Zhou Mengjue stated: In ancient tradition, there are nine types of heart pain. By following the names to investigate their realities, even the slightest details are difficult to confuse. The first is worm pain (Chong Tong). Generally, pain pulses are often hidden (Fu), but if they are instead surging and rapid (Hong Shu), it indicates roundworms (Hui Chong). These worms are named "Hui," about one cun long, with red heads and tails, residing in the epigastric region, sucking blood and essence. No harm to the heart is more tragic than this; using realgar (Xiong Huang), betel nut (Bing Lang), and alum (Bai Fan) made into pills to kill them will naturally relieve the pain. The second is zhu pain (Zhu Tong). Zhu refers to disease transmission from above downwards. It makes people feel heavy and silent, with a dull ache in the chest; severe cases may lead to closed doors (metaphor for critical condition or unconsciousness) without knowing the illness. The pulse may be suddenly short or long, rough or fine. Ordinary medicines cannot cure this; only Suhexiang Wan (Suhexiang Pill) or Awie Gao (Awie Plaster) might treat it. The third is wind pain (Feng Tong). Wind intensifies when meeting fire, and fire becomes more fierce with wind. When wind enters the heart, sudden pain occurs. The pulse is floating, tight, and rapid. Using white chrysanthemum (Bai Ju) and alum (Bai Fan) as chief herbs, Hou Shi Hei Feng San (Hou's Black Wind Powder) can be adopted. The fourth is palpitation pain (Ji Tong). Being startled by contact is called jing (startle); being startled without contact is called ji (palpitation). When palpitation progresses to pain, it is severe palpitation. The pulse is deficient and slippery; adding frankincense (Ruxiang) and myrrh (Moyao) as guides, Li Shi Yang Xin Tang (Li's Heart-Nourishing Decoction) should be used. The fifth is food stagnation pain (Shi Tong). Food entering the stomach remains stagnant and undigested, attacking and causing pain; the pulse is short and rough. Pingwei San (Pingwei Powder) is indeed the correct formula. The sixth is fluid retention pain (Yin Tong). Fluids entering the stomach attack irregularly, agitating and causing pain; the pulse is soggy and slow. Wuling San (Wuling Powder) is truly a diuretic agent. The seventh is cold pain (Leng Tong). Cold qi invading the heart palace results in a pulse that is either slow or knotted. Evodia (Wuyu), Sichuan pepper (Chuanjiao), amomum (Sharen), and costus root (Muxiang) relieve pain; texts provide ample evidence. The eighth is heat pain (Re Tong). Fire qi stagnates in the chest and diaphragm; the pulse is either rapid or hurried. Rehmannia (Sheng Di), gardenia (Zhizi), coptis (Huanglian), and melia bark (Kulian) have clear records for relieving pain. The ninth is intermittent pain (Qulai Tong). Meridian qi flows throughout the body; obstruction causes pain, which stops after passing the obstruction but recurs upon returning to it. If qi and blood are abundant, what obstruction exists? Pulse diagnosis is unnecessary; tonification suffices.
Although all are heart and abdominal pains, worm pain is ranked first because worms cause the most cruel injury; zhu pain second due to its insidious nature; wind pain third for its speed; palpitation pain fourth as it lies between injurious and non-injurious; food stagnation pain fifth and sixth as food rarely causes severe injury; cold and heat pains seventh and eighth as they are constant; and intermittent pain ninth as everyone knows how to treat it. One can imagine the careful consideration古人 (ancients) gave to this arrangement. However, while there are nine names, the principles originate from the Classics. Worm pain corresponds to the Classic's mention of roundworm heart/abdominal pain. Zhu pain corresponds to conditions like Feishi (Flying Corpse) and Dunshi (Escaping Corpse). Wind pain corresponds to Liver Heart Pain (Gan Xintong) in the Classics. Palpitation pain relates to the Hand Shaoyin Heart Meridian originating in the heart. Food and fluid pain relate to the Foot Taiyin Spleen Meridian branch ascending through the diaphragm to the heart. Cold pain relates to cold qi lodging in the back and injecting into the heart. Heat pain relates to cold qi lodging in meridians clashing with heat. Intermittent pain corresponds to the Classic's description of blocked qi circulation. None of these are Zhenxin Tong (True Heart Pain). True Heart Pain presents with cold limbs reaching the joints, death within half a day (morning onset leads to evening death, evening onset leads to morning death). Most formulas passed down by physicians address cold pain. Materia Medica notes occasionally mention treating heat pain. Yet, for cold and heat pain—the most superficial and easiest to distinguish—books still debate extensively. How much more so for pain that is deeply hidden and obscure? Understanding the subtle meanings of the *Ling Shu* and *Su Wen* constitutes medical scholarship; mastering the application of Materia Medica principles reveals the balance of medication.

Tu Weisheng said: Ancients stated that pain above the navel is heart pain, and below is abdominal pain. In reality, pain above the navel is also stomach pain, unrelated to true heart pain. Because the heart is located highest, above the diaphragm, while the stomach is below the chest. (Chen Feixia stated that heart/abdominal pain is divided into Upper, Middle, and Lower Jiao. Upper Jiao pain is above the diaphragm, which is actually Wei Wan pain. This shows ignorance of the Upper Jiao, the diaphragm's location, and the Wei Wan). However, when Wei Wan pain is severe, rebellious qi can disturb the heart, causing pain. Thus, the *Nan Jing* states: There is Jue Heart Pain, but no True Heart Pain. True Heart Pain involves cold limbs to the joints and death within half a day. Zhou Mengjue did not clarify the boundaries of pain, only distinguishing the types. His prescription based on symptoms shows great insight. Regarding his food stagnation treatment, formulas can be modified; applying this logic opens many methods. Chen Feixia stated: Pressable pain is deficiency; unpressable pain is excess. Chronic pain is mostly deficiency; acute pain is mostly excess. Pain relieved by eating is deficiency; worsened by eating is excess. Vague, mild pain is deficiency; severe, localized pain is excess. Also, cold pain presents with blue or pale face, dark complexion, and blue lips/nails in severe cases. Heat pain presents with red face, high fever, restlessness, and hot palms/soles. Food stagnation pain presents with warm breath, yellow face, white lips, lethargy, poor appetite, and sour-smelling stool. Worm pain presents with white face, red lips, surging pulses, salivation/clear fluid during pain, and abdominal masses. However, I treat all these except worm pain with acupuncture for tonification and sedation. Mild cases heal with needling Weizhong, Twelve Hand Ying-Spring points, and Zusanli. Severe cases heal with Zhongwan and Fenshui. For severe heart pain, needling Juque alone is curative. The art of tonification/sedation is hard to master; distinguishing cold/heat is crucial. Regrettably, Chinese acupuncture has declined recently, and physicians lack direction.
