radix et Rhizoma), Sang Pi (Mori Cortex), Di Gu Pi (Lycii Cortex), Jie Geng (Platycodonis Radix), Niu Bang Zi (Arctii Fructus), Jing Jie (Schizonepetae Herba), Lian Qiao (Forsythiae Fructus).
Sha Zheng (Sha disease/Rash), also known as Zhen Zi (Rash), should not be treated merely based on symptoms, but rather by addressing the root cause. The root cause refers to the pathogenic heat in the Hand-Taiyin Lung Meridian and Foot-Yangming Stomach Meridian.
The dosage may be slightly reduced. If after taking the medicine the fever becomes more severe, it is due to the righteous qi expelling the toxin; the toxin will resolve naturally through profuse sweating.
This indicates that Sha toxin is not fully expressed, and one must subsequently guard against Gan accumulation (malnutrition syndrome in children). Treatment should focus on nourishing Yin, using Gan Lu Yin (Sweet Dew Decoction) as the primary formula (see the section on Moistening Dryness).
This has been verified effective repeatedly. (There is a type of infantile Sha Rash that can resolve in one day, without needing to wait for the traditional three or nine days.)
Because children's constitution is still weak and their Shen Qi (Spiritual Qi) is limited. If encountered by an incompetent physician who fails to assess deficiency and excess, and uses excessive diaphoretic methods, the harm will be great. In cases of severe Sha toxin with phlegm and wheezing, even if Shi Gao (Gypsum Fibrosum) and Huang Lian (Coptidis Rhizoma) are used, they are ineffective like water pouring on stone. One must understand that this is...
Use Lou Shuang (Trichosanthis Semen), Zhi Ke (Aurantii Fructus), Hua Fen (Trichosanthis Radix), Jin Fei Cao (Boehmeriae Caulis et Folium) to clear phlegm and fire. If accompanied by Qi deficiency, adding Ren Shen (Ginseng Radix et Rhizoma) yields immediate results.
Using Sheng Ma, Ge Gen, Huang Lian, Gan Cao. Then diarrhea stops spontaneously. Patients with Sha disease do not fear diarrhea; diarrhea allows the pathogenic heat of the Yangming meridian to exit.
Later onset of diarrhea, and pus/blood in stool. All are due to pathogenic heat sinking inward. Astringent and binding agents are strictly contraindicated; only ascending and dispersing methods are suitable.
If there are bean-sized [lesions]... If there is pus/blood in stool, add Hua Shi (Talcum) powder. It will surely heal.
(The formulas and syndromes for external diseases are extremely numerous and cannot all be recorded. Here, a few formulas are selected for reference. As for agents that clear heat, purge fire, detoxify, invigorate blood, and support the interior to expel pus...)
When Cold and Dampness contend, stagnation occurs and movement slows, representing insufficiency. When Heat and Fire contend, boiling occurs and movement accelerates, representing excess. When Qi is constrained by pathogens, body fluids become thick and sticky, forming Tan (Phlegm) or Yin (Fluid retention). After accumulating for a long time, they seep into the vessels, making the blood turbid; this is Yin stagnation blocking Yang. When Blood is constrained by pathogens, the collaterals are blocked, leading to either extravasation or coagulation. After accumulating for a long time, they seep out of the vessels, causing Qi to become chaotic; this is Yang stagnation blocking Yin. All diseases arise from this, not just carbuncles and ulcers.)
